Ultrasonic Doppler assessment of hemodynamics in superficial temporal artery--middle cerebral artery anastomosis.

The pre-, intra-, and postoperative hemodynamics were examined by a newly developed ultrasonic Doppler flowmeter in 20 patients undergoing 24 STA-MCA anastomoses. During the operations, no reverse flow was detected in the cortical branch of the middle cerebral artery. After anastomosis, prominent flow changes in direction and in volume were evident in the cortical branches and the flow pattern of the STA changed from an external carotid type preoperatively to an internal carotid type in well functioning bypasses. The results of postoperative studies were not always in good agreement with those

Mixed Reality-Assisted Surgical Planning of Flow-Augmentation Extracranial-Intracranial Bypass Surgery: A Technical Note.

OBJECTIVE: Precise choice of adequate donor and recipient arteries for an extracranial-intracranial (EC-IC) bypass remains a challenge. Herein, authors present the use of a novel three-dimensional mixed reality (MxR) system for preoperative planning of flow-augmentation EC-IC bypass. METHODS: All elective flow-augmentation EC-IC bypass cases from June 2022 to March 2023 were prepared with MxR and matched with a retrospective cohort of bypass procedures prepared without MxR. Perioperative patient data, holographic preparation time, intraoperative bypass flow, and postoperative patency were doc

Long-Term Outcome of Moyamoya Disease.

Moyamoya disease (MMD) is considered to run a progressive course, but little is known about its long-term outcomes. We conducted a structured phone interview to assess outcomes in a mixed cohort of conservatively and surgically treated patients a median of 9 years and up to 27 years after a diagnosis of MMD. We found that 60% of patients had a favorable outcome (modified Rankin Scale score <3), with only 16% of adult patients working full time. We conclude that MMD considerably impacts daily life in a substantial number of those affected, with only a minority able to provide for themselves.

Effect of Modified Encephalo-Duro-Arterial-Synangiosis on Periventricular Anastomosis in Pediatric Patients with Moyamoya Disease.

OBJECTIVE: Periventricular anastomosis (PA) is a risk factor for hemorrhage in moyamoya disease. In pediatric moyamoya disease, the onset of hemorrhage after adulthood worsens its long-term prognosis. At our institution, modified encephalo-duro-arterial-synangiosis (EDAS) is the first-line treatment for pediatric cases. Herein, we examined the outcome of modified EDAS and its effect on the development of PA. METHODS: A total of 21 patients who underwent revascularization for pediatric moyamoya disease between March 2010 and December 2022 were included. Image analysis was performed on 36 hemis

Long-term results and neuropathy of internal carotid artery aneurysms treated with high-flow bypass using saphenous vein graft and parent artery occlusion.

OBJECTIVES: Giant internal carotid artery aneurysms require treatment owing to the risk of rupture, cranial nerve palsy in the cavernous sinus, and optic nerve symptoms. Among treatment options for internal carotid artery aneurysms, we compared the results of flow diverter stent with those of high-flow bypass. MATERIALS AND METHODS: A total of 45 consecutive patients with large or giant internal carotid artery aneurysms underwent high-flow bypass using saphenous vein graft and double assist superficial temporal artery-middle cerebral artery bypass and proximal ligation of the internal carotid

Adult Moyamoya Disease and Moyamoya Syndrome: What Is New?

BACKGROUND: Recent advances are in the genetics, diagnosis, pathophysiology, and management of moyamoya disease (MMD), and moyamoya syndrome (MMS), a term used to describe moyamoya-like vasculopathy associated with various systemic diseases or conditions. SUMMARY: Ring finger protein (RNF213) has been reported to be a susceptibility gene not only for MMD but also for atherosclerotic intracranial arterial stenosis and ischemic stroke attributable to large artery atherosclerosis. The latest guidelines by the Research Committee on MMD of the Japanese Ministry of Health, Labor, and Welfare, remov

True first-pass effect in basilar artery occlusions: First-pass complete reperfusion improves clinical outcome in stroke thrombectomy patients.

BACKGROUND: Complete reperfusion (mTICI 3) in anterior circulation ischemic stroke patients after a single mechanical thrombectomy (MT) pass has been identified as a predictor of favorable outcome (modified Rankin Score 0-2) and defined as true first-pass effect recently. This effect has not yet been demonstrated in posterior circulation ischemic stroke. We hypothesized a true first-pass effect for the subgroup of acute basilar artery occlusions (BAO). METHODS: Consecutive patients with acute thromboembolic occlusions in the posterior circulation, treated between 2010 and 2017, were screened

Revascularization Evaluation in Adult-Onset Moyamoya Disease after Bypass Surgery: Superselective Arterial Spin Labeling Perfusion MRI Compared with Digital Subtraction Angiography.

Background A superselective (SS) arterial spin labeling (ASL) MRI technique can be used to monitor the revascularization area as a supplementary or alternative modality to digital subtraction angiography (DSA), with the advantage of being noninvasive. Purpose To evaluate whether SS-ASL perfusion MRI could be used to visualize the revascularization area after combined direct and indirect bypass surgery in adults with moyamoya disease compared with DSA. Materials and Methods Patients diagnosed with moyamoya disease who underwent DSA and SS-ASL 6 months after surgery between June 2017 and Novembe

Clinical outcomes after revascularization for pediatric moyamoya disease and syndrome: A single-center series.

