Arterial diameter during central volume depletion in humans.

The luminal diameter of the radial artery was followed by high frequency ultrasound during 50 degrees head-up tilt-induced central volume depletion in ten healthy subjects of whom six were tilted twice and pretreated with the serotonin receptor antagonist methysergide or placebo following a double-blind randomized design. Eight subjects without active treatment experienced presyncopal symptoms after 16-45 (mean 32 min). Central volume depletion was indicated by an increase in mean thoracic electrical impedance [from 31.5 (SEM 1.6) to 33.4 (SEM 1.7) omega; P < 0.05]. Cardiac output decreased [f

Early changes in regional and global left ventricular function after aortic valve replacement. Comparison of crystalloid, cold blood, and warm blood cardioplegias.

BACKGROUND: The clinical effects of different cardioplegic methods on left ventricular (LV) function have not been fully elucidated, particularly in the setting of myocardial hypertrophy. METHODS AND RESULTS: Sixty-four patients (mean age, 62 +/- 12 years; 41 men, 23 women) who were undergoing elective aortic valve replacement (stenosis, 49; regurgitation, 15; concomitant coronary artery bypass grafting, 22), with LV mass index 230 +/- 70 g/m2, were randomized to the following groups: antegrade crystalloid cardioplegia (CCP, 21 patients), antegrade/retrograde cold blood cardioplegia (CBP, 23

Ventilation-perfusion mismatching in chronic obstructive pulmonary disease during ventilator weaning.

Using the multiple inert gas elimination technique, we studied ventilation-perfusion (VA/Q) relationships in eight patients with chronic obstructive pulmonary disease (COPD) during mechanical ventilation (MV) and again during weaning (spontaneous ventilation [SV] through an endotracheal tube) from MV needed for acute respiratory failure. The patients, seven men and one woman with a mean age of 63 +/- 2.8 (SEM) yr (FEV1 33 +/- 5.2% of predicted), required MV for 9.0 +/- 2.4 days prior to the study. The patients were studied at maintenance FIO2 (0.28 to 0.40) while breathing 100% O2, both during

Captopril: pharmacology, metabolism and disposition.

By inhibiting ACE, captopril blocks the conversion of AI or AII and augments the effects of bradykinin both in vitro and in vivo. In rats, dogs, and monkeys with 2-kidney renal hypertension, orally administered captopril rapidly and markedly reduces blood pressure; this antihypertensive effect apparently occurs via a renin-dependent mechanism; that is, the inhibition of ACE. In 1-kidney renal hypertension studies in rats and dogs, it was determined that oral doses of captopril markedly lowered blood pressure, but only after several days of dosing; the mechanism is thought to be non-renin depen

The Association Between Notching of the Right Ventricular Outflow Tract Flow Velocity Doppler Envelope and Impaired Right Ventricular Function After Acute High-Altitude Exposure.

INTRODUCTION: Pulmonary artery pressure (PAP) is increased and right ventricular (RV) function is well preserved in healthy subjects upon exposure to high altitude (HA). An increase in PAP may trigger notching of the right ventricular outflow tract Doppler flow velocity envelope (RVOT notch), which is associated with impaired RV function in patients with pulmonary hypertension. However, whether HA exposure can induce RVOT notch formation and the subsequent impact on cardiac function in healthy subjects remains unclear. METHODS: A total of 99 subjects (69 males and 30 females) with a median ag

Effect of Dapagliflozin on Myocardial Insulin Sensitivity and Perfusion: Rationale and Design of The DAPAHEART Trial.

INTRODUCTION: Sodium-glucose co-transporter-2 (SGLT-2) inhibitors have been shown to have beneficial effects on various cardiovascular (CV) outcomes in patients with type 2 diabetes (T2D) in primary prevention and in those with a high CV risk profile. However, the mechanism(s) responsible for these CV benefits remain elusive and unexplained. The aim of the DAPAHEART study will be to demonstrate that treatment with SGLT-2 inhibitors is associated with greater myocardial insulin sensitivity in patients with T2D, and to determine whether this improvement can be attributed to a decrease in whole-b

Outcome differences in acute vs. acute on chronic heart failure and cardiogenic shock.

