Can Automated Text Messaging Successfully Monitor Antibiotic Adherence for Urban Adolescents and Young Women Managed for Pelvic Inflammatory Disease in the Outpatient Setting.

OBJECTIVE: This study evaluates patient responsiveness to an automated text messaging system for pelvic inflammatory disease (PID) self-care support, and measures the reliability of text-reported adherence. DESIGN: Patients aged 13-25-years with mild to moderate PID were recruited from urban, academic outpatient settings. Enrolled patients received antibiotics and were randomized into a standard of care or intervention group. During a 14-day treatment period, participants in the intervention arm received a community-based nursing visit and daily text message medication reminders with evening

Perceived Social Support, Parental Notification, and Parental Engagement after Pelvic Inflammatory Disease among Urban Adolescent and Young Adults.

BACKGROUND: Urban adolescent and young adult women often require adult support throughout their transition to adulthood particularly regarding seeking healthcare. While confidentiality is crucial feature of care delivery, successful adherence to treatment can be multi-factorial. The purpose of this study is to determine factors associated with parental notification and engagement in self-care of young women diagnosed with Pelvic Inflammatory Disease (PID). METHODS: This study utilizes data from 187 participants in the Technology Enhanced Community Health Nursing (TECH-N) study, a randomized c

Preservation of the azygos vein versus ligation of the azygos vein during primary surgical repair of congenital esophageal atresia.

BACKGROUND: Esophageal atresia is one of the most common life-threatening congenital malformations and is defined as an interruption in the continuity of the esophagus with or without fistula to the trachea or bronchi. Definitive treatment is surgical ligation of the fistula if present and esophageal end-to-end anastomosis of the two pouches, thereby reconstructing the continuity of the esophagus. During this procedure, the surgeon may choose to either ligate or preserve the azygos vein, a major draining vein for the esophagus and surrounding structures, but no definitive consensus on the matt

High versus standard volume enteral feeds to promote growth in preterm or low birth weight infants.

BACKGROUND: Breast milk alone, given at standard recommended volumes (150 to 180 mL/kg/d), is not adequate to meet the protein, energy, and other nutrient requirements of growing preterm or low birth weight infants. One strategy that may be used to address these potential nutrient deficits is to give infants enteral feeds in excess of 200 mL/kg/d ('high-volume' feeds). This approach may increase nutrient uptake and growth rates, but concerns include that high-volume enteral feeds may cause feed intolerance, gastro-oesophageal reflux, aspiration pneumonia, necrotising enterocolitis, or complica

Oral stimulation for promoting oral feeding in preterm infants.

BACKGROUND: Preterm infants (< 37 weeks' postmenstrual age) are often delayed in attaining oral feeding. Normal oral feeding is suggested as an important outcome for the timing of discharge from the hospital and can be an early indicator of neuromotor integrity and developmental outcomes. A range of oral stimulation interventions may help infants to develop sucking and oromotor co-ordination, promoting earlier oral feeding and earlier hospital discharge. OBJECTIVES: To determine the effectiveness of oral stimulation interventions for attainment of oral feeding in preterm infants born before 3

Nasal intermittent positive pressure ventilation (NIPPV) versus nasal continuous positive airway pressure (NCPAP) for preterm neonates after extubation.

BACKGROUND: Previous randomised trials and meta-analyses have shown that nasal continuous positive airway pressure (NCPAP) is a useful method for providing respiratory support after extubation. However, this treatment sometimes 'fails' in infants, and they may require endotracheal re-intubation with its attendant risks and expense. Nasal intermittent positive pressure ventilation (NIPPV) can augment NCPAP by delivering ventilator breaths via nasal prongs. Older children and adults with chronic respiratory failure benefit from NIPPV, and the technique has been applied to neonates. However, seri

Surfactant therapy via thin catheter in preterm infants with or at risk of respiratory distress syndrome.

BACKGROUND: Non-invasive respiratory support is increasingly used for the management of respiratory dysfunction in preterm infants. This approach runs the risk of under-treating those with respiratory distress syndrome (RDS), for whom surfactant administration is of paramount importance. Several techniques of minimally invasive surfactant therapy have been described. This review focuses on surfactant administration to spontaneously breathing infants via a thin catheter briefly inserted into the trachea. OBJECTIVES: Primary objectives In non-intubated preterm infants with established RDS or at

Support during pregnancy for women at increased risk of low birthweight babies.

BACKGROUND: Studies consistently show a relationship between social disadvantage and low birthweight. Many countries have programmes offering special assistance to women thought to be at risk for giving birth to a low birthweight infant. These programmes, collectively referred to in this review as additional social support, may include emotional support, which gives a person a feeling of being loved and cared for, tangible/instrumental support, in the form of direct assistance/home visits, and informational support, through the provision of advice, guidance and counselling. The programmes may

Association of Red Blood Transfusion With Necrotizing Enterocolitis in Very Low Birth-Weight Neonates: A 7-Year Retrospective Cohort Study in Saudi Arabia.

