Patellar instability is a common knee disorder encountered in young athletes. Patients with normal osseous anatomy and mechanical alignment of the lower extremity are candidates for soft-tissue reconstructive procedures. The success rate of the operation of lateral release for pain caused by the patella is reported as being between 14% and 99%. The choice between arthroscopic or open procedures does not seem to affect the results. Bilateral rupture of the infrapatellar tendons is a rare event in an otherwise healthy athletic individual. The event is usually quite severe and diagnosis can be readily made on a clinical examination. In the case presented here, predisposing factors consisted of an underlying patella alta and a long history of repetitive trauma to the knee. Prompt diagnosis and surgical repair will lead to a good functional result.

The efficiency of a knee brace with a 0 degree-30 degrees range of motion was studied in 17 patients with anterior knee pain. Eighteen patients with identical symptoms served as controls. BACKGROUND: Management of patellofemoral instability is complex. It is not known whether the outcomes of medial patellofemoral ligament reconstruction (MPFL) with concomitant tibial tubercle osteotomy (TTO) performed in the revision setting after a failed surgery for patellar instability equal the outcomes of an MPFL-Reconstruction +TTO performed as a primary procedure.
HYPOTHESIS: Both groups would have low recurrent instability rates, significantly improved subjective outcomes, and return to sport (RTS) percentages equivalent to or higher than those currently established in the literature.
