No Difference in Clinical Outcomes for Arthroscopic Suprapectoral Versus Open Subpectoral Biceps Tenodesis at Midterm Follow-up: A Randomized Prospective Analysis.

BACKGROUND: We have previously reported the 1-year outcomes of arthroscopic suprapectoral biceps tenodesis (ASPBT) versus open subpectoral biceps tenodesis (OSPBT) for the management of long head of the biceps tendon (LHBT) pathology. While patients had similar 1-year biceps muscle strength and pain, longer-term functional outcomes are unknown. PURPOSE: To directly compare clinical outcomes of ASPBT versus OSPBT with interference screw fixation, distal to the bony bicipital groove, at a minimum of 2 years' follow-up. STUDY DESIGN: Randomized controlled trial; Level of evidence, 1. METHODS:

Editorial Commentary: Increased Graft Thickness With Superior Capsular Reconstruction Results in Improved Acromiohumeral Distance, but Increased Graft Tear Rate and Lateral Acromial Erosion.

For the treatment of irreparable posterosuperior rotator cuff tears of the supraspinatus and infraspinatus, superior capsular reconstruction (SCR) has continued to supplant other less successful and/or reproducible techniques, including partial rotator cuff repair and latissimus tendon transfer. After its initial description utilizing a folded fascia lata autograft with a thickness of up to 8 mm, many in North America adapted their surgical technique to use commercially available human dermal allograft to mitigate donor site morbidity. Early series have revealed the importance of graft thickne

Arthroscopic Superior Capsular Reconstruction for Dislocation of the Shoulder with an Irreparable Rotator Cuff Tear: A Case Report.

CASE: A 49-year-old man presented with apprehension of a repeat dislocation after a left shoulder dislocation, which was accompanied by pain and restricted active range of motion of the shoulder. We performed an arthroscopic Bankart repair as well as an arthroscopic superior capsular reconstruction (ASCR) with fascia lata for the irreparable massive rotator cuff tear. Two years postoperatively, there was no shoulder pain or repeat dislocation; the range of motion had improved. CONCLUSION: We regard ASCR in combination with a Bankart repair as a useful treatment for recurrent shoulder dislocat

Shoulder Synovial Fluid Lipoprotein Levels and Their Relationship to the Rotator Cuff.

INTRODUCTION: Rotator cuff pathology has been proposed to occur through intrinsic and extrinsic mechanisms. Hyperlipidemia has been proposed as a mechanism of intrinsic rotator cuff pathology. This prospective observational study evaluates serum and synovial lipid profiles in patients with and without rotator cuff tears to further define the relationship of cholesterol and rotator cuff pathology. METHODS: Patients were prospectively enrolled with intact rotator cuff (37 patients) and rotator cuff tear requiring a repair (40 patients) groups. Exclusion criteria were medication for hypercholest

Nature abhors a vacuum: bilateral prominent rotator cable in bilateral congenital absence of the long head of the biceps tendon.

Bilateral agenesis of the long head of the biceps tendon is an exceedingly rare anomaly and can be a challenging diagnostic dilemma whose differential diagnosis includes tear. We present the third case of bilateral agenesis of the long head of the biceps tendon. Absent or shallow intertubercular sulcus is a constant finding and serves to differentiate this entity from tear. Our case is unique in that there was a radial ray anomaly, where prior reports of bilaterality did not demonstrate associated congenital anomalies. We also describe the newly reported finding of thickening of the rotator ca

Stress distribution in the supraspinatus tendon with partial-thickness tears: an analysis using two-dimensional finite element model.

The purpose of this study was to clarify the stress distribution in the rotator cuff tendon with 3 types of partial-thickness tears by use of 2-dimensional finite element analysis. In the finite element model of the normal human shoulder, a tendon defect was created on the articular surface, on the bursal surface, or in the midsubstance close to the insertion. A tensile force was applied to the proximal end of the tendon, and the stress distribution was calculated. In all 3 types of tears, a high stress concentration appeared around the articular surface at the insertion and at the site of the

Reduced glenohumeral rotation, external rotation weakness and scapular dyskinesis are risk factors for shoulder injuries among elite male handball players: a prospective cohort study.

