Custom 3D Printed Jigs in Salvage Reverse Shoulder Arthroplasty for Failed Four-Part Proximal Humerus Fracture Fixation: A Case Report.

INTRODUCTION: Complications of open reduction and internal fixation (ORIF) in four-part proximal humerus fractures (PHFs) include non-union, malunion, avascular necrosis of humeral head, and glenoid defect due to implant failure. Reverse total shoulder arthroplasty is a salvage procedure for cases of failed fixation. In cases with significant abnormal glenoid anatomy, custom-made patient-specific 3D printed jigs play a major role in pre-operative planning and accurate positioning of the glenoid component, thereby improving the final outcome. We report a case of salvage reverse shoulder arthrop…

Autologous Osteochondral Mosaicplasty for Centrally and Laterally Located, Advanced Capitellar Osteochondritis Dissecans in Teenage Athletes: Clinical Outcomes, Radiography, and Magnetic Resonance Imaging Findings.

BACKGROUND: Autologous osteochondral mosaicplasty (ie, mosaicplasty) results in satisfactory clinical outcomes and reliable return to play for patients with large or unstable lesions due to osteochondritis dissecans (OCD) of the humeral capitellum. However, the association between the healing of the reconstructed cartilage and clinical outcomes remains unclear. PURPOSE: To evaluate the efficacy of mosaicplasty in teenage athletes through use of clinical scores and imaging. The secondary purpose was to compare the clinical outcomes with images of centrally and laterally located lesions. STUDY

Examination of concomitant glenohumeral pathologies in patients treated arthroscopically for calcific tendinitis of the shoulder and implications for routine diagnostic joint exploration.

BACKGROUND: Glenohumeral exploration is routinely performed during arthroscopic removal of rotator cuff calcifications in patients with calc … as low. Most frequently, glenohumeral articular cartilage was either entirely intact (ICRS grade 0 (humeral head/glenoid): 46%/48%) or showed very mild degenerative changes (ICRS grade 1: 30%/26%). Two patients (1.3%) required intraarticular surgical treatment due to a SLAP lesion type III (n = 1) and an intraarticular rupture of CD (n = 1). CONCLUSIONS: Rout …

Value of true whole-body FDG-PET/CT scanning protocol in oncology: optimization of its use based on primary diagnosis.

BACKGROUND: No standardized field of view (FOV) currently exists for whole-body (WB) positron emission tomography/computed tomography (PET/CT). Limited WB PET/CT FOV can exclude portions of the head, upper, and lower extremities, because there is little perceived clinical benefit to be gained from imaging these areas. PURPOSE: To determine how often utilizing WB PET/CT changes the clinical stage and management compared to each of the limited WB FOVs used for PET/CT. MATERIAL AND METHODS: We retrospectively identified 556 oncologic patients (804 PET/CT studies) who underwent staging or restag

Functional and clinical outcome of total shoulder arthroplasty with oversized glenoid.

BACKGROUND: The Epoca-Reconstruction-(Reco)®-Glenoid has been developed to treat patients with cuff-tear-arthropathy. The glenoid component of this system has a hemispheric shape that canopies the humeral head. This design is believed to provide a stable fulcrum and restore normal deltoid function. The purpose of this study was to analyse strengths and disadvantages of the Epoca-Reco®-Glenoid in cuff-tear-arthropathy patients. Changes in functional outcome using Constant-Murley-Scoring(CMS), CMS sub-scoring parameters and radiological outcome were analysed. For this purpose, a classification f…

The intrarater and interrater reliability of glenoid version and glenohumeral subluxation measurements in neonatal brachial plexus palsy.

BACKGROUND: Progressive and disabling glenohumeral dysplasia commonly occurs as a secondary deformity in children with neonatal brachial plexus palsy (NBPP). A number of methods for quantifying glenohumeral dysplasia are currently in use; however, the most commonly reported quantitative measures have yet to be validated. The present study assesses the intrarater and interrater reliability of the glenoid version angle (GVA) and percent of the humeral head anterior to the scapular line (PHHA) measurements on axial magnetic resonance images. METHODS: Axial magnetic resonance images of the should…

Anterior translation at the glenohumeral joint: a cause of axillary artery compression?

BACKGROUND: Lesions to the axillary artery are reported in overhead athletes. One speculated cause is repeated transient compression by the humeral head due to excessive laxity at the glenohumeral joint. HYPOTHESES: (1) Anterior translation of the glenohumeral joint alters axillary artery diameter and blood flow characteristics, and (2) individuals demonstrating inducible axillary artery compression with the arm in an overhead position (as indicated by diameter and blood flow characteristics) will demonstrate greater magnitudes of anterior translation. STUDY DESIGN: Descriptive laboratory st…

[Differential therapy for the rheumatoid shoulder].

Despite frequent involvement, the rheumatoid shoulder is neglected in operative treatment of the upper extremities. The slow course of omarthritis, the compensation mechanism of scapulothoracic motion and neighbouring joints as well as dominating disabilities of the lower extremities and the rheumatoid hand are possible explanations. The pattern of destruction of the rheumatoid shoulder is characterized by progressive joint and soft tissue deterioration. Soft tissue involvement determines the course of the shoulder joint. The subacromial space is a common and early site for rheumatoid involvem

Treatment of proximal humeral fractures with reverse shoulder arthroplasty in elderly patients.

BACKGROUND: Proximal humeral fractures in four or even only three parts, with metaphyseal hinge distances of <8 mm, represent a serious and widely debated problem. Reduction is complex and plating is often instable, especially in elderly patients. Failures, sometimes involving necrosis of the head, are frequent. Hemiarthroplasty has long been used for 3- or 4-part complex fractures, even in young patients, although often with sub-optimal results, due to reabsorption of tuberosities. This complication has partly been overcome with reverse shoulder prostheses which, although more invasive than p

Arthroscopic Superior Capsular Reconstruction With Knotless Double-Row Dermal Allograft and Margin Convergence Augmentation.

BACKGROUND: Superior capsular reconstruction (SCR) is an effective treatment option for rotator cuff injury. A variety of techniques and grafts can be used, and no clear method of graft fixation has been established. INDICATIONS: SCR is indicated for the treatment of irreparable rotator cuff tears refractory to conservative measures, as was observed in this 58-year-old male patient. This procedure reduces superior translation of the humeral head and strengthens the superior capsule of the shoulder. TECHNIQUE DESCRIPTION: The patient was positioned in the beach-chair position. Three knotless …

[Fundamentals, possibilities and limitations of sonography of osteofibrous tunnels in the shoulder area. 1].

The first part of this report includes basic morphological as well as morphometric data concerning osteo-fibrous pathways within the shoulder region as determined by investigating a large number of anatomical specimens (286 macerated scapulae, 122 cadaver shoulders). Some parts of these passages, either inaccessible by means of ultrasonography or already sufficiently dealt with in the relevant literature, are not taken into consideration. This applies especially to the so called subacromial space. As to the shape of the scapular notch, five different patterns were found. The spinoglenoid notch