Moyamoya is a progressive cerebrovascular arteriopathy that affects children of any age. The goal of this study was to determine imaging and clinical outcomes as well as complication rates in a pediatric cohort undergoing either a combined direct/indirect or indirect-only revascularization approach. Patients with moyamoya disease or syndrome ≤ 18 years of age at the time of initial surgery were identified, and clinical data were collected retrospectively. Over a 12-year period, 26 patients underwent revascularization procedures on 49 hemispheres with a median follow-up of 2.6 years from surger

The Dilator-Dotter Technique: A Modified Method of Rapid Internal Carotid Artery Revascularization in Acute Ischemic Stroke.

BACKGROUND AND PURPOSE: Acute ischemic stroke due to tandem occlusive lesions involves high-grade ICA stenosis or occlusion with a distal intracranial occlusion. Several approaches and devices exist in the treatment of tandem occlusions; however, a consensus on the optimal technique does not exist. The Dotter technique is a method of catheter-based angioplasty to recanalize cervical ICA occlusions. We present a modified dilator-Dotter technique, which involves a polyethylene, tapered inner dilator to initially cross the occlusion, followed by guide catheterization and subsequent intracranial t

Mechanical thrombectomy of M2 occlusions with distal access catheters using ADAPT.

BACKGROUND AND PURPOSE: The direct aspiration first pass technique (ADAPT) using distal access catheters (DAC) has proven to be an effective and safe endovascular treatment strategy of acute ischemic stroke with large vessel occlusions (LVO). However, data about direct aspiration using DAC in M2 segment occlusions is limited. We assess the safety and efficacy of DACs in acute M2 occlusions using ADAPT with large bore (5 French /6 French) aspiration catheters as the primary method for endovascular recanalization. MATERIALS AND METHODS: From January 2017 to July 2018, 52 patients with an acute

Internal maxillary artery bypass for the treatment of complex middle cerebral artery aneurysms.

OBJECTIVEThe rapid innovation of the endovascular armamentarium results in a decreased number of indications for a classic surgical approach. However, a middle cerebral artery (MCA) aneurysm remains the best example of one for which results have favored microsurgery over endovascular intervention. In this study, the authors aimed to evaluate the experience and efficacy regarding surgical outcomes after applying internal maxillary artery (IMA) bypass for complex MCA aneurysms (CMCAAs).METHODSAll IMA bypasses performed between January 2010 and July 2018 in a single-center, single-surgeon practic

Donor Selection in Flow Replacement Bypass Surgery for Cerebral Aneurysms: Quantitative Analysis of Long-term Native Donor Flow Sufficiency.

BACKGROUND: Graft selection in extracranial-intracranial bypass surgery for cerebral aneurysms has traditionally been based on clinical impression and operator preference. However, decision making can be optimized with a donor selection algorithm based on intraoperative flow data. OBJECTIVE: To present long-term follow-up and quantitative assessment of flow sufficiency for native donors selected in this manner. METHODS: Patients with bypass for anterior circulation intracranial aneurysms using only a native donor (superficial temporal artery) selected on the basis of an intraoperative flow a

Development of a de novo arteriovenous malformation after bilateral revascularization surgery in a child with moyamoya disease.

The development of a de novo arteriovenous malformation (AVM) in patients with moyamoya disease is extremely rare. A 14-year-old girl developed an AVM in the right occipital lobe during the 4-year postoperative period following successful bilateral revascularization surgeries. She suffered a transient ischemic attack with hemodynamic compromise of the bilateral hemispheres at the age of 10 years. Results of an initial examination by 1.5-T MRI and MR angiography satisfied the diagnostic criteria of moyamoya disease but failed to detect any vascular malformation. Bilateral direct and indirect re

Cognitive function of patients with adult moyamoya disease.

BACKGROUND: Neurocognitive impairment is one of several unsolved social issues faced by patients with moyamoya disease. Although efforts have been made to investigate cognitive function using neuropsychologic tasks, generalizability has been limited. Here, in a preliminary study, we used structured neuropsychologic tasks to establish a standardized neuropsychologic assessment for adult moyamoya patients with and without difficulty in social independence. METHODS: Ten patients with neuroradiologically confirmed adult moyamoya disease (3 male, 7 female) participated. Half of all subjects did no

Endovascular therapeutic strategies in ruptured intracranial aneurysms.

The aim of the present study was to evaluate endovascular techniques used currently which were not available at the time of ISAT inclusion period, such as balloon remodelling and flow-divertion, in order to assess whether these new technologies have improved the endovascular approach outcomes. We present a review of articles, published in major journals, with the aim to evaluate the efficacy and the safety of coiling with balloon remodelling for the treatment of ruptured aneurysms in comparison to coiling performed without such coadjutant techniques. Furthermore, we reviewed publications repor
