Depression and somatization in patients with temporomandibular disorders.

STATEMENT OF PROBLEM: Psychological and behavioral traits may be important for the diagnosis and management of orofacial pain. PURPOSE: This study compared the levels of depression and somatization in patients in single and multiple research diagnostic criteria for temporomandibular disorders (RDC/TMD) diagnostic groups. MATERIAL AND METHODS: The RDC/TMD was established to allow standardization and replication of research into the most common forms of muscle- and joint-related research and is divided into 2 axes: axis I, clinical TMD, and axis II, pain-related disability and psychological st

A family study of hypochondriasis.

To examine the diagnostic validity of hypochondriasis, we undertook a preliminary family study. Nineteen probands with and 24 without DSM-III-R hypochondriasis were identified among outpatients attending a general medicine clinic. Seventy-two first-degree relatives of hypochondriasis probands and 97 relatives of control probands were personally interviewed with the use of the Structured Clinical Interview for DSM-IV. These relatives also completed self-administered measures of hypochondriasis, psychological and somatic symptoms, and personality traits. No increase in the rate of hypochondriasi

Group therapy for somatization disorders in general practice: effectiveness of a short cognitive-behavioural treatment model.

The objective of this study was to evaluate the effect of a short cognitive behavioural group therapy programme for somatization disorder in primary care. The treatment model was focused on patient education and stress relaxation. A controlled and prospective study design was used with repeated assessments of the patients' perception of their psychosocial problems, psychological distress and medication usage. The results were analysed up to 6 months after treatment and showed the treated patients to be moderately but significantly improved with respect to physical illness and somatic preoccupa

Application of the relaxation technique in general hospital psychiatry.

The present article demonstrates the clinical application and the efficacy of relaxation techniques in general hospital psychiatry in Japan. During April to December 1993, 20 patients were treated with progressive muscle relaxation and "modified' autogenic training therapies. The targeted symptoms were anxiety attacks (or panic attacks), neurotic insomnia, hyperventilation syndrome, agoraphobia, chronic pain, and distress. Most patients (18/20) improved rapidly with the use of these techniques and their excellence encourages increased use in general hospital psychiatry for the following reason

Prevalence and strong association of high somatic symptom severity with depression and anxiety in a Chinese inpatient population.

BACKGROUND: The prevalence of the high somatic symptom severity (HSSS) and its associations with sociodemographic factors, depression, and anxiety has not been surveyed in inpatient populations at general hospitals. METHODS: A sample including 1329 inpatients in a Chinese general hospital was surveyed using Chinese version of 15-item patient health questionnaire (PHQ-15), 9-item patient health questionnaire, and generalized anxiety disorder 7-item scale. RESULTS: A total of 27.8% (n = 369) of the participants had HSSS (PHQ-15 ≥ 10). The multivariate regression showed that HSSS was significan

Functional neurological symptoms modulate processing of emotionally salient stimuli.

OBJECTIVE: Dysfunctional emotion processing has been discussed as a contributing factor to functional neurological symptoms (FNS) in the context of conversion disorder, and refers to blunted recognition and the expression of one's own feelings. However, the emotion processing components characteristic for FNS and/or relevant for conversion remain to be specified. With this goal, the present study targeted the initial, automatic discrimination of emotionally salient stimuli. METHODS: The magnetoencephalogram (MEG) was monitored in 21 patients with functional weakness and/or sensory disturbance

Consciousness about own and others' affects: a study of the validity of a revised version of the Affect Consciousness Interview.

This study presents a modified version of the affect consciousness interview (Monsen, Eilertsen, Melgård & Odegård, 1996), intended to capture the individual's affective consciousness. The aim of the modified version - The Affect Consciousness Interview - Revised (ACI-R) - is to measure consciousness about own and others' affects. Three groups of patients (with eating disorder, relational and social problems or stress-related problems), and one non-clinical group were included in the study (N= 95). The results indicated that it was possible to achieve adequate interrater reliability, that the

Monoamine system disruption induces functional somatic syndromes associated symptomatology in mice.

Functional somatic syndromes (FSS), a clinical condition manifesting a variety of unexplained somatic symptoms, has been proposed as an inclusive nosology encompassing individual syndromes such as fibromyalgia syndrome and irritable bowel syndrome. Accumulating evidence suggests that disturbance of the endogenous monoamine system could be involved in the aetiology of FSS. Therefore, the purpose of present study was to investigate whether the disturbance of the monoamine system would cause FSS-associated symptomatology in mice. The optimal dose of reserpine, an inducer of endogenous monoamines

Sensations of skin infestation linked to abnormal frontolimbic brain reactivity and differences in self-representation.

