BACKGROUND: Intrauterine contraception is a widely used, highly effective means of birth control. Uterine perforation is a serious, albeit rare, complication of intrauterine device (IUD) use. Although uterine perforation by levonorgestrel-releasing (20 micro g/day) intrauterine system (LNG-IUS) has already been reported, the peritoneal adhesion potential of this IUD is unknown.

Transjugular intrahepatic portosystemic shunt (TIPS) is a useful procedure for patients with variceal bleeding and refractory ascites. Migration of TIPS can potentially complicate the subsequent transplant procedure. The aim of this study was to compare survival, operating time, and blood transfusion requirements in patients with migrated and nonmigrated TIPS undergoing liver transplantation. We report the case of a 74-year-old man with a colovesical fistula caused by a gallstone that was lost during a laparoscopic cholecystectomy 7 months earlier. The patient was cured after undergoing colonoscopic removal of the stone. To our knowledge this is the first case report of such a complication in the English literature. The report reviews the outcome and complications of retained intraperitoneal gallstones.

A 20-year-old female Ethiopian refugee presented with a 6-month history of cough and progressive left shoulder tip pain. After extensive investigation, which failed to demonstrate a cause, she proceeded to thoracotomy, where a 25-cm length of tubing was found that had perforated the left hemidiaphragm and had extended into the apex of the left lung. This appeared to have arisen as a complication of a termination of pregnancy performed years previously. This represents the first reported case of significant pulmonary trauma arising as a complication of a termination of pregnancy.

HISTORY AND CLINICAL FINDINGS: A 70-year-old male patient had a venous port catheter implanted into his right subclavian vein for neoadjuvant radio-chemotherapy of a rectal carcinoma (T3N0N0). Due to the patient's difficult venous access the catheter was left in situ after treatment. 31 weeks later he was admitted to the hospital because of parasternal and subclavicular pain.

The increased use of phacoemulsification and small incision surgery has led to interest in a soft intraocular lens (IOL) for implantation after cataract extraction. We present the pathologic findings of seven eyes obtained postmortem from five patients with silicone IOLs. Length of implantation ranged from 6 weeks to 13 months. There was symmetric bag-bag fixation in all cases. Two IOLs had distorted and compressed polypropylene loops, resulting in decentration. The silicone IOL appeared to be well tolerated in all cases.

BACKGROUND: Silicone oil tamponade is used in treating retinal detachments, but silicone-associated complications remain frequent. Keratopathy and acute pupillary block glaucoma are related to migration of silicone oil into the anterior chamber. We report the results of using 83 expanding intramedullary rods in 24 children with osteogenesis imperfecta after a mean follow-up of five years three months. In all, 62% of the rods have expanded after one primary operation. Teflon orbital floor implants have become controversial owing to reports of implant-related complications. The results and complications of the use of Bailey-Dubow extensible rods in 28 lower limb bones of 10 patients suffering from osteogenesis imperfecta are reviewed. Over the past eight years at the OMCH Fogarty balloon catheter have been used for one hundred instances of acute arterial occlusion and for twenty instances of venous thrombosis. BACKGROUND: A case of chronic osteomyelitis with Brodie's abscess of the cuboid caused by a wooden foreign body penetrating the plantar foot. Total cuboidectomy was carried out with implantation of an anatomically molded antibiotic-impregnated cement spacer with culture-specific postoperative intravenous antibiotics. At six months of follow-up, the patient was completely asymptomatic without evidence of a recurrence of infection. Final radiographs also didn't show spacer migration or surrounding bone erosions. The spacer obviated the need for any foot fusion which preserved foot biomechanics.
