Editorial Commentary: The Long Head of the Biceps Tendon Is Useful for Shoulder Reconstruction Including Glenohumeral Stabilization: From Biceps Killers to Biceps Users.

Historically, it was believed that the shoulder long head of the biceps tendon (LHBT) was a pain generator and had to be routinely sacrificed. Recently, it has become apparent that the LHBT is useful as an autograft for various types of surgical reconstruction, including superior capsular reconstruction for irreparable rotator cuff tears, and augmentation for poor soft-tissue quality during rotator cuff repair or shoulder arthroplasty. In cases of shoulder instability, the biceps can reinforce the capsule or reconstruct a missing labrum for glenohumeral stabilization. Dynamic anterior stabiliz

[A case of secondary central nervous system lymphoma presenting marked hypoglycorrhachia].

A 67-year-old male was transferred to our hospital with diplopia, decreased deep tendon reflex and ataxia. He had been suspected Fisher syndrome because of previous upper respiratory tract infection. A cerebrospinal fluid examination showed marked hypoglycorrhachia, pleocytosis and elevated protein, and cytological examination suggested malignant lymphoma. Abdominal computed tomography revealed a left adrenal mass. A biopsy of the left adrenal mass revealed diffuse large B-cell lymphoma. He was treated with a combination of R-CHOP (rituximab, cyclophosphamide, doxorubicin hydrochloride, oncovi

Reconstruction of the medial patellofemoral ligament for the treatment of habitual or recurrent dislocation of the patella in children.

We investigated the clinical outcome of a reconstructive procedure of the medial patellofemoral ligament for the treatment of habitual or recurrent dislocation of the patella in four children (6 knees), with a minimum follow-up of four years. The technique involves transfer of the tendon of semitendinosus to the patella using the posterior one-third of the femoral insertion of the medial collateral ligament as a pulley. There was no recurrence of dislocation after surgery. The mean Kujala score at follow-up was 96.3 points. Radiological assessment showed that the congruence angle, the tilt ang

[Transport-fixator for reconstructive surgery of the extensor apparatus of the knee joint].

Operative repair in ruptures of the extensor mechanism of the knee joint mostly requires additional external or internal fixation by plaster, transfer of tendons or encircling wires. Reconstruction of initially undiagnosed ruptures has to solve the problems of fixed proximal or distal migration of the patella and tendenous shortening. This can be performed with a tibiopatellar external fixator which allows gradually reduction of the patella in an anatomical position before operative reconstruction by suturing or reinsertion. During this period as well as a post-operative protection this fixato

[Extensive skin losses of the upper limb].

The development of microsurgical techniques and of pedicled local flaps has improved the prognosis of major injuries of the upper limb. Pedicled muscle-skin flaps from the latissimus dorsi muscle allow repairing losses of substance in the arm-pit and arm while restoring the function of the elbow. In the forearm, the same flap, transferred by microsurgery, ensures the rehabilitation of the wrist and hand. In the hand, the Chinese and posterior interosseous flaps allow covering the losses of substance on both the palmar and the dorsal aspects. Here, the free flaps are used only if the palmar vas

Sural nerve grafts for delayed repair of divided posterior interosseos nerves.

Sural nerve grafts were used in three patients to bridge gaps measuring 2 1/2 to 4 1/2 cm in previously transected but not repaired posterio … d results with nerve grafts in delayed nerve repairs. We believe that these results are better than tendon transfers, at least in young patients.

[Reconstruction of pronation of the forearm (experiences using the Zancolli operation)].

A supination contracture of the forearm is a severe handicap in paraplegics as well as in brachial plexus pareses, e.g. in operating wheelchairs, supporting the upper part of the body and other daily tasks. For restoration of the pronation of the forearm, Zancolli et al. have suggested transfer of the biceps tendon. Since 1976, we have carried out this operation on 16 patients. These cases involved plexus pareses due to birth trauma, high transverse lesions of the cord with paraplegia. In two patients, there was spastic hemiparesis. The results are presented and discussed. Full active pronatio

Split Peroneus Longus Free Tendon Autograft Transplantation for the Treatment of Neglected Extensor Hallucis Longus Tendon Laceration: A Case Report.

A neglected laceration of the extensor hallucis longus (EHL) tendon is rare. Retraction of the tendon ends often occurs when a laceration is neglected, leading to a substantial tendon deficit. A paucity of case reports is available describing the treatment of EHL laceration with a large area of gap secondary to retraction. Therefore, the treatment recommendations are limited. We present the case of a neglected EHL tendon laceration with a 10.5-cm gap in a healthy 22-year-old female. The EHL tendon laceration was repaired using a split peroneus longus tendon autograft that, to the best of our k

Assessment of results of opponensplasty.

