Protective mechanisms of CA074-me (other than cathepsin-B inhibition) against programmed necrosis induced by global cerebral ischemia/reperfusion injury in rats.

Many studies have demonstrated the key role of lysosomes in ischemic cell death in the brain and have led to the "lysosomocentric" hypothesis. In this hypothesis, the release of cathepsin-B due to a change of lysosomal membrane permeabilization (LMP) or rupture is critical, and this can be prevented by its inhibitors CA074 and CA074-me. However, the role of CA074-me in neuronal death and its effect on the change of lysosomal membrane integrity after global cerebral ischemia/reperfusion (I/R) injury is not clear, so we investigated this here. Rat hippocampal CA1 neuronal death was evaluated aft

Language Recovery after Brain Injury: A Structural Network Control Theory Study.

Aphasia recovery after stroke depends on the condition of the remaining, extralesional brain network. Network control theory (NCT) provides a unique, quantitative approach to assess the interaction between brain networks. In this longitudinal, large-scale, whole-brain connectome study, we evaluated whether controllability measures of language-related regions are associated with treated aphasia recovery. Using probabilistic tractography and controlling for the effects of structural lesions, we reconstructed whole-brain diffusion tensor imaging (DTI) connectomes from 68 individuals (20 female, 4

A Proposed Mechanism for Visual Vertigo: Post-Concussion Patients Have Higher Gain From Visual Input Into Subcortical Gaze Stabilization.

PURPOSE: Post-concussion syndrome (PCS) is commonly associated with dizziness and visual motion sensitivity. This case-control study set out to explore altered motion processing in PCS by measuring gaze stabilization as a reflection of the capacity of the brain to integrate motion, and it aimed to uncover mechanisms of injury where invasive subcortical recordings are not feasible. METHODS: A total of 554 eye movements were analyzed in 10 PCS patients and nine healthy controls across 171 trials. Optokinetic and vestibulo-ocular reflexes were recorded using a head-mounted eye tracker while part

Accidental fall due to abnormal behavior after experiencing a Streptococcus pyogenes infection: febrile delirium or pediatric autoimmune neuropsychiatric disorder associated with streptococcal infection?

An 11-year-old boy with a high fever (39.4 degrees C) presented at a local medical institution. His condition was diagnosed as hemolytic streptococcal infection, and he was prescribed an antibiotic. After returning home, he took a dose of his medication and rested; however, he suddenly began to run around while yelling incomprehensible words. He ran up to his room on the second floor and fell from the second floor window down to the ground. He lost consciousness and was transferred to our department. His history included being born as a twin with a low birth weight and pneumonia at 1 year of a

Relationship between clinician documented blast exposure and pulmonary function: a retrospective chart review from a national specialty clinic.

BACKGROUND: Service member exposure to explosive blast overpressure waves is common with considerable attention to traumatic brain injury (TBI) and neuropsychological sequalae. Less is known about the impacts on the respiratory system, particularly long-term effects, despite vulnerability to overpressure. Using a national registry, we previously observed an independent relationship between self-reported blast exposure and respiratory symptoms; however, the impact on objective measures of pulmonary function is poorly understood. METHODS: 307 Veterans referred to our national specialty center f

Diffusion-weighted imaging in the follow-up of treated high-grade gliomas: tumor recurrence versus radiation injury.

BACKGROUND AND PURPOSE: Diffusion-weighted (DW) MR imaging is a means to characterize and differentiate morphologic features, including edema, necrosis, and tumor tissue, by measuring differences in apparent diffusion coefficient (ADC). We hypothesized that DW imaging has the potential to differentiate recurrent or progressive tumor growth from treatment-induced damage to brain parenchyma in high-grade gliomas after radiation therapy. METHODS: We retrospectively reviewed follow-up conventional and DW MR images obtained starting 1 month after completion of radiation treatment with or without c

Disrupted age-related glymphatic patterns in children with deep gray matter hypoxic-ischemic encephalopathy: a diffusion tensor imaging analysis along the perivascular space study.

