Extrapulmonary involvement associated with Mycoplasma pneumoniae infection.

Mycoplasma pneumoniae infection usually presents with upper and lower respiratory tract infection. Extrapulmonary involvement is not uncommon, however. We report two cases of predominantly extrapulmonary manifestations of Mycoplasma pneumoniae infection without significant pulmonary involvement. Both cases were diagnosed by serology. These cases illustrate the diversity of clinical presentations of Mycoplasma pneumoniae infection. Clinicians should maintain a high index of suspicion.

Smallpox vaccination: a review, part II. Adverse events.

Smallpox vaccination of health care workers, military personnel, and some first responders has begun in the United States in 2002-2003 as one aspect of biopreparedness. Full understanding of the spectrum of adverse events and of their cause, frequency, identification, prevention, and treatment is imperative. This article describes known and suspected adverse events occurring after smallpox vaccination.

Disseminated vaccinia false alarm.

We report the case of a 6-year-old girl who presented in December 2002 with fever and a vesicular rash. Initially she was diagnosed with Stevens-Johnson syndrome. The differential diagnosis was expanded, however, when an exposure to a person who was believed to be vaccinated recently against smallpox was revealed. We describe the sequence of events that ensued and the workup for a possible case of disseminated virus.

Two men with toxic shock syndrome presenting with targetoid and spotty skin rashes.

Two previously healthy men who presented with hypotension, constitutional symptoms, and targetoid and discrete spotty erythematous plaques were diagnosed with toxic shock syndrome based on histopathological findings. Specifically, their biopsies revealed necrotic keratinocytes, neutrophils in the epidermis, and neutrophils surrounding dilated superficial vessels. In one case, the diagnosis of toxic shock syndrome was confirmed with rising titers to toxic shock syndrome toxin-1. Both patients recovered with supportive care and clindamycin administration. We suggest that patients with fever, hyp

Recurrent annular erythema in a case of seronegative Sjögren's syndrome.

We describe a case of Sjögren's syndrome who repeatedly developed annular erythema on her extremities. Her anti-nuclear antibody, anti-SSA/Ro antibody, and anti-SSB/La antibody were all negative. Characteristics of the annular erythema included a tendency to appear on the extremities especially in summer, spontaneous regression after 1-2 weeks, and residual slight pigmentation. The histological findings revealed dermal perivascular lymphocytic infiltration admixed with some neutrophils. Slight exsudative changes were found in the upper dermis. There were no epidermal changes. This case suggest

[Skin and hair].

Data deriving from comprehensive hospital monitoring systems suggest that drug-induced skin effects occur in 2-5% of patients receiving any drug medication. Exanthematous (maculopapular) reaction (75%) and urticaria with/without angioedema (30%) are the most frequent of all cutaneous reactions to drugs. The incidence of cutaneous reactions relates to the quantity of the drugs which is prescribed and consumed worldwide. Thus penicillin, sulfonamides and nonsteroidal antiinflammatory drugs show the highest rate of cutaneous side effects. Drug reactions may be classified as either predictable (e.

Erythema gyratum perstans: association with a familial neurologic disease.

Two members of the same family with erythema gyratum perstans and hypertrophic neuritis are reported. The dermatosis could be an expression of localization of neuritis to nerva vasorum with abnormal neurovascular response of cutaneous small vessels to normal stimuli with active erythema followed by cyanosis.

[Atypical herpetic manifestations].

Herpetic lesions may be readily identified by modern virological techniques. In immunocompromised patients, the herpetic lesions are often atypical and even completely different from conventional forms; in these patients, the course of herpes virus infections may be life-threatening. Therefore, early diagnosis of herpetic infections is essential in order to begin with acyclovir treatment in time.
