We have studied the frequency and structure of cardiac rhythm (CR) disturbances in the women presenting with abdominal obesity (AO) in the postmenopausal period. The single-step study involved 210 postmenopausal women (median age 57 years) who were examined by 24-hour ECG monitoring, measurement of arterial pressure, body mass index, and the relationship between waist and hip circumferences. CR disturbances were revealed in all 159 women with abdominal obesity, ischemia in 16.9%, supraventricular extrasystole in 89.3%, ventricular extrasystole (VE) in 58.5%, high-grade extrasystole in 23.9%, p

OBJECTIVE: To examine the hypothesis that the anatomic equivalents of the fast and slow pathways identified in patients with atrioventricular (AV) nodal tachycardia may be universal and represent the principal sites of atrial input into the normal compact AV node.
METHODS: 15 patients undergoing complete AV junction ablation for paroxysmal atrial fibrillation were studied. Radiofrequency energy was delivered first in the anterior "fast pathway" position so as to prolong the atrium to bundle of His (AH) interval by over 50% of baseline (protocol 1) and then to the "slow pathway" position using…

Progressive cardiac conduction disease (PCCD) is a common pediatric heart conduction disorder. It is an autosomal inheritance of rare mutations, which leads to familial cases of PCCD. In these cases, the His-Purkinje system's conductive capacity is progressively deranged, involving either right or left bundle branch block. Also, QRS complexes display widening is an important characteristic that culminates in complete AV block, syncope, and sudden death. Mutations in TRPM4 gene that encodes for transient receptor potential melastatin 4 have recently been reported to cause familial cases of PCCD

BACKGROUND: The efficacy of ibutilide in conversion of atrial fibrillation and flutter (AFL) has been demonstrated. However, its electrophysiological effects on human atria have not been fully studied.
METHODS AND RESULTS: Twelve patients with typical AFL were studied. Electrograms were recorded from the anterolateral right atrium, His bundle position, and coronary sinus. During AFL, we measured the conduction time, CTi, through the isthmus between the tricuspid annulus and eustachian ridge and the conduction time, CTni, through the remainder of the right atrium. Resetting response curves and

INTRODUCTION: Histologic studies of autopsy specimens described the coexistence of two distinct AV nodes (so-called "Minckeberg sling" or "twin AV nodes") in specific congenital heart defects; however, the clinical electrophysiologic (EP) characteristics of twin AV nodes have not been characterized in detail.
METHODS AND RESULTS: Since April 1993, a total of seven patients with complex congenital heart disease presented with AV reciprocating tachycardia suspected to be mediated by twin AV nodes. A common anatomic finding was AV discordance ([S,L,L] or [I,D,D]) with a malaligned complete AV ca

AF may appear as an irregular broad complex tachycardia (BCT) if atrioventricular conduction occurs via an accessory pathway (preexcited AF) or if bundle branch block (BBB), preexistent or rate related, exists in the His-Purkinje system (BBB-AF). While BBB-AF is relatively benign, preexcited AF may herald sudden cardiac death. Hence it is important that the two conditions can be reliably distinguished. Yet, there is no preexistent algorithms for this purpose. Griffith et al. previously proposed a simple algorithm for a similar problem, that of distinguishing the two differential diagnoses for

Atrioventricular (AV) nodal reentrant tachycardia is a common cause of supraventricular tachycardia. The present study describes catheter ablation of this form of tachycardia in 23 patients using direct current shocks. The aim of ablation was to abolish conduction through the retrograde pathway while preserving the anterograde conduction. All patients had symptomatic, drug resistant, slow-fast variety of dual atrioventricular nodal reentrant tachycardia. Using the retrograde atrial activation in the His bundle catheter as the reference, the optimal ablation site was selected by positioning an

Cardiac conduction defects (CD) are common in patients with aortic valve diseases. Several studies have suggested that the occurrence of complete heart block at the time of valve replacement is related with preoperative conduction defects and with other factors like calcium deposits, aortic gradient or poor left ventricular function. We evaluated 36 patients undergoing isolated aortic valve replacement in 29 of them, combined mitroaortic in four, isolated mitral valve replacement in two and subvalvular myectomy in one. In all of them an electrophysiologic study was done at the postoperative pe

Between 1973 and 1983, we operated on 160 patients with Wolff-Parkinson-White syndrome at Kanazawa University Hospital; 126 had Wolff-Parkinson-White syndrome alone, and 34 had combined cardiac diseases. Of the 160 patients, 140 were completely cured; postoperatively, they had no delta wave on electrocardiograms and no tachycardia attacks. In 11 patients, the delta wave reappeared an average of 56 days after operation. They, and three other patients with concealed Wolff-Parkinson-White syndrome, experienced several postoperative episodes of tachycardia. However, in 12 of the 14 patients with p

In this study we sought to determine whether characteristics of ventricular-induced atrial preexcitation during reciprocating tachycardia could help differentiate atrioventricular (AV) nodal reentry from orthodromic AV reentry using an accessory pathway and to identify the site of accessory pathways in patients with Wolff-Parkinson-White syndrome. Fifty-five patients with orthodromic AV reciprocating tachycardia and 22 patients with AV nodal reentrant tachycardia were studied with standard electrophysiologic techniques. There were 24 left free wall, 23 posterior septal, seven anterior septal,