AIMS: Despite advances in coronary reperfusion and percutaneous mechanical circulatory support, mortality among patients presenting with cardiogenic shock (CS) remains unacceptably high. Clinical trials and risk stratification tools have largely focused on acute CS, particularly secondary to acute coronary syndrome. Considerably less is understood about CS in the setting of acute decompensation in patients with chronic heart failure (HF). We sought to compare outcomes between patients with acute CS and patients with acute on chronic decompensated HF presenting with laboratory and haemodynamic

Biomechanical and neuroautonomic adaptation to acute blood volume displacement in ischemic dilated cardiomyopathy: the predictive value of the CD25 test.

We aimed to examine biomechanical and neuroautonomic adaptation to blood volume displacement induced by tilt test in patients with previous inferoapical/inferolateral (IA-IL) or basal/apical septal (BS-AS) myocardial infarction (MI). Twenty-four patients with heart failure (HF) and previous IA-IL MI and 30 patients with HF and previous BS-AS MI were enrolled. All patients underwent head-up tilt test, radionuclide ventricular function monitoring (VEST), sympathovagal balance evaluation, and chronotropic 25-dose isoproterenol infusion test (CD25). Physiopathological reactions to stress-tests wer

Hypoxia-elicited mesenchymal stem cell-derived exosomes facilitates cardiac repair through miR-125b-mediated prevention of cell death in myocardial infarction.

Exosomes (Exo) secreted from hypoxia-conditioned bone marrow mesenchymal stem cells (BM-MSCs) were found to be protective for ischemic disease. However, the role of exosomal miRNA in the protective effect of hypoxia-conditioned BM-MSCs-derived Exo (Hypo-Exo) remains largely uncharacterized and the poor specificity of tissue targeting of Exo limits their clinical applications. Therefore, the objective of this study was to examine the effect of miRNA in Hypo-Exo on the repair of ischemic myocardium and its underlying mechanisms. We further developed modified Hypo-Exo with high specificity to the

Mitral reconstruction in cardiomyopathy.

Functional mitral regurgitation (MR) is a significant complication of end-stage cardiomyopathy, and may affect most heart failure patients as a pre-terminal or terminal event as it develops secondary to a change in the annular-ventricular apparatus and altered ventricular geometry; this results in incomplete leaflet coaptation.. Historically, surgery in MR patients was mitral valve replacement, but the adverse effects on LV systolic function of interrupting annulus-papillary muscle continuity was poorly understood. At the University of Michigan (1993-2000), 140 patients with end-stage cardiomy

Cerebral injury after cardiac surgery: identification of a group at extraordinary risk. Multicenter Study of Perioperative Ischemia Research Group (McSPI) and the Ischemia Research Education Foundation (IREF) Investigators.

BACKGROUND AND PURPOSE: Cerebral injury after cardiac surgery is now recognized as a serious and costly healthcare problem mandating immediate attention. To effect solution, those subgroups of patients at greatest risk must be identified, thereby allowing efficient implementation of new clinical strategies. No such subgroup has been identified; however, patients undergoing intracardiac surgery are thought to be at high risk, but comprehensive data regarding specific risk, impact on cost, and discharge disposition are not available. METHODS: We prospectively studied 273 patients enrolled from

De novo design of modular and tunable protein biosensors.

Naturally occurring protein switches have been repurposed for the development of biosensors and reporters for cellular and clinical applications1. However, the number of such switches is limited, and reengineering them is challenging. Here we show that a general class of protein-based biosensors can be created by inverting the flow of information through de novo designed protein switches in which the binding of a peptide key triggers biological outputs of interest2. The designed sensors are modular molecular devices with a closed dark state and an open luminescent state; analyte binding drives

Deep Learning Blood Dosimetry Predicts Severe Lymphopenia and Survival After Craniospinal Irradiation.

