Transforming Hematology Care in Africa: Strategies for Enhanced Diagnosis and Treatment.

BACKGROUND: Hematologic disorders contribute significantly to morbidity and mortality across Africa. Challenges include delayed diagnosis, limited access to essential therapies, workforce shortages, and fragmented blood transfusion services. Although global advances in diagnostics and therapeutics exist, their translation into African contexts remains inconsistent. OBJECTIVE: To provide a comprehensive narrative review of the current state of hematology care in Africa, highlighting diagnostic and treatment gaps, country-level experiences, and evidence-informed strategies for improving patient

Creating "Safer Ground": implementation of a service-rich housing program for people experiencing homelessness during the COVID-19 pandemic.

BACKGROUND: Operation Safer Ground (OSG) was created in response to the COVID-19 pandemic. Without safe locations to shelter in place, people experiencing homelessness (PEH) were especially vulnerable to COVID-19. In March 2020, California's Governor signed an executive order that provided local governments with flexible emergency funding to develop "immediate solutions" to mitigate COVID-19 infection risk among PEH. Alameda County Health used those funds to create OSG, a robust and expansive program to address not only COVID-19 risk among PEH, but also the larger health and housing challenges

Effectiveness of One Health approach for control of Kyasanur Forest Disease in Wayanad, Kerala, India.

BACKGROUND & OBJECTIVES: Kyasanur Forest Disease (KFD) is a vector borne haemorrhagic fever that is endemic in the Wayanad region located in Northern part of Kerala, India. The region is managing the outbreak well ever since the major epidemic of 2015. This was because of the successful implementation of One Health (OH) initiative concentrating on multisectoral collaboration between regional institutions involved in public, animal and environmental health domains. The article presents how OH was implemented for the first time in the district in the year 2015 and evaluates the degree OH-ness of

Cell dynamics and immune response to BLV infection: a unifying model.

INTRODUCTION: In countries such as South Africa with a high prevalence of HIV and TB policy directives support program integration. Operational research suggests this is desirable, at least for increasing coverage of HIV and TB services, but warns that implementation models must take local health service infrastructure into account. METHODS: A program evaluation of HIV and TB prevention and therapeutic services was performed at facility level in two purposefully selected districts in South Africa - one deep rural and an urban district - in order to describe integration and how it is implement

Factors associated with quality of life among older persons in urban and rural settings: a cross-sectional population-based study.

INTRODUCTION: Quality of life (QoL) is a core aspect of healthy ageing, reflecting the ability of older persons to adapt and maintain well-being in later life. Urban–rural differences pose distinct challenges, with urban areas facing overcrowding and high costs, whereas rural areas lack healthcare and infrastructure, necessitating targeted public health strategies. This study identified factors associated with QoL among older Malaysians, with analyses stratified by urban and rural areas. METHODS: This cross-sectional study analysed data from the National Health and Morbidity Survey 2018: Elder

Implementing a multi-centric cohort study during the SARS-CoV-2 pandemic in a resource-constrained setting: challenges, adaptive solutions, and lessons from Brazil.

BACKGROUND: Conducting multicenter, longitudinal epidemiological studies on emerging pathogens in real time during health emergencies presents significant challenges, especially in low- and middle-income countries, where substantial logistical and resource constraints hinder the rapid design and implementation of research protocols. This is particularly difficult when the disease spreads rapidly and its nature changes due to the onset of vaccination. METHODS: During the COVID-19 pandemic, a cohort study (AVISA) was conducted in eleven research sites to assess seroprevalence. The objective of t

Low physical activity-related disease burden, 1990-2021: assessment of global trends and social determinants based on GBD 2021 data.

BACKGROUND: Low physical activity (LPA) is associated with cardiovascular and cerebrovascular pathologies. This study aimed to assess the prevalence of several noncommunicable diseases relating to LPA. METHODS: Using the 2021 Global Burden of Disease data set, we modelled LPA-related disease burdens across 204 countries and territories, quantifying mortality counts, age-standardised mortality rates, and disability-adjusted life years (DALYs) for five noncommunicable diseases. We conducted multivariable stratification analyses to assess variations by gender, age, and sociodemographic index (SD

Generation Victoria (GenV): protocol for a longitudinal birth cohort of Victorian children and their parents.

BACKGROUND: In a world confronted with new and connected challenges, novel strategies are needed to help children and adults achieve their full potential, to predict, prevent and treat disease, and to achieve equity in services and outcomes. Australia's Generation Victoria (GenV) cohorts are designed for multi-pronged discovery (what could improve outcomes?) and intervention research (what actually works, how much and for whom?). Here, we describe the key features of its protocol. METHODS: GenV is a whole-population longitudinal platform open to all ≈150,000 children born within a two-year wi

Integrating HIV prevention services into care settings for people with opioid use disorder (OUD): a study protocol for implementation strategy development and modeling.

