Current Management of Zygomaticomaxillary Complex Fractures: A Multidisciplinary Survey and Literature Review.

Despite the prevalence of zygomaticomaxillary complex (ZMC) fractures, there is no consensus regarding the best approach to management. The aim of this study is to determine differences in ZMC fracture treatment among various surgical specialties. A survey was conducted regarding treatment of patients with different ZMC fractures that included a minimally displaced fracture (Case 1), a displaced fracture without diplopia (Case 2), a displaced fracture with diplopia (Case 3), and a complex comminuted fracture (Case 4). The survey was distributed to members of plastic surgery, oral maxillofacial

Volar plate fixation of intra-articular distal radius fractures: a retrospective study.

Background Intra-articular fractures of the distal radius represent a therapeutic challenge as compared with the unstable extra-articular fractures. With the recent development of specifically designed internal fixation materials for the distal radius, treatment of these fractures by fragment-specific implants using two or more incisions has been advocated. Purpose The purpose of this study was to investigate the efficacy of a fixed-angle locking plate applied through a single volar approach in maintaining the radiographic alignment of unstable intra-articular fractures as well as to report th

[Operative treatment of complex acetabular fractures through the modified extensile iliofemoral approach].

Fourty-nine patients with 50 complex acetabular fractures were treated with open reduction and internal fixation from August 1993 to February 1996 in a prospectively documented study. The fractures were fixed with small fragment implants through a modified extensile iliofemoral approach (Maryland approach). Thirty-seven patients were polytraumatized with an ISS of 20 pts. on average. In 10 patients the ISS was > 25 pts. Good or anatomic initial fracture reduction with a fragment stepoff K 2 mm could be achieved in 48 fracturers (96 %). Thirty-six patients with 37 fractures could be evaluated a

A proposed surgical strategy for complex articular distal radius fractures: The Marseille approach for radius salvage (MARS).

INTRODUCTION: Complex intra-articular distal radius fractures with severe comminution or fragment rotation may be difficult to reduce using conventional fixation. We describe a dorsal salvage strategy, the Marseille Approach for Radius Salvage (MARS), enabling direct articular visualization in selected cases. HYPOTHESIS: The MARS strategy allows reliable reduction and acceptable short-term functional outcomes in fractures not amenable to standard fixation. MATERIALS AND METHODS: Single-center prospective case series (May 2021-October 2025). CT assessed reduction quality (step-off ≤ 2 mm). Fu

Risk factors and complications in surgical management of proximal humeral fractures: a retrospective analysis of 132 cases.

Proximal humeral fractures (PHF), ranking as the third most common osteoporotic fractures, pose a significant challenge in management. With a rising incidence in an aging population, controversy surrounds surgical versus nonoperative treatments, particularly for displaced 3- and 4-part fractures in older patients. Locking plates (LP) and proximal intramedullary nails (PHN) are primary choices for surgical intervention, but both methods entail complications. This retrospective study of 132 patients undergoing surgery for PHF (LP or PHN) aims to identify risk factors for postoperative complicati

Case Report: Proximal Phalangeal Fracture Management in a European Bison (Bison bonasus).

Fracture of the digits is a well-known orthopedic condition in adult cattle, and mainly the distal phalanx (P3) is involved. To our knowledge, the treatment of fractures of the middle (P2) and proximal (P1) phalanges with orthopedic claw blocks has not yet been described in cattle and other ruminants. This report describes the first case of the successful management of a P1 fracture in an adult European bison. A 5-year-old female European bison (Bison bonasus) presented with severe weight bearing lameness of the left hind limb and a marked soft tissue swelling accentuated over the plantar and

A RETROSPECTIVE CASE-SERIES ON THE USE OF S53P4 BIOACTIVE GLASS FOR THE ADJUNCTIVE TREATMENT OF SEPTIC DIAPHYSEAL NON-UNION.

OBJECTIVE: Non-union and persistence of infection at a fracture site for long periods are always described as a challenge to orthopedists, especially in cases of severe compound fractures with comminution and segmental bone loss. This is a case series of septic non-unions, using S53P4 bioactive glass for adjunctive treatment, using internal syntheses or external fixators. The objective is to retrospectively evaluate the results of the use of S53P4 bioglass for the adjunctive treatment of septic non-unions. METHODS: We reviewed 18 patients with septic non-unions. The patients were preoperative

Donor site complications following anterior iliac crest bone graft for treatment of distal radius fractures.

INTRODUCTION: In distal radius fractures with metaphyseal comminution, bone grafting or the use of a bone substitute may be necessary. Harvesting autologous iliac crest bone graft for other orthopedic procedures has complications. The aim of this study was to evaluate the complication rate after harvesting a small amount of bone as used for the treatment of radius fractures. PATIENTS AND METHODS: Patients treated in a single level I trauma center with surgical treatment for distal radius fracture with iliac crest bone graft between January 2008 and December 2012 were included in this retrospe

Osteonecrosis of the Jaw and Bilateral Atypical Femoral Fracture Both Occurring During Treatment for Osteoporosis: A Case Report.

Osteonecrosis of the jaw (ONJ) and atypical femoral fracture (AFF) are rare potential adverse effects of bisphosphonates and RANKL antibody therapy. The pathogenic mechanisms of both conditions are known to be independent of each other. Here, we report both conditions sequentially occurring in the same patient. An 81-year-old, obese, diabetic, female was admitted due to hypertensive urgency and persistent jaw pain after tooth extraction. The patient has postmenopausal osteoporosis for fourteen years and was on intermittent, unsupervised treatment with alendronate, denosumab and ibandronate. Up

A case study in bilateral radial head fractures in apparently trivial trauma: a subtle diagnosis.

