A series of 68 chest injuries treated at the Resuscitation and Intensive Care Unit. S. Orsola Policlinic, Bologna, between May 1967 and December 1973 is presented. Treatment was given by the staff of the anaesthesia and resuscitation service and the chest surgery section, 2nd Surgical Clinic, University of Bologna. The types of case observed and the treatment methods employed are bierfly explained, together with the clinical and statistical data. Emphasis is laid on the relation between clinical course and blood gas values. The value of tracheostomy and protracted intubation is discussed, toge

11 children were admitted with electrical injuries to the Department of Paediatrics, University of Graz, over the 10-year period 1965 to 1974. The electricity was of low voltage in the case of 9 of these children, who were injured whilst playing with plug sockets of defective cables and of high voltage in 4 case. 1 child was stuck by lightning. Cessation of respiration occurred in 4 of the patients and was successfully counteracted in all cases by prompt mouth-to-mouth breathing. 7 children were still shocked on admission. All children showed the characteristic skin lesions of electrical injur

The effects of cocaine and theophylline on the changes in core temperature, O2 consumption and the rate of heat loss from the tail were studied in 20° and 3° acclimated rats during and after a 4-hr. period of bilateral hindlimb ischaemia at an environmental temperature of 20°. Cocaine (7 mg. base per kg. body wt. intravenously) had no effect on injured 20° acclimated rats but increased the core temperature in both injured and uninjured 3° acclimated ones. The changes in O2 consumption were unaffected and heat loss from the tail decreased. Cocaine acted mainly by increasing peripheral vasoconst

Wunderlich syndrome (WS) is a rare but life-threatening condition characterized by spontaneous, non-traumatic renal hemorrhage, which may present with Lenk's triad. Early recognition and prompt management are crucial to improve prognosis. We report the case of a 49-year-old man with end-stage renal disease secondary to antineutrophil cytoplasmic antibody-associated vasculitis on maintenance hemodialysis who developed sudden left flank pain during a routine hemodialysis session, without preceding trauma. Laboratory evaluation revealed anemia and elevated international normalized ratio. Computed

The rate of gallbladder injury in the setting of blunt abdominal trauma is ⁓2%. Furthermore, an isolated gallbladder injury in a case with blunt trauma is even rarer. A 16-year-old female patient fell from the third floor of a secondary school building and was admitted to the Emergency Department with traumatic shock, and multiple organ injuries including head injury, spinal fracture, femur fracture, and blunt abdominal trauma. The patient received initial first aid and diagnostic tests, including blood tests, abdominal ultrasound, and contrast-enhanced abdominal computed tomography, which rev

Peliosis hepatis (PH) is a rare vascular condition of the liver characterized by the presence of cystic blood filled cavities distributed randomly throughout the liver parenchyma. We describe a case of a 42-year-old previously healthy male patient, airlifted to us in a state of hemorrhagic shock after undergoing percutaneous diagnostic liver biopsy for lesions seen on CT scan. Repeat CT scan with IV contrast on presentation in our hospital showed intraperitoneal bleeding. Hepatic angiography failed to identify any specific bleeding source. A diagnostic laparoscopy was performed and approximate

The authors report their experience with combined percutaneous nephrostomy and extracorporeal shock-wave lithotripsy to treat obstructive uratic ureteral stones. The role of nephrostomy is stressed as a diagnosis and treatment method before, during and after lithotripsy. Thus, the method proved especially useful to drain obstructed kidneys and restore peristalsis, to evacuate septic urine, to facilitate the elimination of lithiasic fragments, to perform anterograde pyelography before, during and after lithotripsy and finally to allow pharmacological litholysis. Fourteen patients were successfu

In modern war or daily life, blast-induced traumatic brain injury (bTBI) is a growing health concern. Our previous studies demonstrated that inflammation was one of the main features of bTBI, and CD28-activated T cells play a central role in inflammation. However, the mechanism of CD28 in bTBI remains to be elucidated. In this study, traumatic brain injury model induced by chest blast exposure in male mice was established, and the mechanism of CD28 in bTBI was studied by elisa, immunofluorescence staining, flow cytometry analysis and western blot. After exposure to chest shock wave, the inflam

BACKGROUND Coagulopathy caused by trauma itself is defined as trauma-induced coagulopathy (TIC). The pathophysiology of TIC is considered to consist of coagulation activation, hyperfibrinolysis, and consumption coagulopathy, similar to disseminated intravascular coagulation (DIC). This report describes a 68-year-old man with a history of epilepsy presenting with TIC associated with multiple traumatic fractures and hemothorax. CASE REPORT A 68-year-old man with a history of epilepsy fell while working on a second-floor roof and had right rib fractures (6th to 12th rib), right hemothorax, right

Peri-operative hemorrhagic shock and resuscitation (HSR), a severe traumatic stress, is closely associated with post-operative anxiety, depression, and cognitive dysfunction, subsequently causing a serious burden on families and society. Following the co-release of corticotropin-releasing factor and catecholamine, traumatic stress activates dopaminergic neurons, increasing the addictive behavior and neurocognitive impairment risks. This study investigates the association between cognitive dysfunction and dopaminergic neurons in the mPFC under HSR conditions. This study established an HSR model

BACKGROUND: Fear responses are particularly intense and persistent in post-traumatic stress disorder (PTSD), and can be evoked by unspecific cues that resemble the original traumatic event. Overgeneralisation of fear might be one of the underlying mechanisms. We investigated the generalisation and discrimination of fear in individuals with and without PTSD related to prolonged childhood maltreatment.
METHODS: Sixty trauma-exposed women with (N = 30) and without (N = 30) PTSD and 30 healthy control participants (HC) underwent a fear conditioning and generalisation paradigm. In a contingency le

OBJECTIVE: Early death following cranial trauma is often considered unsurvivable traumatic brain injury (TBI). However, Impact Brain Apnoea (IBA), the phenomenon of apnoea following TBI, may be a significant and preventable contributor to death attributed to primary injury. This paper reviews the history of IBA, cites case examples and reports a survey of emergency responder experience.
METHODS: Literature and narrative review and focused survey of pre-hospital physicians.
RESULTS: IBA was first reported in the medical literature in 1705 but has been demonstrated in multiple animal studies a

PURPOSE OF REVIEW: Despite advances in the care of the injured, the morbidity and mortality of traumatic hemorrhage remain a significant problem. Traumatologists continue to look for ways to treat bleeding and prevent the sequelae of hemorrhagic shock. Recombinant factor VIIa, developed for the treatment of patients with hemophilia, has been used with some success in acute bleeding associated with injuries.
RECENT FINDINGS: The mechanism of action is via a tissue factor-dependent effect and/or platelet activation. Coagulation occurs at the site of tissue injury, where tissue factor is exposed

The influence of trauma and hemorrhagic shock on the non-specific immune system has been pointed out in various experimental studies. Other investigations have also been able to find a relationship between these changes and a higher incidence of post-traumatic complications in the form of organ failure. Our aim was to demonstrate the potential changes in the cellular defense system in a clinical study on multiple trauma patients. The polymorphonuclear leukocytes (PMNL) are the main representative of the mobile, non-specific immune system. Our study revealed a significant deterioration of PMNL