BACKGROUND: The overlapping epidemics of opioid use disorder (OUD) and HIV present a critical public health challenge. Although people with OUD frequently engage with healthcare settings, uptake of HIV prevention services such as pre-exposure prophylaxis (PrEP) remains low. Integrating HIV prevention into routine OUD care could reduce new infections, but scalable, evidence-based strategies are lacking. Rhode Island offers a unique opportunity to design and evaluate such strategies using its robust data infrastructure and high OUD burden. METHODS: We will conduct a three-phase, sequential impl

Predicting determinants of unimproved water supply in Ethiopia using machine learning analysis of EDHS-2019 data.

Over 2 billion people worldwide are impacted by the global dilemma of access to clean and safe drinking water. The problem is most acute in low-income nations, where many people still use unimproved water sources such as exposed wells and surface water. Public health systems are heavily burdened by these sources since they are closely associated with the spread of waterborne illnesses. As a result, there are still many people who suffer from water-related health problems, especially in underdeveloped nations where access to healthcare is limited and sanitation is often inadequate. However, the

SAR317461 Inhibit JAK2/STAT3 Phosphorylation for Enhanced Anti-leukemia Activity.

Breast cancer is commonly diagnosed at late stages in countries with limited resources. Efforts aimed at early detection can reduce the stage at diagnosis, potentially improving the odds of survival and cure, and enabling simpler and more cost-effective treatment. Early detection of breast cancer entails both early diagnosis in symptomatic women and screening in asymptomatic women. Key prerequisites for early detection are ensuring that women are supported in seeking care and that they have access to appropriate, affordable diagnostic tests and treatment. We therefore propose the following seq

Modeling Hepatitis C Elimination Among People Who Inject Drugs in New Hampshire.

IMPORTANCE: The success of direct-acting antiviral therapies for chronic hepatitis C virus (HCV) infection led the World Health Organization to set elimination targets by 2030. For the United States to achieve these benchmarks, public health responses must target high-risk populations, such as people who inject drugs (PWID), a group with high rates of HCV incidence and low rates of treatment uptake. OBJECTIVE: To evaluate potential improvements in the HCV care cascade among PWID, focusing on improved testing, treatment uptake, and access to harm reduction. DESIGN, SETTING, AND PARTICIPANTS:

Rapid Cas13a-based penA genotyping for cefixime susceptibility in Neisseria gonorrhoeae.

UNLABELLED: Antimicrobial resistance in Neisseria gonorrhoeae is an urgent public health threat. Resistance-guided therapy can assure appropriate treatment and reintroduce alternative therapeutic options by identifying genetic predictors of resistance. Mosaicism at codons 375-377 of the penA gene is associated with cefixime resistance. Rapid, field-deployable assays for predicting cefixime susceptibility are lacking. We used a machine-learning algorithm to develop a CRISPR Cas13a-based assay to detect the absence of mosaicism at codons 375-377 of the penA gene, combined with isothermal amplifi

Data-Driven Identification of Factors That Influence the Quality of Adverse Event Reports: 15-Year Interpretable Machine Learning and Time-Series Analyses of VigiBase and QUEST.

BACKGROUND: The completeness of adverse event (AE) reports, crucial for assessing putative causal relationships, is measured using the vigiGrade completeness score in VigiBase, the World Health Organization global database of reported potential AEs. Malaysian reports have surpassed the global average score (approximately 0.44), achieving a 5-year average of 0.79 (SD 0.23) as of 2019 and approaching the benchmark for well-documented reports (0.80). However, the contributing factors to this relatively high report completeness score remain unexplored. OBJECTIVE: This study aims to explore the ma

Convergence in Mobility Data Sets From Apple, Google, and Meta.

BACKGROUND: The higher movement of people was one of the variables that contributed to the spread of the infectious agent SARS-CoV-2 during the COVID-19 pandemic. Governments worldwide responded to the virus by implementing measures that would restrict people's movements, and consequently, the spread of the disease. During the onset of the pandemic, the technology companies Apple, Google, and Meta used their infrastructure to anonymously gather mobility reports from their users. OBJECTIVE: This study aims to compare mobility data reports collected by Apple, Google, and Meta (formerly Facebook

Leveraging community insights and navigating logistical challenges: a case study of the 2024 polio vaccination campaign in Gaza, State of Palestine.

This case study describes how high vaccination coverage was achieved during the 2024 polio vaccination campaign in Gaza amid an ongoing humanitarian crisis marked by damaged critical infrastructure, an obstruction of the entry of health supplies, mass displacement and security concerns for health workers. Despite the immense challenges, 559 161 children were vaccinated in the first round (94% of the revised target of 591 714) and 556 774 in the second round, exceeding expectations. While this represents an impressive achievement, some areas remained inaccessible, which prevented an estimated 7

The impact of donor transition on continuity of maternal and newborn health service delivery in Rwenzori sub-region of Uganda: a qualitative country case study analysis.

BACKGROUND: The transition of donor-supported health programmes to country ownership is gaining increasing attention due to reduced development assistance for health globally. It is further accelerated by the ineligibility of previously Low-Income Countries' elevation into Middle-income status. Despite the increased attention, little is known about the long-term impact of this transition on the continuity of maternal and child health service provision. Hence, we conducted this study to explore the impact of donor transition on the continuity of maternal and newborn health service provision at
