The self-reinforcing cycle of financial toxicity in Epilepsy: an evolutionary concept analysis.

Epilepsy is a neurological condition that affects the entire household through its chronic course, lifelong medication requirements, and intensive caregiving demands. Financial toxicity, a concept originating in oncology, describes the harm that arises when the costs of care extend beyond the depletion of household resources to affect patients' and families' quality of life, psychological wellbeing, and treatment decisions. Despite these characteristics, financial toxicity in epilepsy has not yet been conceptualized as a unified clinical phenomenon; instead, research has remained fragmented ac

Living with the Dead: Mad or Bad? Five paradigmatic cases of unprocessed grief.

The death of a loved one is an event with the highest imbalance for all individuals. Although it is typically a painful but normal process, in specific vulnerable subjects, it may evolve into maladaptive trajectories, culminating in extreme behaviors that became the objective of interdisciplinary studies. This paper analyzed five cases focused on the continuation of cohabitation between survivors and corpse. This unconventional behavior draws its origins from the co-presence of three pathological elements: psychiatric disorders, fragile personality structures, and dysfunctional family systems.

Are they looking for support? Mental health and trauma awareness as predictors of mental health help-seeking behaviour among young adults and adolescents in Bangladesh, Egypt, Ghana and the United Arab Emirates.

BACKGROUND: Mental health is increasingly recognized worldwide as a significant public health challenge. However, progress in developing interventions to address mental health burdens, particularly in non-Western countries, has been remarkably slow. This stagnation may be attributed to limited research on the underlying causes of mental health issues, such as trauma, as well as a lack of awareness regarding mental health disorders and help-seeking behaviours. The current study seeks to mainstream the understanding of mental health disorders by investigating the impact of trauma awareness and m

Prevalence and Factors Associated With Depression and Depressive Symptoms Among Chinese Older Persons: An Integrative Review.

China is the country with the largest population of older persons. Depression is the most common mental health issue among older adults, a trend expected to increase as societies continue to age. With the global increase in depression and depressive symptoms among this demographic, the resulting disease burden poses a significant challenge to health and social care systems in China. To map, summarise and examine the empirical literature on the prevalence and factors associated with depression and depressive symptoms in Chinese older adults, an integrative literature review was conducted inform

Single-cell analysis reveals cellular heterogeneity, gene expression profiles, and pathway dynamics in acne vulgaris.

Acne vulgaris is a common dermatological condition, particularly in adolescents, and is associated with significant physical and psychological impacts. Its pathogenesis involves genetic factors, sebaceous gland dysfunction, inflammatory responses, and alterations in skin microbiota. Despite advancements in treatment, a comprehensive understanding of its cellular and molecular mechanisms remains limited, making therapeutic strategies challenging. Single-cell RNA sequencing (scRNA-seq) data from six acne vulgaris patients were obtained from the GEO database (GSE175817), filtered, and integrated.

Preoperative intrinsic capacity phenotypes forecast adverse physical resilience trajectories following surgery in older bladder cancer patients.

PURPOSE: This study assessed the predictive value of preoperative intrinsic capacity (IC) phenotypes for physical resilience (PR) trajectories in older adults undergoing elective bladder cancer surgery. METHODS: A prospective study was conducted from October 2024 to March 2025 on 327 elderly patients (mean age 69.71 ± 6.96 years) who underwent elective bladder cancer surgery. Latent class analysis (LCA) was used to identify different intrinsic capacity types based on the five domains: vitality, locomotion, cognition, psychology, and sensory. Latent class growth model (LCGM) was applied to ide

German National Guidelines for Epidural Spinal Cord Stimulation for the Treatment of Chronic Pain.

INTRODUCTION: Epidural spinal cord stimulation (SCS) or dorsal root ganglion stimulation (DRG-S) is a neuromodulatory, reversible interventional procedure for patients with pain who cannot be adequately treated with conservative and less invasive means. This review provides the recently updated German recommendations for SCS and DRG-S, followed by a standardized diagnostic and therapeutic procedure. OBJECTIVE: The purpose of this work is to present the current evidence-based results of SCS, including DRG-S, for various neuropathic and nonneuropathic pain conditions. Moreover, it aims to optim

Development and validation of a risk prediction model for co-existing depression in middle-aged and older adults with low back pain.

