A better statistical method of predicting postsurgery soft tissue response in Class II patients.

OBJECTIVE: To propose a better statistical method of predicting postsurgery soft tissue response in Class II patients. MATERIALS AND METHODS: The subjects comprise 80 patients who had undergone surgical correction of severe Class II malocclusions. Using 228 predictor and 64 soft tissue response variables, we applied two multivariate methods of forming prediction equations, the conventional ordinary least squares (OLS) method and the partial least squares (PLS) method. After fitting the equation, the bias and a mean absolute prediction error were calculated. To evaluate the predictive performa

Limitations in the surgical treatment of skeletal Class II deep bite malocclusions.

The present article illustrates a treatment option for managing the skeletal Class II malocclusion complicated by a deep bite and short face in an adult and emphasizes the limitations of such an approach and its potential complications. The patient had an inadequate tooth display upon smiling and a deep lower curve of Spee. Treatment consisted of a double jaw surgery with a maxillary three-piece LeFort to expand, downgraft, and advance the maxilla and a bilateral sagittal split osteotomy to advance the mandible. Treatment was staged so that the leveling of the mandibular arch was completed pos

[Symphyseal distraction: a simplified procedure].

INTRODUCTION: Severe mandibular incisor crowding with bone insufficiency is usually treated by dental extractions and orthodontic management. Symphyseal distraction was proposed to avoid tooth extraction. This technique is still considered as invasive, complex and unsafe. We evaluated symphyseal distraction to clarify its indications, and to suggest a simplified surgical protocol. PATIENTS AND METHODS: Six patients were treated by symphyseal distraction: three with a bone-anchored device and three with a dental anchored one. Pre and postoperative inter-canine measurements and occlusal stabili

Soft tissue facial morphometry in subjects with Moebius syndrome.

Moebius syndrome is a congenital facial palsy associated with the impairment of ocular abduction. The three-dimensional characteristics of the facial soft tissues of 12 male and 14 female subjects [3-52 yr of age (mean age + standard deviation: 17 + 14 yr)] were measured using a non-invasive, computerized system; facial volumes, areas, angles, and distances were computed and compared with those obtained in reference subjects of the same age and gender. When compared with reference subjects, patients with Moebius syndrome had a more prominent and hyperdivergent face in the sagittal plane, a sma

Comparison of relative mandibular growth vectors with high-resolution 3-dimensional imaging.

INTRODUCTION: The mandibular rami and their endochondrally growing condyles develop in many directions relative to the variable anatomic patterns of the nasomaxilla and middle cranial fossae during growth and response to orthopedic treatment. METHODS: High-resolution magnetic resonance images were used to compare 3-dimensional (3D) growth vectors of skeletal displacement and bone remodeling in 25 untreated subjects with Class II malocclusions, 28 subjects with Class II malocclusions who were treated with Fränkel appliance therapy, and 25 subjects with normal occlusions. Marked differences wer

CBCT analysis of incisor movement and alveolar bone changes in class two malocclusion treatment with premolar extraction using clear aligner: a retrospective study.

OBJECTIVES: This retrospective study evaluates alveolar bone remodeling patterns and their association with incisor displacement in adults undergoing clear aligner therapy with premolar extractions for Class II malocclusion correction. METHODS: Cone-beam computed tomography (CBCT) scans of 38 maxillary and 37 mandibular incisors were analyzed. Displacement vectors for four anatomical landmarks (cusp tip [C], root apex [R], root neck midpoint [M], labial cementoenamel junction [L]) were quantified. Alveolar bone thickness was measured at mid-root and apical levels. Statistical analyses include

Craniofacial characteristics of successful responders to mandibular advancement splint therapy: a pilot study.

Cephalometric variables that can be used to identify patients with obstructive sleep apnoea who are suitable for mandibular advancement splints and surgical maxillomandibular advancement are lacking. The aim of this pilot study was to describe the craniofacial characteristics of patients whose symptoms of obstructive sleep apnoea were successfully treated with mandibular advancement splints and who were subsequently considered for maxillomandibular advancement. We retrospectively compared the craniofacial characteristics of our patients with data from 2 previously published studies. There were

Wassmund osteotomy for excessive gingival display: a case report with three-dimensional facial evaluation.

AIMS: This case report describes the management of an adult patient presenting with a skeletal Class II malocclusion with congenitally missing lower second premolars, maxillary dentoalveolar protrusion, and an excessive gingival display in full smile. METHODS: The treatment plan consisted of a combination of a segmental osteotomy to reposition the anterior part of the maxilla after upper premolar extractions and a bilateral sagittal split osteotomy (BSSO) to advance the mandible. Because the upper left permanent second molar required extraction, the upper left third molar was moved mesially d

One-stage microvascular mandible reconstruction and alloplastic TMJ prosthesis.

Severely deformed or absent temporomandibular joints (TMJ) benefit from total alloplastic joint replacement and large mandibular defects from revascularized free tissue transfer for reconstruction. However no cases of their combined one-stage placement with outcomes can be found in the literature. We present two cases with different indications and reconstruction. The first patient required mandibular body and ascending ramus reconstruction after previous sarcoma resection. This was with a condyle-bearing reconstruction plate which resulted in significant dysfunction, leaving the patient unabl

Orthognathic surgery for the complete rehabilitation of Moebius patients: principles, timing and our experience.

Moebius syndrome is a rare disorder found in approximately 1/100,000 neonates and the treatment of facial palsy is now well established worldwide and consists of free-muscle transplants reinnervated with motor nerves. Dentofacial deformities are often detected in Moebius patients, and different degrees of micrognathia are often present, particularly in patients with complete expressions of Moebius syndrome. However only two published reports have described the surgical treatment of such anomalies in these patients; in both cases, the suggested approach consisted of orthognathic surgery followe

Complex prosthodontic treatment with dental implants for a patient with polymyalgia rheumatica: a clinical report.

Prosthodontic and implant treatment for a patient with polymyalgia rheumatica can be complicated not only by its symptoms, but also by the side effects of long-term use of certain medications, particularly systemic glucocorticoids. This clinical report presents a polymyalgia rheumatica patient who required full-mouth rehabilitation with dental implants. The patient had a sensitive gag reflex and refused the use of any removable prostheses. She presented clinically with a skeletal Class II malocclusion with severe overbite and overjet. All her remaining dentition was determined to be unrestorab

Combined orthodontic-orthopedic approach: a second choice in some surgical cases.

The present case involves the treatment of a 21-year-old speech therapy student presenting a severe Class II/I occlusion with marked oral dysfunction handicapping her profession. In addition to a multibond appliance, the following orthopedic adjuncts were used sequentially: cervical headgear, a bimaxillary functional appliance (Biopart), a provisional occlusal plane, and composite reshaping of canine crowns. The survey of 8 years after recovery suggests that this technique is a viable alternative when patients do not opt for surgery.
