Improving PICC Placement Using Bedside Ultrasound in a Level IV NICU: A Quality Improvement Project.

BACKGROUND: Accurate peripherally inserted central catheter (PICC) tip placement in neonates is critical to prevent complications and minimize repeated radiographic exposure. Reliance on radiography may delay confirmation and increase radiation risk in this vulnerable population. PURPOSE: To evaluate the impact of a standardized point-of-care ultrasound (POCUS) protocol on PICC placement accuracy, radiographic utilization, and procedural efficiency in a Level IV neonatal intensive care unit (NICU). METHODS: A longitudinal pre-post quality improvement design compared early (March-May 2025) an

Single Persistent Left Superior Vena Cava Draining Into the Left Atrium Causing Neonatal Cyanosis: A Case Report.

BACKGROUND: The persistence of the left superior vena cava (PLSVC) is the most common variant of systemic venous drainage. It is usually diagnosed through echocardiography, with the PLSVC typically draining into the right atrium via the coronary sinus without hemodynamic consequences. In cases of a single PLSVC (without right superior vena cava), it generally drains into the right atrium. However, when it drains directly into the left atrium, systemic venous return bypasses the right atrium, causing cyanosis and hypoxemia. CLINICAL FINDINGS: Term newborn was admitted to the neonatal intensive

Congenital Genu Recurvatum: Report of Two Cases.

BACKGROUND: Congenital hyperextension of the knee joint is a rare malformation with a variety of names, principally genu recurvatum of knee, backward bending of the knee, and dislocation of knee. Abnormal anterior hyperextension position and limitation of flexion of tibiofemoral joint are the classical clinical presentation in an infant at birth. The accurate etiology of the malformation is undetermined. Congenital genu recurvatum (CGR) can occur as an isolated entity or can present in association with other congenital abnormalities like talipes equinovarus, developmental dysplasia of the hip,

Iron-Clad Care for Neonates: Precision Iron Supplementation Using Reticulocyte Hemoglobin Values, a Quality Improvement Project.

BACKGROUND: Iron is the most common nutrient deficiency globally, affecting nearly 2 billion people. It may cause long-term neurocognitive impairments and behavioral deficits in premature and low-birthweight neonates. Reticulocyte hemoglobin (Ret-He) is a reliable, valid, low-cost, and minimally invasive phlebotomy laboratory test for assessing iron status and guiding iron dosing. PURPOSE: The overarching aim is to standardize Ret-He use as a guide for precision iron supplementation that may increase the rate of iron sufficiency among infants born at less than 32 weeks of gestational age or 1

Evaluating the Interrater Reliability of the COMFORTneo Scale in Infants: Influence of Gestational Age and Rater Experience.

BACKGROUND: Pain assessment in preterm infants is challenging because of the absence of an objective reference standard, as self-report is not possible. Behavioral tools, including the COMFORTneo scale, are used in NICUs to assess pain and distress. Although interrater reliability (IRR) is generally adequate, its variation across gestational ages and rater experience remains unclear. PURPOSE: To examine the influence of gestational age and rater experience on the interrater reliability (IRR) of the COMFORTneo scale (primary aim) and to evaluate overall IRR (secondary aim). METHODS: We conduc

Impact of a Sedation Reduction Protocol in Infants Undergoing MRI Scanning.

BACKGROUND: Brain magnetic resonance imaging (MRI) is an important diagnostic tool for infants with possible brain abnormalities. While sedation may be necessary for high quality images, it carries risks of complications. The sedation rate for MRI procedure varies widely, ranging from 0% to 100%, influenced by infant characteristics and institutional practices, with an increasing focus on non-sedated or minimally sedated approaches to reduce risks. PURPOSE: We studied sedation utilization in infants undergoing MRI scanning before and after implementing an MRI bundle. METHODS: This cohort stu

An Examination of Acoustic Neuroprotection in the Neonate: A Systematic Review.

BACKGROUND: Hearing loss in neonates is more than 100 times that of the general pediatric population. Research suggests that there is a large amount of toxic noise exposure for these neonates in the neonatal intensive care unit and in the transport environments. Thus, acoustic neuroprotection, or auditory protection and support of neurodevelopment, is an important concept, particularly for neonates. PURPOSE: A systematic review of the literature was conducted to determine the possibility of successfully supporting acoustic neuroprotection and decreasing the ambient noise level to the recommen

Maternal Perceptions of Feeding Behaviors in Infants With Neonatal Opioid Withdrawal Syndrome.

BACKGROUND: Neonatal opioid withdrawal syndrome (NOWS) significantly affects newborns, particularly when it comes to feeding. This underscores the need for comprehensive strategies to support growth, development, and the mother-infant bond. Despite the prevalence of NOWS, evidence-based feeding guidelines are scarce, and maternal perceptions of feeding behaviors remain underexplored, limiting tailored interventions. PURPOSE: Explore maternal perceptions of infant feeding behaviors in infants with NOWS. METHODS: This pilot, qualitative descriptive study used data from semi-structured intervie
