Compensatory strategies during manual wheelchair propulsion in response to weakness in individual muscle groups: A simulation study.

BACKGROUND: The considerable physical demand placed on the upper extremity during manual wheelchair propulsion is distributed among individual muscles. The strategy used to distribute the workload is likely influenced by the relative force-generating capacities of individual muscles, and some strategies may be associated with a higher injury risk than others. The objective of this study was to use forward dynamics simulations of manual wheelchair propulsion to identify compensatory strategies that can be used to overcome weakness in individual muscle groups and identify specific strategies tha

Anatomy and biomechanics of the shoulder in throwing, swimming, gymnastics, and tennis.

As the most mobile joint in the body, the shoulder is structurally insecure. The ball-shaped humeral head rotates and glides on a shallow scapular cup. A limited amount of passive stability is provided by the glenoid labrum, which slightly deepens the scapular cup, and by ligaments reinforcing the capsule on its superior and anterior surfaces. At peak maturity ligamentous restraint equals 50 to 80 kg. These structural limitations indicate that the primary source of joint stability must be balanced muscle control. Joint compression is the major factor. This is supplemented by active tangential

Is there an association between SLAP lesions and biceps pulley lesions?

PURPOSE: The purpose of this study was to evaluate the prevalence of SLAP and biceps reflection pulley lesions. The hypothesis was that these lesions do not occur concomitantly and they have a different etiology. METHODS: We prospectively analyzed 3,395 shoulder arthroscopies that were independently performed by 2 surgeons between 2004 and 2008 regarding the prevalence of SLAP and biceps pulley lesions. Intraoperative findings were correlated with patient history and clinical examination. Exclusion criteria were type I SLAP lesions, full-thickness rotator cuff tears, and a history of shoulder

[Follow-up treatment with physiotherapy after arthroscopic reconstruction in SLAP lesions].

An appropriate physiotherapeutic treatment program complements an optimal operative result. In the instance of SLAP lesions, a differentiated therapy is only possible on consultation with the surgeon. A detailed treatment design allows optimal guidance of the patient to his or her personal goal. Important treatment elements include the optimization of the ability of the humerus head to centralize and the proprioception of the shoulder joint, as well as reinstatement of strength and endurance, especially of the rotator cuff.

Non-Vascularized Fibular Autograft for Two-Stage Scapulohumeral Arthrodesis in Ballistic Shoulder Injuries: A Case Series and Review.

BACKGROUND: Ballistic trauma to the proximal humerus poses a significant challenge because of extensive bone loss and soft tissue damage, often involving the rotator cuff, deltoid muscle, and axillary nerve. In cases where limb preservation is the priority, scapulohumeral arthrodesis (SHA) remains a viable option, particularly when prosthetic reconstruction is contraindicated because of severe musculotendinous injury. The Masquelet 2-stage induced membrane technique has shown promising results in lower limb reconstruction, yet its application for upper limb arthrodesis in ballistic injuries re

The Role of Hyaluronic Acid for Soft Tissue Indications: A Systematic Review and Meta-Analysis.

CONTEXT: Soft tissue injuries are often treated with injectables such as corticosteroids and platelet-rich plasma (PRP) to reduce inflammation and promote healing. There is increasing evidence examining the use of hyaluronic acid (HA) for the management of soft tissue injuries. OBJECTIVE: To evaluate the treatment effect and role of HA for available soft tissue indications. DATA SOURCES: A search of PubMed, MEDLINE, EMBASE, and CENTRAL from the inception date of each database through February 24, 2021, was conducted for all randomized controlled trials (RCTs) involving the use of HA for soft

Role of MRI in a Series of Cases with Biceps Injuries of the Shoulder.

Dislocation and injury of the biceps is associated with acute and chronic trauma, degenerative changes, repetitive microtrauma, or injuries associated with recurrent and neglected cases of shoulder dislocation. The cases of displaced biceps injuries are relatively rare (only 1/6th of all rotator cuff injuries). The dislocation of biceps is manifested with pain and restriction of movements. The X-ray shoulder is not of much help in this condition barring for ruling out the fractures of humerus and scapula. Ultrasound is an important tool in the dislocation and shows empty bicipital groove which…

Vertical versus horizontal suture configuration for the repair of isolated type II SLAP lesion through a single anterior portal: a randomized controlled trial.

PURPOSE: To compare the clinical and functional outcomes of the repair of an isolated type II SLAP lesion by 2 different configuration techniques (vertical v horizontal suture) through a single anterior portal. METHODS: We designed a prospective, double-blinded, randomized clinical trial. A junior orthopaedic surgeon, who made the initial diagnosis, used a 10-point visual analog scale for pain and subjective instability and the American Shoulder and Elbow Surgeons (ASES) scoring system and evaluated the range of motion. After a diagnostic arthroscopy that ascertained the presence of an isolat

Rotator cuff repair augmentation using a novel polycarbonate polyurethane patch: preliminary results at 12 months' follow-up.