PURPOSE: Craniospinal irradiation (CSI) is a standard treatment for pediatric brain tumors and is increasingly used for leptomeningeal metastases. Although effective, CSI often causes radiation-induced severe lymphopenia (RISL), which is associated with poor outcomes across various cancer types. We developed and validated a novel platform for personalized dosimetry of circulating blood cells (CBCs) and assessed its predictive value for RISL. METHODS AND MATERIALS: We combined a prospective pediatric CSI registry (initiated April 2018; n = 48) and retrospectively collected patients (January 20

Cigarette smoking slows the on- and the off- cardiorespiratory and gas-exchange kinetics during moderate exercise in young, physically active adults.

PURPOSE: Cigarette smoking (CS) impact on cardiopulmonary function has been extensively investigated on sedentary, middle-aged smokers (SMK) with pulmonary disease, but not on young SMK with high fitness level. This study evaluated the cardiopulmonary and gas-exchange kinetics during and after moderate exercise in young, physically active SM without known diseases. METHODS: Ten SMK (age: 21 ± 2 year., body mass: 78 ± 6 kg; stature: 1.79 ± 0.07 m; 12 ± 5 cigarette/day for 6 ± 2 year.; mean ± SD) and twelve non-smokers (CTRL; age: 24 ± 3 year., body mass: 78 ± 9 kg; stature: 1.80 ± 0.08 m) matc

Machine learning prediction of right heart failure after left ventricular assist device implantation with individual risk factor visualization.

BACKGROUND: Right ventricular failure (RVF) is a major adverse event after left ventricular assist device (LVAD) implantation, but its complex mechanisms make accurate prediction challenging. While supervised machine learning (ML) can predict outcomes from complex models, understanding the conclusions is often difficult. This study aimed to develop an interpretable supervised ML model to assess patients' RVF risk and identify key risk factors. METHODS: We analyzed 327 patients from 482 consecutive LVAD implantations performed between June 2010 and January 2024 who underwent preoperative right

Prognostic implications of echocardiographic and hemodynamic profiles in cirrhosis and high-output heart failure.

INTRODUCTION: Cirrhosis is a recognized cause of high-output heart failure (HOHF), yet the prognostic relevance of paired invasive hemodynamic and echocardiographic remodeling profiles in this population remains incompletely defined. METHODS: We retrospectively analyzed 41 adults with cirrhosis-associated HOHF who underwent right heart catheterization between 2015 and 2023. HOHF was defined as thermodilution cardiac index (CI) ≥ 4.0 L/min/m2 with elevated filling pressures. The primary outcome was all-cause mortality. RESULTS: During a median follow-up of 1.66 years, 18 patients died and 21

The safety and efficacy of low pneumoperitoneum pressure compared with standard pneumoperitoneum pressure in laparoscopic inguinal hernia repair: a prospective, double-blind, randomized controlled study protocol design.

BACKGROUND: Inguinal hernia is a common disease, and laparoscopic tension-free inguinal hernia repair has become the standard procedure for treating inguinal hernia. During surgery, carbon dioxide gas is injected into the patient's abdominal cavity to maintain a specific pneumoperitoneum pressure. Under standard pneumoperitoneum pressure (SPP), we occasionally observe that some patients are prone to subcutaneous emphysema and hypercarbia, especially elderly patients with inguinal hernias, where the occurrence is relatively high. The aim of this study was to analyse whether using lower pneumope

Enhancing post-induction hypotension prediction based on exemplar learning with crossover restart strategy driven feature selection.

BACKGROUND AND OBJECTIVE: Post-induction hypotension (PIH), primarily resulting from the vasodilatory effects and reduced cardiac output induced by anesthetic agents, is widespread among patients with pre-existing cardiovascular conditions or those who have experienced suboptimal fluid management. This condition can lead to inadequate perfusion of critical organs such as the brain and heart, increasing the risk of lengthy postoperative recovery, complications, and mortality. Therefore, early identification and prediction of PIH are crucial for improving postoperative management and patient out
