Dementia, gait disturbance, and urinary incontinence in a patient with pulmonary sarcoidosis.

Hydrocephalus is an uncommon presentation of neurosarcoidosis. We discuss the case of a 67-year-old woman with a prior diagnosis of hypothyroidism, systemic hypertension, and lung sarcoidosis who presented with a 1-month history of progressive impairment of consciousness leading to prostration and loss of sphincter control. At admission, patient was febrile with altered speech and without focalization. Laboratory results showed leucocytosis and mild anaemia. Cerebrospinal fluid (CSF) analysis was characterized by mild elevated protein level, increased cell count, normal glucose and adenosine d

Use of magnetic resonance imaging to measure intracranial cerebrospinal fluid volume.

Magnetic resonance imaging was used to measure intracranial extraventricular and ventricular cerebrospinal fluid (CSF) volume. In 10 normal subjects lateral ventricular and extraventricular intracranial CSF volumes were 25.3 +/- 4.6 ml (mean +/- SD) and 97.6 +/- 6.6 ml, respectively (total 122.8 +/- 38.7). These volumes were measured in 4 patients and the results were: 11.0 ml ventricular volume, 68.7 ml total cranial CSF in the patient with benign intracranial hypertension; 606.6 ml ventricular, 174.1 ml total in the patient with hydrocephalus due to a blocked ventriculo-peritoneal (V-P) shun

Introduction of a Novel, Continuous, Noninvasive Estimation of Intracranial Pressure and Cerebral Perfusion Pressure Based on Tympanic Membrane Temperature.

OBJECTIVE: Hydrocephalus is a common but potentially life-threatening condition. However, valve malfunction makes further diagnosis difficult. Thus, we tried to develop a noninvasive method to detect the hydrocephalus intracranial pressure (ICP) during routine follow-up. METHODS: In group I, the patient was recruited because a spinal tap test was necessary for either disease diagnosis or treatment. In group II, patients were diagnosed with high ICP hydrocephalus and received shunt surgery. The tympanic membrane temperatures (TMTs) were recorded and plotted against the spinal tap pressure (STP

Resolved posterior fossa neurocysticercosis mimicking a fourth ventricular tumour and cervical cyst, with a postoperative course complicated by very low-pressure hydrocephalus.

A female in her late 40s presented with progressively worsening headaches, nausea, vomiting and gait imbalance and was ultimately found to have a fourth ventricular mass and cervical cyst. Initially presumed to be a neoplastic process, histopathology revealed longstanding neurocysticercosis and reactive changes. The patient underwent suboccipital craniotomy, C1 laminectomy and external ventricular drain placement. Postoperative complications included persistent very low-pressure hydrocephalus (VLPH), venous sinus thrombosis and fluctuating mental status requiring prolonged active cerebrospinal

Cerebrospinal fluid shunt dynamics in patients with idiopathic adult hydrocephalus syndrome.

The objective was to assess CSF dynamics of different shunt constructions in patients with adult hydrocephalus syndrome and correlate these findings to clinical outcome, neuroradiology, and the specifications of the shunts provided by the manufacturer. Thirty four patients with idiopathic adult hydrocephalus (normal pressure hydrocephalus) syndrome were included in a prospective, consecutive case series. A differential pressure valve (Cordis Hakim standard system) was used in 28 patients and a variable resistance valve (Cordis Orbis-Sigma) in six. A constant pressure infusion method was used;

Bilateral vocal cord palsy after ventricular drainage in a child.

Vocal cord palsies are caused by high intracranial pressure and normally improve with treatment. Our case report implies that stridor after drainage of a hydrocephalus, in susceptible patients, can be a result of worsening of vocal cord palsies.

Obstructive hydrocephalus from venous sinus thrombosis.

BACKGROUND: Cerebral venous sinus thrombosis (CVST) is a rare form of stroke. Although increased intracranial pressure is a well-described complication of CVST, hydrocephalus has rarely been reported. CASE: We present the case of a 49-year-old woman who presented with one week of headache, vomiting, and progressive neurologic deterioration leading to coma. Head CT and MRI identified extensive CVST and accompanying hydrocephalus. She was treated with systemic anticoagulation as well as mechanical and pharmacologic endovascular thrombolysis. An external ventricular drain (EVD) was not inserted

Ventricular dilation following intraventricular hemorrhage in the premature infant.

Periventricular/intraventricular hemorrhage occurs commonly in the premature newborn. Recent studies indicate an incidence of 35-45%. Following PVH/IVH, the likelihood of developing hydrocephalus is related to the severity of the hemorrhagic lesion. Ventricular dilation may be due to an obliterative arachnoiditis affecting principally the posterior fossa or, less commonly, due to obstruction of flow of cerebrospinal fluid within the ventricular system by clots or other debris. With moderate to severe hemorrhagic lesions, ventricular dilation may occur at the time of PVH/IVH. More commonly, pro

Hydrocephalus: Molecular and Neuroimaging Biomarkers in Diagnosis and Management.

Hydrocephalus is a complex neurological condition marked by abnormal cerebrospinal fluid (CSF) accumulation, often leading to elevated intracranial pressure and structural brain damage. Despite advances in surgical treatment, diagnostic precision and prognosis remain challenging, especially in idiopathic normal pressure hydrocephalus (iNPH). This narrative review aims to synthesize the current knowledge regarding molecular and neuroimaging biomarkers that hold diagnostic, prognostic, and therapeutic significance in hydrocephalus. A comprehensive literature search was conducted across PubMed, S

Revision to an adjustable non-siphon control valve in low pressure hydrocephalus: therapeutic siphoning and a new perspective on NPH: series of 3 cases and review of the literature.