BACKGROUND: Necrotizing enterocolitis (NEC) is a multifactorial disorder of the gastrointestinal tract of very low birth-weight (VLBW) infants. Blood transfusions are suspected to be associated with NEC in this population. PURPOSE: To examine the association between packed red blood cells (PRBCs) transfusion and NEC in VLBW neonates (less than 1500 gm) or less than 32 weeks. METHODS: A retrospective cohort study of all VLBW neonates admitted to the neonatal intensive care unit (NICU) at King Abdulaziz University Hospital (KAUH) from January 1, 2014, to December 31, 2020. RESULTS: Of 3018 ne

Matriarchal Influence on Black Women's Decisions to Provide Mother's Own Milk for Preterm Infants: An Integrative Review.

BACKGROUND: Black preterm infants cared for in the neonatal intensive care unit (NICU) are less likely to receive mothers' own milk (MOM) at NICU discharge than non-Black preterm infants. This disparity increases the risk of health and neurodevelopmental problems and their associated costs throughout the lifespan. One modifiable barrier to receipt of MOM at NICU discharge may lie within the role of Black maternal matriarchs; however, this role is not well characterized. PURPOSE: To describe and synthesize evidence related to the Black matriarch's influence on a mother's decision to provide MO

Neonatal Cardiac Tamponade Related to Peripherally Inserted Central Catheter: A Rare Case Report.

BACKGROUND: Peripherally inserted central catheters (PICCs) are commonly used in neonatal intensive care units (NICUs) to provide prolonged intravenous access for preterm and critically ill infants. They are generally preferred over surgically placed central venous catheters because of easier insertion and, in some settings, lower infection rates. However, rare but life-threatening complications may occur. Cardiac tamponade, resulting from catheter malposition or delayed migration with perforation of the superior vena cava (SVC) or cardiac wall, is one such complication. This case highlights t

Optimizing Enteral Nutrition During and After Therapeutic Hypothermia: An Evidence-Based Practice Initiative.

BACKGROUND: Feeding practices during therapeutic hypothermia (TH) vary widely across neonatal intensive care units, and many centers delay enteral nutrition because of concerns about gastrointestinal intolerance and necrotizing enterocolitis. Current evidence suggests that early, clinically guided enteral feeding may be safe and beneficial for neonates with hypoxic-ischemic encephalopathy. An evidence-based feeding guideline was implemented to promote enteral nutrition during and after TH. PURPOSE: To evaluate whether initiating minimal enteral feeding in clinically stable neonates undergoing

Competency Scale for Developmentally Supportive Parenting of High-Risk Infants (CS-DSPHI): Development and Psychometric Testing.

BACKGROUND: Developmentally supportive parenting (DSP) is the parental competency that optimizes developmental outcomes in high-risk infants, spanning developmental knowledge, emotional responsiveness, cognitive stimulation, and the psychological capacity to sustain these behaviors. Existing measures assess caregiving confidence or require trained observers; none captures DSP competency by self-report. PURPOSE: This study aimed to develop a self-report scale measuring DSP competency among parents of high-risk infants and to evaluate its psychometric properties. METHODS: A total of 440 parent

Effect of Adhesive Dwell Time on Skin Injury in Neonates: A Randomized Controlled Trial.

BACKGROUND: Neonatal skin is immature and highly susceptible to injury, particularly in neonatal intensive care units (NICUs) where medical adhesives are frequently used. Medical adhesive-related skin injury is a common clinical concern, yet evidence regarding optimal adhesive dwell time remains limited. PURPOSE: This study aimed to evaluate the effect of adhesive dwell time on neonatal skin integrity and its association with skin injury outcomes. METHODS: A randomized controlled trial was conducted between August and December 2023 in a tertiary NICU. Sixty term neonates (37-40 weeks) were r

Caring for Preterm Infants on Home Oxygen: A Descriptive Qualitative Study of Caregivers' Experiences in Belgium.

BACKGROUND: In the Flemish part of Belgium, preterm births and NICU admissions for acute lung diseases have increased. Nearly half of extremely preterm infants develop bronchopulmonary dysplasia (BPD), requiring prolonged oxygen therapy. Oxygen dependency often delays hospital discharge, leading to more infants going home with oxygen. Caregivers, especially mothers, face significant stress and emotional challenges during this transition, worsened by social isolation due to infection fears. PURPOSE: To gain a clearer understanding of caregivers' experiences caring for oxygen-dependent infants

Leveraging Health Technology to Support Early Bilingual Development: Insights From Neonatal Clinicians.

BACKGROUND: Establishing a strong language foundation in early childhood is essential for learning and social equity. The first 3 years of life are especially impactful, as rapid brain development makes children most receptive to language input. Early linguistic disparities increase risks for academic difficulties and dropout, widening the opportunity gap and affecting long-term health. Háblame Bebé is a free bilingual app that promotes early language development and sociolinguistic pride among Hispanic families through culturally grounded tools. Melatonin as a Supportive Biomarker in Early-Onset Neonatal Sepsis: A Prospective Study.

BACKGROUND: Early-onset neonatal sepsis (EOS) remains a diagnostic challenge because clinical signs are nonspecific and blood cultures, although the diagnostic gold standard may yield false-negative results due to low blood volumes or maternal antibiotic exposure. PURPOSE: To determine whether serum melatonin levels differ between term neonates with clinically suspected EOS and healthy controls, and to evaluate the diagnostic performance of melatonin relative to standard inflammatory markers. METHODS: This prospective case-control study included 84 term infants: 42 with clinically suspected