BACKGROUND: Preventing anatomic failure after rotator cuff repair (RCR) remains a challenge. Augmentation with a surgical mesh may permanently reinforce the repair and decrease failure rates. The purpose of this study is to assess the postoperative outcomes of open RCR augmented with a novel reticulated polycarbonate polyurethane patch. MATERIALS AND METHODS: Ten patients with supraspinatus tendon tears underwent open RCR augmented with a polycarbonate polyurethane patch secured in a 6-point fixation construct placed over the repaired tendon. Patients were evaluated with preoperative and post

The influence of the acromial coverage index in rotator cuff tears.

Several intrinsic and extrinsic factors have been advocated in the pathogenesis of rotator cuff tears, but it is still unclear whether the origin of the tear is related to tendon degeneration itself or induced by several morphologic changes. The purpose of this study is to determine the relationship between the acromial coverage of the humeral head and the presence of a cuff tear. We evaluated 148 shoulders, including 45 that underwent surgical rotator cuff repair (group I), 26 with documented rotator cuff tears treated conservatively (group II), and 77 with no cuff pathology as a control grou

Arthroscopic evaluation and management of rotator cuff tears.

The development of arthroscopic techniques has provided the shoulder surgeon with new opportunities to improve the chances of salvaging these difficult rotator cuff tears while generally avoiding the potential disastrous consequences attendant to the open procedures. The arthroscope can be used to review and document the status of the joint and cuff, assess the tear pattern, debride damaged tissues, and smooth the acromion. In addition, a trained shoulder arthroscopist can mobilize the available tissues and repair any viable tendon to bone, without damage to the overlying muscles, through smal

[Biomechanical investigations for the development of a SLAP-II-lesion].

The superior labral-biceps-tendon-complex forms an anatomical and functional unit and combines static and dynamic elements of shoulder stability. At present, only theoretical hypotheses exist on the etiology of the microtraumatic SLAP-II-lesion. To gain further insight into this, an instrument was developed to simulate throwing motions such as the late-cocking/early acceleration phase as well as deceleration/follow-through. Sixteen freshly frozen shoulder specimens were tested, varying the loads on the biceps tendon (25 N, 50 N, 100 N) and the compression of the humeral head against the glenoi

Platelet-rich plasma: where are we now and where are we going?

CONTEXT: Platelet-rich plasma (PRP) may affect soft tissue healing via growth factors released after platelet degranulation. Because of this potential benefit, clinicians have begun to inject PRP for the treatment of tendon, ligament, muscle, and cartilage injuries and early osteoarthritis. EVIDENCE ACQUISITION: A PubMed search was performed for studies relating to PRP, growth factors, and soft tissue injuries from 1990 to 2010. Relevant references from these studies were also retrieved. RESULTS: Soft tissue injury is a major source of disability that may often be complicated by prolonged an

Rehabilitation protocols after arthroscopic rotator cuff repair: a survey of active members of the Korean Shoulder and Elbow Society.

BACKGROUND: Despite the clinical importance of rehabilitation after arthroscopic rotator cuff repair, standardized postoperative rehabilitation protocols are yet to be established. Therefore, this study aimed to investigate the current consensus on rehabilitation protocols after arthroscopic rotator cuff repair among active members of the Korean Shoulder and Elbow Society (KSES). We hypothesized that rehabilitation protocols would vary and that there might be a tendency to adjust rehabilitation based on the preoperative tear size and level of physical demand of the individual patient. Editorial Commentary: Fascia Lata Allograft for Shoulder Superior Capsular Reconstruction: In the Fight Over Optimal Graft Choice for Irreparable Rotator Cuff Tears, Superior Capsular Reconstruction Proponents May Be Changing Their Gloves.

Optimal treatment of irreparable rotator cuff tears is still debated. Proponents of the superior capsule reconstruction (SCR) have previously used fascia lata autograft and acellular dermal allograft. Interest is growing in using fascia lata allograft as a new graft material. Well-designed biomechanical studies are important to understand the mechanical properties of the superior capsular tissue and fascia lata allograft. Recent biomechanical research shows that fascia lata allograft has similar initial stiffness (over the first 2 mm) and ultimate load compared to the native superior capsule.

Occupational Stress as a Possible Risk Factor for Rotator Cuff Damage - Systematic Review and Meta-analysis.

PURPOSE: In September 2021, the Federal Ministry of Labour and Social Affairs in Germany (BMAS) recommended recognising rotator cuff lesions caused by occupational long-term overhead work, repetitive movements, hand-arm-vibration and carrying and lifting heavy loads as an occupational disease. To gain a more detailed understanding of the correlation between occupational risk factors and lesions of the rotator cuff, a systematic literature review was carried out to determine their influence on the development of rotator cuff disease. MATERIAL AND METHOD: A systematic literature review was used

Tenodesis yields better functional results than tenotomy in long head of the biceps tendon operations-a systematic review and meta-analysis.

BACKGROUND: Pathology of the long head of the biceps tendon (LHBT) is a common disorder affecting muscle function and causing considerable pain for the patient. The literature on the two surgical treatment methods (tenotomy and tenodesis) is controversial; therefore, our aim was to compare the results of these interventions. METHODS: We performed a meta-analysis using the following strategy: (P) patients with LHBT pathology, (I) tenodesis, (C) tenotomy, (O) elbow flexion and forearm supination strength, pain assessed on the ten-point Visual Analog Scale (VAS), bicipital cramping pain, Constan