PURPOSE: Hypoxic-ischemic encephalopathy (HIE) remains a major cause of pediatric morbidity, with deep gray matter injury reflecting a severe pattern. The glymphatic system, a brain-wide perivascular network facilitating cerebrospinal and interstitial fluid exchange, may be vulnerable to disruption after HIE. We assessed glymphatic function in children with HIE-induced deep gray matter injury using diffusion tensor imaging analysis along the perivascular space (DTI-ALPS) and examined age-related patterns in glymphatic function. METHODS: DTI-ALPS analysis was retrospectively studied in 38 norm

Ventilator-Associated Pneumonia Predicts Severe Cognitive Disability in Severe Traumatic Brain Injury.

Background: Ventilator-associated pneumonia (VAP) is linked to poor outcomes in patients with severe traumatic brain injury (TBI), yet its effect on cognitive disability is unknown. We hypothesized that there would be an association between severe cognitive disability and VAP in this patient population. Methods: We performed a post hoc analysis of a prospective, multi-center, observational study of adults with a severe, blunt TBI from 2020 to 2023. Patients were grouped by whether they developed VAP. Our primary outcome was severe cognitive disability, defined as a disability rating scale (DRS

The subtype matters: unraveling the clinical impact of TBI variants among patients with isolated blunt head injury and skull fractures.

INTRODUCTION: Traumatic Brain Injury (TBI) is a major cause of morbidity and mortality worldwide, with isolated blunt TBIs presenting unique clinical challenges. Despite extensive research, limited studies have examined how specific TBI subtypes, subdural hematoma (SDH), subarachnoid hemorrhage (SAH), epidural hemorrhage (EDH), diffuse axonal injury (DAI), and contusions-affect critical outcomes such as in-hospital mortality, ICU length of stay (LOS), and ventilation duration. Understanding these associations is essential for improving patient management and resource allocation. OBJECTIVE: Th

An exploratory study on the association between blood-based biomarkers and subacute neurometabolic changes following mild traumatic brain injury.

BACKGROUND AND OBJECTIVES: Blood-based biomarkers and advanced neuroimaging modalities such as magnetic resonance spectroscopy (MRS) or diffusion tensor imaging (DTI) have enhanced our understanding of the pathophysiology of mild traumatic brain injury (mTBI). However, there is limited published data on how blood biomarkers relate to neuroimaging biomarkers post-mTBI. METHODS: To investigate this, 30 patients with mTBI and 21 healthy controls were enrolled. Data was collected at two timepoints postinjury: acute, < 24 h, (blood) and subacute, four-to-six weeks, (blood and imaging). Interleukin

Early identification of brain injury in infants with hypoxic ischemic encephalopathy at high risk for severe impairments: accuracy of MRI performed in the first days of life.

BACKGROUND: Despite therapeutic hypothermia 30-70% of newborns with moderate or severe hypoxic ischemic encephalopathy will die or survive with significant long-term impairments. Magnetic resonance imaging (MRI) in the first days of life is being used for early identification of these infants and end of life decisions are relying more and more on it. The purpose of this study was to evaluate how MRI performed around day 4 of life correlates with the ones obtained in the second week of life in infants with hypoxic-ischemic encephalopathy (HIE) treated with hypothermia. METHODS: Prospective obs

Early hematoma expansion provides incremental prognostic information beyond admission characteristics for 6-month functional outcome after adult traumatic brain injury.

BACKGROUND: Early hematoma expansion (HE) reflects interval progression of traumatic intracranial hemorrhage, but its association with long-term outcome and its prognostic contribution beyond admission characteristics remain incompletely defined. We evaluated whether HE was independently associated with 6-month functional outcome and provided incremental prognostic information beyond admission characteristics. METHODS: This single-center retrospective study included 640 adults with traumatic brain injury, including 285 with HE. HE was determined from the first two in-hospital head computed to

CMX-2043 Treatment Limits Neural Injury Pathophysiology and Promotes Neurological and Cognitive Recovery in a Pediatric Porcine Traumatic Brain Injury Model.

Traumatic brain injury (TBI) represents a major global health issue contributing to significant disability and mortality. The TBI-induced secondary injury cascade, characterized by neuroinflammation, neural cell death, and tissue damage, results in lifelong functional deficits. Alpha-N-[(R)-1, 2-dithiolane-3-pentanoyl]l-glutamyl-l-alanine (CMX-2043) is a novel alpha lipoic acid (ALA)-based therapeutic that has neuroprotective, anti-apoptotic, and anti-inflammatory properties that mitigate cellular, tissue, and functional deficits following TBI. In this study, we evaluated the therapeutic effic
