Intestinal ischemia: current treatment concepts.

PURPOSE: Mesenteric ischemia is a condition well-known among physicians treating patients with abdominal symptoms. Even so, mortality rates have not decreased significantly over the last decades. The purpose of this article is to review current treatment concepts of acute and chronic mesenteric ischemia. RESULTS: Early diagnosis is one of the most important features that determine a patient's prognosis. Conventional angiography and multidetector computed tomography are therefore appropriate to quickly diagnose mesenteric ischemia, the latter being commonly more available. Once a patient prese…

Unregistered subtracted CT angiography for the visualization of intracranial arteries at or near the skull base: preliminary experience.

PURPOSE: Restrictions with computed tomography angiography (CTA) regarding the visualization of arteries near the skull base are well known. Today, the gold standard for overcoming this is the matched mask bone elimination method. Worldwide use of this method is limited since it requires advanced imaging physics and software. A more simple method was introduced recently that avoided motion artifacts significantly by restraining the patient's head with a vacuum-type head holder. The purpose of this study was to investigate the feasibility of using unregistered subtracted CTA without such head-h

Automatic quantification of the myocardial extracellular volume by cardiac computed tomography: Synthetic ECV by CCT.

BACKGROUND: The quantification of extracellular volume fraction (ECV) by Cardiac Computed Tomography (CCT) can identify changes in the myocardial interstitium due to fibrosis or infiltration. Current methodologies require laboratory blood hematocrit (Hct) measurement - which complicates the technique. The attenuation of blood (HUblood) is known to change with anemia. We hypothesized that the relationship between Hct and HUblood could be calibrated to rapidly generate a synthetic ECV without formally measuring Hct. METHODS: The association between Hct and HUblood was derived from forty non-con

Quantification of orbital apex crowding for screening of dysthyroid optic neuropathy using multidetector CT.

BACKGROUND AND PURPOSE: DON, a serious complication of GO, is frequently difficult to diagnose clinically in its early stages because of confounding signs and symptoms of congestive orbitopathy. We evaluated the ability of square area measurements of orbital apex crowding, calculated with MDCT, to detect DON. MATERIALS AND METHODS: Fifty-six patients with GO were studied prospectively with complete neuro-ophthalmologic examination and MDCT scanning. Square measurements were taken from coronal sections 12 mm, 18 mm, and 24 mm from the interzygomatic line. The ratio between the extraocular musc

Inter-rater and intra-rater reliability of multi-slice CT and three-dimensional reconstructed imaging analysis of mesenteric vascular anatomy for planning and performing complete mesocolic excision.

AIM: Complete mesocolic excision (CME) for colon cancer has been associated with improved oncological outcomes but requires a detailed understanding of complex mesenteric vasculature. Three-dimensional (3D) reconstructed models derived from patient imaging could enhance preoperative anatomical comprehension, enabling safer, precision CME. METHODS: In this two-phase, blinded, crossover study, four expert CME surgeons evaluated mesenteric vascular anatomy on CT scans and 3D models. In phase 1, surgeons assessed 66 cases, while 20 were re-evaluated in phase 2. The primary outcome measure was int

CT annulus sizing prior to transcatheter aortic valve replacement (TAVR): evaluation of free-breathing versus breath-holding acquisition.

OBJECTIVES: To assess feasibility and accuracy of aortic annulus measurements using cardiac computed tomography angiography (CTA) performed during free-breathing prior to transcatheter aortic valve replacement (TAVR). MATERIALS AND METHODS: Sixty consecutive TAVR candidates underwent free-breathing wide-detector cardiac CTA, followed by a percutaneous valve replacement. For each, a theoretical valve size was suggested through CT measurements of the annulus, then compared to the size of the actual implanted transcatheter heart valve (THV). The procedural success and the 30-day outcomes were co

Imaging-based analysis of right inferior accessory hepatic vein variations in living donors: relevance to operative decision-making and graft outcomes.

AIM: Variations in the anatomy of the right inferior accessory hepatic vein (IRHV), particularly in their presence and size are known to influence hepatic venous dominance and the need for middle hepatic vein (MHV) reconstruction. Understanding these variations is crucial for optimising surgical planning and ensuring graft survival. This study aims to analyse IRHV variations using multidetector computer tomography (MDCT) in live liver donors and assess their impact on hepatic venous dominance and the need for MHV reconstruction to mitigate posttransplant complications. MATERIALS AND METHODS:

Transcatheter Aortic Valve Replacement With Self-Expanding Valves in Severe Pure Native Aortic Regurgitation-Is It Still Anchored Stably in a Significantly Dilated Ascending Aorta Population?

BACKGROUND: The non-coronary sinus pivot implantation (NCPI) technique has notably improved procedural success rate in transcatheter aortic valve replacement (TAVR) for cases of severe pure native aortic regurgitation (PNAR) when utilizing self-expanding transcatheter heart valves (THVs). Nonetheless, the intrinsic anchoring mechanism remains obscure. AIMS: The aim of this study was to assess the feasibility of implementation of the NCPI method using self-expanding THVs for TAVR in severe PNAR and dilated ascending aortas (AAOs), and to elucidate the anchoring mechanism of the NCPI. METHODS:

Imaging in traumatic mandibular fractures.

A fracture of the mandible is a common trauma presentation amongst young males and represents one of the most frequently encountered fractured bones within the viscerocranium. Historically, assault was the dominant contributing factor but now due to the increased number of vehicles used per capita, motor vehicle accidents are the primary cause. Mandibular fractures can be classified anatomically, by dentition, by muscle group and by severity. The fracture may also be closed, open, comminuted, displaced or pathological. It is important that the imaging modality used identifies the classificatio
