Fat Grafting for the Treatment of Scleroderma.

BACKGROUND: Scleroderma is a chronic connective tissue disease that results in fibrosis of the skin and internal organs. Although internal organ involvement corresponds with poor prognosis, systemic agents are effective at improving the effects of scleroderma on internal organs. In contrast, skin manifestations are universally present in all patients diagnosed with scleroderma, yet no systemic agents have been shown to be successful. Fat grafting has been shown to improve skin quality and improve contour irregularities and may be helpful in the treatment of patients with scleroderma. METHODS:

Comparison of diagnostic methods for onychomycosis, and proposal of a diagnostic algorithm.

BACKGROUND: Traditionally, the gold standard for diagnosis of onychomycosis has been the combination of direct microscopy with potassium hydroxide (KOH) staining and fungal culture. However, several studies have suggested that periodic-acid-Schiff (PAS) staining of nail-plate clippings may be a very sensitive method for the diagnosis of onychomycosis. AIM: To compare the sensitivities of direct microscopy with KOH, fungal culture and PAS staining of nail-plate clippings, and to define an efficient, high-yield and cost-effective diagnostic strategy for the diagnosis of onychomycosis in the cli

Improvement in onychomycosis after initiation of combined antiretroviral therapy.

BACKGROUND: Onychomycosis is frequent in patients with late and advanced HIV disease; immunocompromised patients may develop atypical clinical presentations that can be difficult to control. Current treatment for onychomycosis is based on the prolonged administration of antifungal therapies that may have significant interactions with combined antiretroviral therapy (cART). An improvement in certain HIV-associated opportunistic infections has been associated with initiation of cART. OBJECTIVES: The aim of this study was to analyze the influence of cART on the outcome of onychomycosis in HIV-in

New alternative in treatment of callus.

The pathological transformation of the skin into a thick and hard callus due to repetitive trauma or friction is commonly known as corn. Although a variety of medical and operative treatment choices have been proposed, an ideal treatment method is yet to be defined. Effectiveness of tangential excision together with topical cantharidin has been evaluated. We used Canthacur-PS as an adjunct to excision in an outpatient setting. Canthacur-PS is a commercially available topical solution that includes 1% cantharidin, 30% salicylic acid and 5% podophyllin. The treatment has been applied to 72 patie

Clinical characteristics and mycology of onychomycosis in autoimmune patients.

Onychomycosis is the most common nail disorder in adults. Predisposing factors are immunosuppression, poor peripheral circulation, diabetes mellitus, increasing age, nail trauma, and tinea pedis. Autoimmune patients, who carry many of these predisposing factors, have never been studied. Autoimmune patients, with underlying autoimmune skin diseases; pemphigus, systemic lupus erythematosus (SLE), mixed connective tissue disease (MCTD), scleroderma, dermatomyositis and cutaneous vasculitis, as well as having abnormal-appearing nail(s) with suspicion of fungal nail infection were included. Modern treatment of warts: cure rates at 3 and 6 months.

Four-hundred consecutive referrals with viral warts of the hands and/or feet were investigated to determine the cure rate from a combination of cryotherapy, keratolytic wart paint and paring. For treatment failures after 3 months, the value of continuing cryotherapy and of additional treatment with the immunomodulator inosine pranobex were assessed. Subjects were treated for 3 months with wart paint and cryotherapy and were randomized to receive, or not, paring in addition. Those who did not respond by 3 months were randomized to receive, or not, 3 months further cryotherapy, and to receive in

Hand Eczema and Self-Reported Work Ability in Middle-Aged Cohort: A Northern Finland Birth Cohort 1966 Study.

BACKGROUND: Hand eczema (HE) can cause itching, fissures and pain, and reduce overall functioning, including the ability to work. However, few studies have examined self-rated work ability in individuals with HE. OBJECTIVES: To investigate self-reported work ability in individuals with and without HE in a middle-aged cohort in the north of Finland. METHODS: As part of the 46-year follow-up visit of a large birth cohort study program, participants were invited to complete an extensive questionnaire, which included questions about their education level, health status and health-related lifesty

Skin Barrier Biomarkers in Patch-Induced and Clinical Allergic and Irritant Contact Dermatitis.

BACKGROUND: Skin barrier impairment is central to irritant (ICD) and allergic contact dermatitis (ACD). Stratum corneum (SC) components cholesterol sulphate (CholSulph), glucosylcholesterol (CholGlc) and natural moisturising factor (NMF) are critical for barrier function, but their changes in ICD and ACD remain underexplored. OBJECTIVES: To measure CholSulph, CholGlc, NMF and IL-1α in patch-induced ICD and ACD and in hand dermatitis (HD) diagnosed as ICD or ACD. METHODS: SC samples were collected from HD patients undergoing patch testing. Biomarkers were analysed in positive reactions to sod

Chronic Hand Eczema Guidelines From an Expert Panel of the International Eczema Council.

BACKGROUND: Assessment of chronic hand eczema (CHE) is complex and warrants standardization. OBJECTIVE: We sought to guide clinicians on the assessment of CHE. METHODS: An electronic questionnaire regarding the diagnosis and assessment of CHE was completed by councilors (n=45) of the International Eczema Council, an international group of clinicians and researchers with expertise in CHE. The survey consisted of 52 statements for consensus. RESULTS: Overall, nine statements (17.3%) had strong, twenty-three (44.2%) moderate, 12 (23.1%) low, and 8 (15.4%) very low levels of agreement. Five sta

DECISA Project (DErmatology Clinics in Italy: Survey on Alitretinoin): A real-life retrospective cohort multicenter study on 438 subjects with chronic hand eczema.

Alitretinoin is the only systemic agent approved to treat moderate-severe chronic hand eczema (CHE) unresponsive to potent topical corticosteroids. No nationwide Italian data regarding real-life efficacy, safety, and tolerability of treatment are available. The DECISA project (DErmatology Clinics in Italy: Survey on Alitretinoin) retrospectively examined data from a registry including 15 Dermatology Clinics authorized to prescription of alitretinoin for CHE patients. Disease severity was assessed at baseline, and after 3 and 6 months of treatment, using the 5-point Physician Global Assessment