Some patients experience skin sensations of infestation and contamination that are elusive to proximate dermatological explanation. We undertook a functional magnetic resonance imaging study of the brain to demonstrate, for the first time, that central processing of infestation-relevant stimuli is altered in patients with such abnormal skin sensations. We show differences in neural activity within amygdala, insula, middle temporal lobe and frontal cortices. Patients also demonstrated altered measures of self-representation, with poorer sensitivity to internal bodily (interoceptive) signals and

Anxiety and depression predicted by medically unexplained symptoms in Pakistani children: a case-control study.

OBJECTIVE: To explore association between medically unexplained symptoms in children in Pakistan with emotional difficulties and functional impairments. METHODS: We conducted a matched three-group case-control study of 186 children aged 8-16 years in Lahore, Pakistan. Cases were 62 children with chronic somatic symptoms for which no organic cause was identified after investigations. Two control groups of 62 children with chronic medical paediatric conditions, and 62 healthy children were identified. Cases and controls were matched for gender, age, and school class. Somatisation was measured w

The psychology of LUTS: ICI-RS 2011.

INTRODUCTION: There is undoubtedly a link between lower urinary tract and psychological symptoms. The association is likely to be multifactorial, but whether psychological symptoms are causal or coincidental remains unknown. METHODS: This paper is a result of a think tank session conducted at the ICI-RS meeting 2011 in Bristol and a PubMed search on the subject of psychology and LUTS. RESULTS: Recent population studies have shown that LUTS are prevalent and that psychological morbidity is related to severity of LUTS. Evidence suggests that the psychological status of patients plays a signifi

Sexual abuse, physical abuse, chronic fatigue, and chronic fatigue syndrome: a community-based study.

Using a randomly selected community-based sample, this investigation examined whether histories of childhood sexual, physical, and death threat abuse predicted adulthood outcomes of specific medical and psychiatric conditions involving chronic fatigue. This study also tested prior suggestions that most individuals with chronic fatigue syndrome report a past history of interpersonal abuse. Multinomial logistic regression was used to examine the relationship between abuse history and chronic fatigue group outcomes while controlling for the effects of sociodemographics. Work disability and health-affecting psychosocial problems among patients in general practice.

Psychosocial problems are often ignored among patients in general practice. By identifying high risk groups this situation could possibly be altered. This study aimed to explore if patients in general practice perceiving themselves as work-disabled by at least 50% more often have health-affecting psychosocial problems than those not work-disabled. In a geographically defined population, 1,058 consecutive adult patients consulting 89 general practitioners were approached during one regular working day in March 1995. They completed a questionnaire at home, returning it directly to the department

The impact of catastrophic beliefs on functioning in chronic fatigue syndrome.

This study investigated the association between catastrophic beliefs and disability in the context of Chronic fatigue syndrome (CFS). A sample of 282 CFS sufferers were asked about the consequences of pushing themselves beyond their present physical state. Responses were coded into catastrophic or non-catastrophic categories. While not differing on the length of illness or psychological adjustment, subjects demonstrating catastrophic responses evidenced significantly higher levels of fatigue and were more disabled in terms of their ability to work both in their normal occupation and around the

Effectiveness of psychiatric intervention with somatization disorder patients: improved outcomes at reduced costs.

In order to determine the health effects of an intervention that reduces the cost of care for somatization disorder (SD) patients, 59 primary care physicians were randomized to receive a psychiatric consultation letter providing treatment recommendations for 73 patients either at baseline or the end of the year-long study. Seventy of these patients (96%) were followed every 4 months for 1 year by a research assistant blind to randomization. A year following the intervention, patients of experimental physicians reported greater physical capacity than patients of control physicians (mean differe

Sleep disorders in Chinese culture: experiences from a study of insomnia in Taiwan.

Traditional Chinese culture-constituted health beliefs continue to influence the Taiwanese people after more than one hundred years of contact with Western medicine. Medicine for sleep disorders, as well as psychiatric medicine, meets some specific difficulties in the professional development. A study of insomnia in Taiwan showed that patients might seek help from a traditional physician and visit a modern hospital at the same stage of medication. General internists and neurologists help to differentiate organic conditions underlying sleep problems but may generalize insomnia to a psychogenic