INTRODUCTION: High intracranial compliance states requiring negative pressure drainage, otherwise known as low-pressure hydrocephalus syndromes, are rare conditions. The use of siphoning, enabled by revision to an adjustable shunt without an anti-siphon device, has been largely unexplored in low-pressure hydrocephalus. METHODS: Three patients with presumed normal pressure hydrocephalus (NPH) presented with unresolved symptoms, including urinary incontinence, disturbed gait, and cognitive dysfunction. Each was inadequately treated despite confirmed functioning Strata II valves (with built-in s

A simplified pressure adjustment clinical pathway for programmable valves in NPH patients.

OBJECTIVE: The goal of this study is to provide a clinical pathway for shunt valve adjustment in the treatment of normal pressure hydrocephalus (NPH) patients. PATIENT AND METHODS: The authors conducted a single-center retrospective study of 102 patients (mean age 74 years, 66 men, 36 women) diagnosed with NPH. In all cases, a Medtronic Strata Adjustable Pressure valve set initially at 1.5 was implanted. Outcome was based on the clinical status of the patient at the last contact point with the senior author. Patients were adjusted with reductions or increases of 0.5 per follow-up visit to ach

CSF and ECF glutamate concentrations in head injured patients.

Excitatory Amino Acids (EAAs) release has been considered to be neurotoxic in traumatic brain injury patients. Microdialysis samples of extracellular space (ECS) and high glutamate concentrations in cerebrospinal fluid (CSF) following Traumatic Brain Injury (TBI) have been documented. The objective of this study was to determine the correlation between EAA release in ECS and CSF in focal and diffuse injury. Head injury patients (GCS < or = 8, n = 16) admitted to Medical College of Virginia Hospital were instrumented for microdialysis collection of ECS samples. CSF samples were collected throug

The pulsatility curve-the relationship between mean intracranial pressure and pulsation amplitude.

The amplitude of cardiac-related pulsations in intracranial pressure has recently been suggested as useful for selecting patients for shunt surgery in hydrocephalus. To better understand how shunting affects these pulsations, we aim to model the relationship between mean pressure and pulsation amplitude in a wide range, including low pressures typically found after shunt surgery. Twenty-five patients with probable idiopathic normal pressure hydrocephalus were examined with lumbar constant pressure infusion investigations including drainage of cerebrospinal fluid. Mean pressure and pulsation am

Senescent changes in cerebrospinal fluid circulatory physiology and their role in the pathogenesis of normal-tension glaucoma.

PURPOSE: To evaluate the evidence supporting a role for senescent changes in cerebrospinal fluid (CSF) circulatory physiology in the pathogenesis of normal-tension glaucoma (NTG). DESIGN: Literature review and personal perspective of the authors. METHODS: Analysis of selected articles in the peer-reviewed literature with interpretation and perspective. RESULTS: Recent studies have reported that intracranial pressure is lower in patients with NTG when compared with patients with primary open-angle glaucoma and nonglaucomatous control subjects. It has been suggested that a low intracranial pr

Distinctive hydrocephalus-like phenotype in NOTCH2NLC-related neuronal intranuclear inclusion disease: clinicopathological features and therapeutic implications.

NOTCH2NLC-related neuronal intranuclear inclusion disease (NIID) is a progressive neurodegenerative disorder with marked clinical heterogeneity and an increasingly recognized phenotypic spectrum. Although hydrocephalus-like presentations, i.e. ventriculomegaly with concomitant with cognition, motor, and/or bladder dysfunctions, have been observed in NIID sporadically, their prevalence, clinicopathological characteristics, and potential clinical implications remain poorly understood. This retrospective cohort study systematically characterized this newly recognized hydrocephalus-like phenotype.

Choroid plexus papilloma in early infants.

Four infants with choroid plexus papillomas which were successfully treated with surgery are described. All patients showed the clinical signs of increased intracranial pressure and hydrocephalus within 8 weeks after birth. The tumors were in the lateral ventricles and histologically three of them were benign papillomas and other one was a malignant papilloma. Computerized tomography scan was useful for the diagnosis of both of the tumor and the hydrocephalus that was caused by the overproduction of cerebrospinal fluid and/or the obstruction of it's pathway. The tumor stain on the angiograms w

Radionuclide Cisternography Revealed Neck Cyst Formed by Cerebrospinal Fluid Leakage After Cervical Spine Surgery.

A 71-year-old man presented with nausea, vomiting, headache, and neck swelling two months after cervical spine surgery. Initial brain computed tomography and magnetic resonance imaging revealed ventricular dilation, suggestive of hydrocephalus. However, radionuclide cisternography subsequently demonstrated cerebrospinal fluid (CSF) leakage into a prevertebral cyst, rather than normal pressure hydrocephalus, thereby confirming communication between the prevertebral cyst and the spinal canal. These findings suggest that the patient's postoperative symptoms were likely attributable to CSF leakage

Episodic ventriculomegaly due to hypernatremia mimicking shunt malfunction: case report.

Patients with shunted hydrocephalus presenting with altered mental status and ventriculomegaly are generally considered to be in shunt failure requiring surgical treatment. The authors describe a case of shunted hydrocephalus secondary to a disseminated neuroectodermal tumor in a pediatric patient in whom rapid fluctuations in sodium levels due to diabetes insipidus repeatedly led to significant changes in ventricle size, with invasively confirmed normal shunt function and low intracranial pressure. This clinical picture exactly mimics shunt malfunction, requires urgent nonsurgical therapy, an