Radial head and neck fractures are common in young to middle age adults and are seen in nearly 20 % of acute elbow injuries in this age group. These are usually associated with high energy traumas like falls from height, road traffic accidents and sports injuries. Unilateral radial head fractures are relatively common and may be associated with other concomitant injuries. Bilateral radial head fractures are rare and are mostly seen in situations when the patient has a fall on outstretched, supinated hands or a direct fall on the elbow. These injuries can be easily missed by the attending physi

Computed tomographic assessment of old calcaneal fractures.

Prolonged pain and disability are common complications of calcaneal fractures. A prospective study was conducted on the value of computed tomography (CT) in 63 patients with 73 calcaneal fractures sustained more than 6 months before. The fractures were classified from the initial plain radiographs, and the patients' current clinical condition assessed in terms of a "disability score". The mean disability scores (MDS) associated with the different types of fracture were correlated with the CT findings. The highest MDS was found in the intra-articular fracture group which comprised 71% of the se

Scapulothoracic Dissociation and Clavicle Fracture with Associated Brachial Plexus Palsy.

Scapulothoracic dissociation is the traumatic separation of the scapula from the thoracic wall. It is the outcome of rare, high-energy trauma to the shoulder girdle and may include vascular and neurologic damage of the upper extremity. Of the few cases presented in the literature, the majority refer to polytrauma patients. We present a rare variant of scapulothoracic dissociation combined with comminuted fractures of both the scapula and the clavicle associated with brachial plexus palsy. There were also vertebral fractures of the spinous processes of C6, T4, T5, and T6, of the transverse proc

Palate Fracture Repair With Light-Cured Resin Splint: Technical Note.

Palate fractures are rare, and their treatment is a matter of debate. Although some investigators have favored rigid plate fixation, others have reported successful treatment without it. Sagittal split and comminuted fractures can require rigid fixation to reduce the maxillary width; however, additional stabilization is needed. Also, palate repair without a splint is complicated by prolonged intermaxillary fixation (IMF), causing stiffness to the temporomandibular joint. We introduce a technique using a rapid light-cured resin (TRIAD TranSheet) frequently used by orthodontists for making denta

Surgical Techniques to Manage the Unstable Volar Lunate Facet Fragment of Distal Radius Fractures.

INTRODUCTION: The surgical treatment of distal radius fractures is one of the most common procedures performed by orthopaedic surgeons. Intraarticular fracture patterns, specifically those involving the volar lunate facet, can be particularly problematic because this fragment is crucial to radiocarpal and distal radioulnar joint (DRUJ) stability. The problems associated with these patterns are attributed to the anatomy of the volar ulnar corner of the distal radius as well as the limitations of conventional volar distal radius implants and common surgical exposures. METHODS: This article will

Frontal Sinus Fractures: 10-Year Contemporary Experience at a Level 1 Urban Trauma Center.

Frontal sinus fractures account for 5% to 15% of all facial fractures, and have traditionally been associated with high kinetic energy blunt injury. Surgical management is largely focused on minimizing potentially serious sequelae including frontal sinus dysfunction, CSF leak, and significant cosmetic deformity. An institutional database of 1944 patients presenting with maxillofacial fractures over a 10-year period was queried. Demographics, mechanism of injury, yearly trends, surgical approaches, and follow-up data were examined. A total of 160 (8.3%) patients presented with at least 1 fractu

Traumatic Obturator Dislocation of the Hip Joint Associated with Greater Trochanter Fracture: A Case Report.

BACKGROUND: Traumatic obturator dislocation of the hip joint associated with greater trochanter fracture is a rare injury. We used the lateral approach through the rectus abdominis to remove the femoral head dislocated into the obturator, and the posterolateral approach was used for reduction and internal fixation of the femoral greater trochanteric fracture and total hip replacement (THR). Good follow-up results were achieved. To the best of our knowledge, this is the first report on this particular type of injury and on this approach to treating this type of injury. CASE REPORT: The patient

Ulnar-sided carpometacarpal fractures and fractures-dislocations. A systematic review and publication guidelines.

BACKGROUND: Intra-articular fractures and fracture-dislocations of the ulnar-sided carpometacarpal joints (US-CMC) are common and under-reported. Their diagnosis is often delayed or incomplete, and there is no consensus as to the best treatment. The aim of this systematic literature review was to focus on the quality of existing data and to describe in detail the available evidence on the diagnosis and treatment of these lesions. Based on our findings, we will propose guidelines for improving the design of future studies on these lesions. METHODS: This systematic literature review looked at a

Influence of screw to joint distance on articular subsidence in tibial-plateau fractures.

BACKGROUND: Tibial-plateau fractures with depressed osteochondral fragments have a high incidence of articular subsidence post reduction. Locking plates aim to prevent this via 'raft' screws below the subchondral bone. However, differences in plate design and patient anatomy result in variability of screw position in relation to the fragments they are designed to support. We evaluate the effect of screw placement and articular subsidence with this fracture pattern. METHODS: A retrospective cohort study of operatively treated tibial-plateau fractures with free osteochondral fragments was perfo