BACKGROUND: Low back pain (LBP), a prevalent disorder among middle-aged and older adults, imposes substantial medical and socioeconomic burdens on individuals and society. Worse still, comorbid depression is frequently present in chronic LBP, which aggravates the functional prognosis. It is of significance to identify relevant predictors associated with the occurrence of depression in LBP. Therefore, this study developed a risk prediction model to estimate depression risk in LBP patients, which may support its early detection and intervention. METHODS: This study used representative data from

Editorial Commentary: Preoperative Mental Health Screening, Early Identification, and Use of Multidisciplinary Patient-Specific Care May Help Improve Postoperative Outcomes in Patients With Mental Health Disorders Undergoing Anterior Cruciate Ligament Reconstruction.

Within the field of sports medicine, it is estimated that more than 200,000 anterior cruciate ligament tears occur every year and nearly one half of these patients will undergo anterior cruciate ligament reconstruction (ACLR) surgery. Although ACLR remains the gold standard for treatment and has demonstrated widely successful outcomes over the years, a small percentage of patients will experience postoperative complications such as instability, persistent knee pain, osteoarthritis, graft rupture, infection, and/or knee stiffness that may impact function and quality of life. Despite the overall

Quantitative assessment of self-management in patients with non-alcoholic fatty liver disease: An unmet clinical need.

In this editorial we comment on the article titled "Establishment and validation of an adherence prediction system for lifestyle interventions in non-alcoholic fatty liver disease" by Zeng et al published in a recent issue of the World Journal of Gastroenterology. Non-alcoholic fatty liver disease (NAFLD) represents one of the current challenges in hepatology and public health, due to its continuous growing prevalence and the rising incidence of NAFLD-related fibrosis, non-alcoholic steatohepatitis and cirrhosis. The only effective therapeutic strategy for this disease is represented by encour

BACKGROUND: Management of cirrhosis is challenging and has been complicated by the COVID-19 pandemic due to decreased access to care, increased psychological distress, and alcohol misuse. Recently, The National Institute on Alcohol Abuse and Alcoholism has broadened the definition of recovery from alcohol use disorder to include quality of life (QoL) as an indicator of recovery. This study examined the associations of alcohol-associated cirrhosis etiology and problematic drinking with liver disease QoL (LDQoL). METHODS: Patients with cirrhosis (N=329) were recruited from 3 sites (63% from 2 V

BACKGROUND: The implementation of patient-reported outcome measures (PROMs) in the clinical identification of immunotherapy toxicities is a complex intervention. There has been very little work evaluating the clinical utility and generalisability of PROMs used after immune checkpoint inhibitor (ICI) treatment to date. We reviewed evidence on the use of PROMs assessing toxicities and health-related quality of life in patients treated with ICIs. METHODS: PubMed, EMBASE, MEDLINE, PsycInfo, CINAHL, Web of Knowledge, the Cochrane Library were searched (January 2008 - October 2024). Quantitative st

Predictive Factors for Poor Prognosis in Non-Surgical Treatment of Proximal Humerus Fractures in Elderly Patients.

INTRODUCTION: Proximal humerus fractures (PHF) are common in the elderly, accounting for significant morbidity and mortality. Non-surgical treatment is a common option for low-demand elderly patients, but it can lead to unsatisfactory functional outcomes in some cases. The identification of predictive factors for poor prognosis in non-surgical management remains unclear. This study aimed to determine the predictive factors for poor prognosis in elderly patients treated non-surgically for displaced PHF and to assess associated complications. METHODS: A retrospective cohort study was conducted

Italian Expert Opinion on Chronic Hand Eczema: from Guidelines to Clinical Practice.

INTRODUCTION: Chronic hand eczema (CHE) is an inflammatory skin condition characterized by different pathomechanisms, clinical presentations, and prognoses. Treatment is often challenging because of limited approved drugs, and severe CHE is associated with reduced quality of life (QoL) and poor overall health measures in terms of psychological, functional, and occupational challenges. This study aims to describe the real-life management practices of Italian dermatologists who frequently treat patients with CHE, compare these practices with existing guidelines, and propose practical clinical re

Scoping review and expert-based consensus recommendations for assessment and management of psychogenic non-epileptic (functional) seizures (PNES) in children: A report from the Pediatric Psychiatric Issues Task Force of the International League Against Epilepsy.

Limited guidance exists regarding the assessment and management of psychogenic non-epileptic seizures (PNES) in children. Our aim was to develop consensus-based recommendations to fill this gap. The members of the International League Against Epilepsy (ILAE) Task Force on Pediatric Psychiatric Issues conducted a scoping review adhering to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-SR) standards. This was supplemented with a Delphi process sent to pediatric PNES experts. Consensus was defined as ≥80% agreement. The systematic sea