One hundred and fifty-six opponensplasties carried out on 115 patients at Anandaban Hospital between 1987 and 1997 were reviewed. In most cases a flexor digitalis superficialis opponensplasty was performed. The outcome was assessed by measuring the finger to which the thumb could obtain a pinch grip, the gap between the thumb and little metacarpophalangeal joints, and the satisfaction of the patient. The objective assessments demonstrated excellent or good results in 89%. Good or fair patient satisfaction was obtained in 93%. Early complications were seen in seven cases (4%). Objective measure

[Treatment of severe concave clubfoot in neural muscular atrophy].

The deformity with which this neurological condition first shows itself is the pes cavus. During the period of growth a severely instable cavovarus deformity develops. The author avoids operations on children because the results are usually poor; the basic problem cannot be resolved with surgical measures. He recommends that the affected feet be immobilized by plastic boots until puberty is reached, at which time only minor bone resections for correcting the form of the feet are necessary. In cases of severe cavovarus deformity in early adulthood the author does not use the usual treatment, tr

Correction of arthrogrypotic clubfoot with a modified Ponseti technique.

Surgical releases for arthrogrypotic clubfeet have high recurrence rates, require further surgery, and result in short, painful feet. We asked whether a modified Ponseti technique could achieve plantigrade, braceable feet. Ten patients (mean age, 16.2 months; range, 3-40 months), with 19 arthrogrypotic clubfeet, underwent an initial percutaneous Achilles tenotomy to unlock the calcaneus from the posterior tibia followed by weekly Ponseti-style casts. A second percutaneous Achilles tenotomy was performed in 53%. Mean number of casts was 7.7 (range, 4-12). From pretreatment to completion of init

Latissimus dorsi transfer for external rotation weakness of the shoulder in obstetric brachial plexus palsy.

This report describes the results of latissimus dorsi transfer for weakness of shoulder external rotation in 12 children with obstetric brachial plexus palsy. Selection criteria for the muscle transfer procedure included good passive external rotation of the shoulder, normal skeletal development of the shoulder joint, and adequate power of the deltoid. In addition to measuring the ranges of motion of joints before and after surgery, a modified Mallet grading system was utilized for the functional assessment of shoulder external rotation. At a mean follow-up of 4 years, two of the 12 children h

Changes in the Neurovascular Anatomy of the Shoulder After an Open Latarjet Procedure: Defining a Surgical Safe Zone.

BACKGROUND: Although previous literature has described the relevant anatomy for an open anterior Bankart approach of the shoulder, there is little known regarding the anatomic relationship changes in the neurovascular structures after an open Latarjet procedure. PURPOSE: To define the neurovascular anatomy of the native shoulder in relation to the coracoid and to define the anatomy after the Latarjet procedure in relation to the glenoid to determine distances to these neurovascular structures with and without neurolysis of the musculocutaneous nerve (MCN) from the conjoint tendon. The 'three-in-one' surgical procedure for habitual dislocation of the patella in children and adolescents provides functional knee improvement without recurrences at mid-term follow-up: a retrospective case series.

PURPOSE: To introduce the 'three-in-one' surgical procedure, and to report its clinical outcomes. METHODS: This retrospective clinical study included 42 skeletally immature patients with habitual patellar dislocation who underwent a 'three-in-one' surgical procedure from 2017 to 2022. This procedure comprised an extensive lateral release, half medial transfer of patellar tendon with subperiosteal fixation, and reconstruction of the medial patellofemoral ligament with semitendinosus autograft. The clinical results were evaluated using preoperative and postoperative functional score and imaging

The Latarjet-Patte Procedure Past, Present, and Future.

The Latarjet-Patte procedure has a long and storied history that predates shoulder arthroscopy, but its popularity has increased over the past two decades as a treatment for anterior glenohumeral instability with bone loss. Transfer of the coracoid process and conjoint tendon to the anteroinferior glenoid creates a triple-blocking effect that is both static and dynamic, lending it biomechanical superiority over alternative bone block techniques. Patients undergoing the Latarjet-Patte procedure have low rates of recurrent instability compared to arthroscopic soft-tissue stabilization procedures

Up to 18% of multiligament knee injuries (MLKI) have an associated vascular injury. All MLKI should be assessed using the ankle brachial pressure index (ABPI) with selective arteriography if ABPI is < 0.9.An ischaemic limb following knee dislocation must be taken to the operating theatre immediately for stabilization and re-vascularization.Partial common peroneal nerve (CPN) injury following MLKI has better recovery than complete palsy.Posterior tibial tendon transfer is offered to patients with complete CPN palsy if there is no recovery at six months.Operative treatment with acute or staged re…