AIM: To determine whether rotator cuff strength, glenohumeral joint range of motion and scapular control are associated with shoulder injuries among elite male handball players. METHODS: A total of 206 players in the Norwegian elite handball league for men were tested prior to the 2011-2012 season. Measures included: (1) glenohumeral internal and external rotation range of motion, (2) isometric internal rotation, external rotation and abduction strength and (3) assessment of scapular dyskinesis. Players were followed prospectively for the entire regular season (30 weeks), with shoulder proble

Humeral Avulsion of Glenohumeral Ligament Lesion - A Case Report and Surgical Considerations.

INTRODUCTION: Humeral avulsion of the glenohumeral ligament (HAGL) is a rare but important cause of shoulder instability, often underdiagnosed due to subtle imaging findings and associated common injuries like rotator cuff tears, resulting in persistent pain, recurrent instability, and failure of conventional stabilization procedures. CASE REPORT: We present a case of a 48-year-old male who sustained an anterior HAGL lesion with an associated subscapularis tear following a traumatic anterior shoulder displacement. Magnetic resonance imaging and arthroscopic visualization confirmed the diagnos

Large animal models for the study of tendinopathy.

Tendinopathy has a high incidence in athletes and the aging population. It can cause pain and movement disorders, and is one of the most difficult problems in orthopedics. Animal models of tendinopathy provide potentially efficient and effective means to develop understanding of human tendinopathy and its underlying pathological mechanisms and treatments. The selection of preclinical models is essential to ensure the successful translation of effective and innovative treatments into clinical practice. Large animals can be used in both micro- and macro-level research owing to their similarity t

Extracorporeal Shockwave Therapy in the Management of Sports Medicine Injuries.

Treatment of musculoskeletal conditions in athletes with extracorporeal shockwave therapy (ESWT) is gaining popularity as greater evidence supports its use. ESWT protocols (describing energy flux density, number of impulses, type of shockwave (focused or radial), number/frequency/duration of treatment session, area of application, and postprocedural therapy protocols) can be adjusted in the clinical setting. Protocols vary across studies, and optimal protocols for most indications are yet to be determined. ESWT can safely be used to treat various musculoskeletal conditions in athletes, includi

Management of malunion of the proximal humerus: current concepts.

Proximal humerus fractures remain one of the most common orthopaedic injuries, particularly in the elderly. Displaced fractures often require surgery, and management can be challenging because of comminution and poor bone quality. Despite advances in surgical technique and implant design, reoperation for malunion or nonunion of the tuberosity (arthroplasty) or screw penetration (open reduction and internal fixation) remains problematic. Recent studies have demonstrated acceptable results following nonsurgical management of displaced proximal humerus fractures in elderly, low-demand patients. I

The deltoid, a forgotten muscle of the shoulder.

The deltoid is a fascinating muscle with a significant role in shoulder function. It is comprised of three distinct portions (anterior or clavicular, middle or acromial, and posterior or spinal) and acts mainly as an abductor of the shoulder and stabilizer of the humeral head. Deltoid tears are not infrequently associated with large or massive rotator cuff tears and may further jeopardize shoulder function. A variety of other pathologies may affect the deltoid muscle including enthesitis, calcific tendinitis, myositis, infection, tumors, and chronic avulsion injury. Contracture of the deltoid

Shoulder pain: a diagnostic dilemma.

The etiology of shoulder pain can usually be placed into one of five categories: fracture and/or contusion, shoulder separation involving the clavicle, instability of the glenohumeral joint, impingement syndrome involving the rotator cuff or biceps tendinitis, and frozen shoulder. Non-shoulder pathology, such as cervical strain, thoracic outlet syndrome and referred pain from phrenic nerve irritation, should be excluded when evaluating patients presenting with shoulder pain. After inflammatory or infectious arthritis has been ruled out, the history should point to either acute trauma or an ove

Superior labral anterior and posterior (SLAP) lesions of the long bicep insertion on the glenoid: management in athletes.

INTRODUCTION: SLAP injuries are common in athletes but there is no consensus on different aspects such as pathophysiology and treatment options. Currently, the main controversy in the treatment of SLAP II injuries is deciding whether to make a repair or tenodesis. Clinical outcomes have varied according to the patients' age, sports, or work activity. This review aims to present the evidence at points of contention regarding pathophysiology, treatment options, outcomes, return to activities, and complications of type II SLAP.
