[Cyclocryotherapy. Experimental studies of the breakdown of the blood-aqueous barrier and analysis of a long term follow-up study (author's transl)].

We report on experimental results concerning the breakdown of the blood-aqueous barrier following cyclocryotherapy (CTh). The various factors (tip of the cryoprobe, simple or repeated application, pretreatment with prostaglandin-synthetase inhibitors and with phenylephrine, grade of pigmentation and preoperative inflammation of the anterior uvea) were studied, which influence the immediate increase in intraocular pressure (IOP) and the rise in protein concentrations in aqueous humour after the procedure. Early and late histological findings after CTh. were described. Fifty-two patients (60 eye

Ichthyosis hystrix (epidermal nevus syndrome) and Coats' disease.

A 4-year-old girl had the ichthyosis hystrix variant of the epidermal nevus syndrome with ocular fundus manifestations of Coats' disease. Her hearing was impaired because of serous otitis media and her visual acuity decreased because of hard yellow exudate in the macula as a result of decompensation and plasma leakage from the peripheral retinal vascular anomalies. Treatment of the vascular malformations with cryotherapy resulted in a reabsorption of the retinal edema and hard yellow exudate.

Acute Procedural Results of Pulsed Field Cryoablation for Persistent Atrial Fibrillation: Multicenter First-in-Human PARALELL Trial.

INTRODUCTION: Pulsed Field Cryoablation (PFCA) is a dual-energy cardiac ablation modality consisting of short-duration ultra-low temperature cryoablation (ULTC) followed immediately by pulsed field ablation (PFA) delivered from the same catheter. It is hypothesized that PFCA may improve contact stability during PFA, while maintaining lesion depth and effectiveness of ULTC. METHODS: PARALELL is a first-in-human multicenter study evaluating safety and effectiveness of a novel PFCA catheter and system in patients with persistent atrial fibrillation (PsAF) using the combination of pulmonary vein

Atrial fibrillation burden and symptom, quality of life, and healthcare resource utilization after cryoballoon ablation in persistent atrial fibrillation.

AIMS: To investigate the relationship between continuously monitored atrial fibrillation (AF) burden after cryoballoon ablation (CBA) and improvements in AF-related symptoms, quality of life (QoL), and AF-related healthcare resource utilization (HCRU) in early persistent AF. METHODS AND RESULTS: This secondary analysis of the multicentre COOL-PER trial included patients with early persistent AF who underwent CBA and continuous monitoring via an implantable loop recorder. Post-CBA AF burden was defined as the percentage of time in AF between 9- and 12-month after CBA, categorized as <0.1%, 0.1

A Prospective Study of Safety and the Incremental Diagnostic Value of Transbronchial Cryobiopsy Incorporated into Robotic-Assisted Bronchoscopy in a Cancer Population.

BACKGROUND: Shape-sensing robotic-assisted bronchoscopy (ssRAB) combined with intraoperative imaging optimizes tool-lesion relationship during the sampling of focal pulmonary lesions. Nevertheless, confirmation of tool-in-lesion status is not equivalent to diagnostic sampling. This gap may be bridged by the addition of transbronchial cryobiopsy (TBCB) to traditional transbronchial needle aspiration (TBNA) and forceps biopsy (TBFB); however, the performance of TBCB, specifically with the 1.7-mm cryoprobe, has raised significant safety concerns. OBJECTIVE: Evaluate the safety of TBCB using the

Using computed tomography atrial myocardial thickness maps in cryoballoon pulmonary vein isolation: the UTMOST AF II randomized clinical trial.

AIMS: Whether adjusting the duration of ablation based on left atrial wall thickness (LAWT) provides extra benefits for pulmonary vein (PV) isolation for atrial fibrillation (AF) is uncertain. We studied the safety and efficacy of tailored cryoballoon PV isolation (CB-PVI) based on LAWT for paroxysmal AF. METHODS AND RESULTS: Two hundred seventy-seven patients with paroxysmal AF refractory to anti-arrhythmic drug were randomized 1:1 to either LAWT-guided CB-PVI (n = 135) and empirical CB-PVI (n = 142). Empirical CB-PVI was performed using a 28 mm cryoballoon with recommended application for 2

Clinical Outcome of Cryoballoon Ablation for Long-Standing Persistent Atrial Fibrillation: From the Korean Cryballoon Ablation Registry.

INTRODUCTION: Pulmonary vein isolation (PVI) by cryoballoon ablation (CBA) is considered an effective primary strategy for early persistent atrial fibrillation (AF). However, data regarding CBA for long-standing persistent AF (PeAF) are limited. We evaluated the efficacy and safety of CBA for long-standing PeAF compared to PeAF. METHODS: The study included 1484 patients with non-paroxysmal AF from Korean CBA registry data with follow-up of > 12 months after de novo CBA. The primary outcome was recurrence of atrial tachyarrhythmias (ATs) of ≥ 30-s after a 3-month blanking period. RESULTS: A t

Enhancing the Appearance of the "Six-Pack" Muscles Using Cryolipolysis: A Safe and Effective Method.

Many individuals desire a defined and athletic body with "six-pack" abdominal muscles, but even with a strict regimen of diet and exercise, this result is unattainable for many. Cryolipolysis is a noninvasive method of reducing the subcutaneous adipose tissues lying above the rectus abdominis muscles, enhancing the six-pack appearance. Eleven nonobese adults with an average body mass index of 22.5 kg/m2 were enrolled in this study. All subjects were treated with the CoolSculpting cryolipolysis system, with most of them undergoing two rounds of treatment. Improvement in appearance of the abdomi

Impact of the ablation technique on release of the neuronal injury marker S100B during pulmonary vein isolation.

AIMS: S100B, a well-known damage-associated molecular pattern protein is released acutely by central and peripheral nerves and upon concomitant denervation in pulmonary vein isolation (PVI). We aimed to investigate whether the ablation technique used for PVI impacts S100B release in patients with paroxysmal atrial fibrillation (AF). METHODS AND RESULTS: The study population consisted of 73 consecutive patients (age: 62.7 ± 10.9 years, 54.8% males) undergoing first-time PVI with either radiofrequency (RF; n = 30) or cryoballoon (CB; n = 43) for paroxysmal AF. Impact of pulmonary vein isolation on mechanisms sustaining persistent atrial fibrillation: Predicting the acute response.

BACKGROUND: Noninvasive mapping identifies potential drivers (PDs) in atrial fibrillation (AF). We analyzed the impact of pulmonary vein isolation (PVI) on PDs and whether baseline PD pattern predicted termination of AF. METHODS: Patients with persistent AF less than 2 years underwent electrocardiographic imaging mapping before and after cryoballoon PVI. We recorded the number of PD occurrences, characteristics (rotational wavefronts ≥ 1.5 revolutions or focal activations), and distribution using an 18-segment atrial model. RESULTS: Of 100 patients recruited, PVI terminated AF in 15 patients

Ultrasound-Guided Lateral Femoral Cutaneous Nerve Cryoneurolysis for Analgesia in Patients With Burns.

Autologous skin grafting from the thigh is frequently required for treatment of burns and is associated with intense pain at the donor site. Local anesthetic-based (LA) nerve blocks of the lateral femoral cutaneous nerve (LFCN) have been demonstrated to provide analgesia when the graft is taken from the lateral thigh. However, the duration of these single injection blocks has been reported to average only 9 hours, whereas the pain from the procedure lasts days or weeks. Continuous LA nerve blocks can also be used to provide analgesia during serial debridement of burns, although this requires p