BACKGROUND: Patients with complex congenital heart defects, such as tricuspid atresia, often require palliative surgical interventions like the Fontan procedure to optimize systemic and pulmonary circulation. While these surgeries improve survival rates, they are associated with long-term complications, including arrhythmias, due to significant anatomical and electrophysiological alterations. Among these, atrioventricular nodal reentrant tachycardia (AVNRT) is relatively uncommon but poses substantial challenges in diagnosis and management.
CASE SUMMARY: This case report describes a 36-year-o

In a clinical context highly suggestive of acute coronary syndrome (ACS), the presence of a bifascicular block (BFB) should be considered a potential ST-elevation myocardial infarction (STEMI) equivalent. A right bundle branch block (RBBB) may obscure accurate assessment of the ST segment, potentially masking ischemic changes. Therefore, in patients with suspected ACS, BFB (RBBB and left anterior fascicular block), whether new or pre-existing, should be managed as a high-risk finding and treated according to STEMI protocols if clinical suspicion is strong. We report the case of a man in his 70

Congenital hypertrophy of intrinsic foot muscles is a rare condition. We report an unusual case of a 24-year-old male with a painless swelling at the plantar and medial aspect of the right foot, which was present since birth. The significant size of deformity and discomfort in wearing shoes were major concerns. MRI revealed expansion of multiple intrinsic foot muscles, which are abductor hallucis, flexor digitorum brevis, and quadratus plantae. We used shear wave elastography (SWE) as an imaging technique besides MRI and ultrasonography, which has not been used or published previously. Muscle

Mitochondrial disorder (MtD) is usually a multisystem disease due to impaired mitochondrial energy production. Severe hypokalemia resulting in muscle weakness and rhabdomyolysis has not been reported as a phenotypic feature of MtD. Here we describe a 60-year-old male patient who developed myalgias followed by generalized muscle weakness a few days before admission. Symptoms were attributed to severe hypokalemia that occurred after the patient had discontinued spironolactone, a competitive antagonist of the aldosterone receptor, four months earlier on his own judgment. Spironolactone was given

BACKGROUND: Recognition of the presence and role of decremental fibers during wide QRS tachycardia requires carefully executed intracardiac studies.
OBJECTIVE: This study sought to determine the value of the atrioventricular (AV) conduction time during pre-excited tachycardia to differentiate a fast from a decrementally conducting accessory pathway (AP).
METHODS: Fifty-one patients with 56 pre-excited tachycardias were included in the study: Group I: 27 patients with 31 antidromic tachycardia (ADT) using an atriofascicular pathway, Group II: 2 patients with pre-excited tachycardia due to bys

AIMS: Left bundle branch block (LBBB) is the most common conduction disorder after transcatheter aortic valve replacement (TAVR) with an increased risk of atrioventricular (AV) block. The aim of the current study was to identify non-invasive predictors for infranodal conduction delay in patients with LBBB.
METHODS: We analyzed consecutive patients undergoing TAVR with pre-existing or new-onset LBBB between August 2014 and August 2020. His ventricular (HV) interval measurement was performed on day 1 after TAVR. Baseline, procedural, as well as surface and intracardiac electrocardiographic para

BACKGROUND Eccrine porocarcinoma (EPC) was first described in 1963 as an epidermotropic eccrine carcinoma. Fifty years later, its etiology remains poorly understood. The infrequent nature of this disease merits further inquiry into its etiology, presentation, and standards of management. Furthermore, the propensity for metastasis, which may be as high as 31% on presentation, increases the importance of investigating this rare disease. CASE REPORT The patient was a 63-year-old mechanic who presented with the lesion as a chronic wound following a chemical exposure. The lesion involved the ulnar

BACKGROUND: Linear microwave ablation has been shown to be effective for treatment of atrial fibrillation during open-heart surgery by producing transmural lesions in the atrium to isolate the pulmonary veins. However, the safety and efficacy of percutaneous, transcatheter, linear microwave ablation for atrial arrhythmias, while demonstrated in animal models, is unknown in humans. Therefore, we studied the safety and efficacy of linear microwave ablation of the cavotricuspid isthmus (CTI) in humans with typical atrial flutter, utilizing a 2-cm long microwave antenna mounted on a steerable 9-Fr

BACKGROUND Immune checkpoint inhibitors (ICIs) have transformed the treatment of advanced malignancies but can cause life-threatening immune-related adverse events. Myocarditis, myositis, and myasthenia gravis (MMM) overlap syndrome is a rare, highly morbid complication with high mortality. Most cases develop early in therapy, sometimes after a single dose. CASE REPORT A 73-year-old man with resected stage IIIC malignant melanoma presented 4 weeks after his first dose of adjuvant nivolumab with progressive weakness, gait instability, dyspnea, and dark-colored urine. Workup revealed markedly el
