Presence of surfactant lamellar bodies in normal and diseased sinus mucosa.

BACKGROUND: Pulmonary surfactant originates from phospholipid lamellar bodies secreted from the type II epithelial cell of the alveolus. In the lower airway, surfactant optimizes surface tension and oxygen exchange, decreases mucus viscosity and aids in mechanical elimination of inhaled pathogens. In addition to the lung, lamellar bodies have been identified in many other cell types throughout the human body. However, no prior studies have identified lamellar bodies in human sinus mucosa. OBJECTIVES: We performed ultrastructural studies to assess whether lamellar bodies are present in the hum

In-office, multisinus balloon dilation: 1-Year outcomes from a prospective, multicenter, open label trial.

BACKGROUND: The objective of this prospective, multicenter study was to assess 1-year changes in sinonasal symptoms and health care use after office-based multisinus balloon dilation. METHODS: Adults diagnosed with chronic or recurrent acute rhinosinusitis per the 2007 adult sinusitis guidelines were enrolled in this Institutional Review Board-approved study. Balloon dilation of the maxillary sinuses/ethmoid infundibula with or without frontal or sphenoid ostial dilation was performed in the physician's office under local anesthesia. Intraoperative procedure technical success and subject proc

Evaluation of Practice Patterns for 6-Sinus Balloon Sinus Dilation.

IMPORTANCE: Balloon sinus dilation (BSD) is a tool for treating patients with chronic rhinosinusitis or recurrent acute rhinosinusitis. The use of BSD has increased exponentially since its introduction, with concerns for potential misuse. Expert consensus reports that most patients who are candidates for BSD do not need to have all 6 sinuses dilated (bilateral frontal, maxillary, and sphenoid). OBJECTIVE: To determine the percentage of patients who are undergoing 6-sinus BSD, evaluate factors associated with this practice, and explore outlier practice patterns when performing 6-sinus BSD. Computed Tomographic Assessment of Individual Paranasal Sinus Compartment and Nasal Conchal Bulla Involvement in 300 Cases of Equine Sinonasal Disease.

Background: Computed tomographic (CT) imaging has allowed new anatomical studies and detailed clinical imaging of the complex, overlapping equine sinonasal structures. Despite the widespread use of CT, no study has specifically identified which compartments are most commonly affected with sinus disorders. CT has also shown the presence of intercurrent, ipsilateral nasal disorders, especially infection of the nasal conchal bullae (NCB) in many cases of sinus disease, but the frequency of intercurrent NCB infections has not been reported. Objectives: To identify which sinus compartments are most

Sino-orbital osteoma: a clinicopathologic study of 45 surgically treated cases with emphasis on tumors with osteoblastoma-like features.

CONTEXT: Osteomas are limited almost exclusively to craniofacial and jaw bones. Histologically, they can be divided into ivory, mature, or mixed types. Osteomas may have osteoblastoma-like areas and distinguishing it from true osteoblastoma can be challenging. Some believe osteomas with osteoblastoma-like features behave more aggressively. OBJECTIVE: To perform a clinicopathologic comparison of sino-orbital osteomas both with and without osteoblastoma-like features. DESIGN: We studied 45 surgically excised sino-orbital osteomas. Tumors were categorized as ivory, mature, or mixed type and pre

Contemporary indications for external approaches to the paranasal sinuses.

The term minimally invasive functional endoscopic sinus surgery is surely a misnomer. First, the surgery is not minimally invasive. Extensive surgery, even skull base tumor resection, can be accomplished through the approach. In fact, because of the improved visualization provided by the adaptation of the endoscope to this technique, it can be argued that the surgery is now more invasive than it has ever been historically. More correctly, the technique is "minimal incision" surgery, and this term more accurately reflects the philosophy. External incisions, however, can be cleverly minimized or

Disease severity and outcome in thymoma myasthenia gravis: a long-term observation study.

ABSTRACT: The anatomy of the sinonasal area has a very wide rage of anatomical variations. The significance of these anatomical variations in pathogenesis of rhinosinusitis, which is the commonest disease in the region, is still unclear. The aims of the study were to compare the rate of sinonasal anatomical variations with development and severity of chronic rhinosinusitis patients. CT scan of paranasal sinuses images of 99 individuals were retrospectively reviewed. 65 cases of chronic rhinosinusitis (study group) who had undergone endoscopic sinus surgery were compared with 34 cases without c

Direct dacryoendoscopic probing and endonasal dacryocystorhinostomy for treatment of acquired dacryocystocele: A case series.

RATIONALE: Acquired dacryocystocele is a rare condition in adults and is often associated with distal and proximal lacrimal drainage obstructions. While proximal obstruction has generally been presumed to be functional, this assumption has not been definitively confirmed in previous studies. In this case series, we report for the first time the use of direct dacryoendoscopic probing as both a diagnostic and therapeutic adjunct to endonasal dacryocystorhinostomy (DCR) in the management of acquired dacryocystocele. PATIENT CONCERNS: Three patients (1 male and 2 females; age, 66-88 years) presen

[Cerebral venous thrombosis after spinal anesthesia: case report].

INTRODUCTION: Cerebral venous thrombosis (CVT) is a rare, but serious complication after spinal anesthesia. It is often related to the presence of predisposing factors, such as pregnancy, puerperium, oral contraceptive use, and malignancies. Headache is the most common symptom. We describe a case of a patient who underwent spinal anesthesia who had postoperative headache complicated with CVT. CASE REPORT: Male patient, 30 years old, ASA 1, who underwent uneventful arthroscopic knee surgery under spinal anesthesia. Forty-eight hours after the procedure, the patient showed frontal, orthostatic

[How to manage intracranial empyemas in the absence of computerized tomography?].

BACKGROUND: Intracranial, and especially subdural, empyemas are a medical emergency usually requiring surgical intervention. We suggest herein a protocol for their management in areas where CT is not yet available. MATERIALS AND METHODS: 16 patients with supratentorial empyemas (6 epidural abscesses, 6 subdural empyemas and 4 with both) diagnosed by surgical intervention in our department between 1986 and 1995 were studied. The etiological features were frontal sinusitis (7 cases), mastoid infection (1), trauma (5), previous operations (2) and unknown (1). Three patients were in a normal stat…

Management of a decompensated acute-on-chronic intracranial venous sinus thrombosis.

A 34-year-old female patient presented during the 10th week of her second gravidity with headache, nausea and vomiting 2 weeks before admission. Her medical history was remarkable for a heterozygous factor V Leiden mutation, elevated lipoprotein A, and a cerebral venous thrombosis (CVT) after oral contraceptive intake 15 years before. Magnetic resonance imaging (MRI) suggested acute and massive intracranial sinus thrombosis. Despite full-dose anticoagulation, the patient deteriorated clinically and eventually became comatose. Now, MRI/magnetic resonance angiography revealed vasogenic edema of

Prospective evaluation of aerosol delivery by a powered nasal nebulizer in the cadaver model.

BACKGROUND: The objective of this study was to compare the distribution of aerosol delivered via a powered nasal nebulizer device in 5 fresh frozen-cadaver heads (10 total sides). METHODS: Nasoneb® (Medinvent, St. Paul, MN) was used to deliver a total volume of 10 mL (9 mL saline and 1 mL of 10% fluorescein). Aerosol distribution was assessed in 3 trials: (1) unoperated nose; (2) post-functional endoscopic sinus surgery (FESS); and (3) post-FESS with endoscopic modified Lothrop procedure (EML). Two independent observers rated the distribution of the fluorescein-dyed saline in the anterior nas

Severe Cerebral Complications Secondary to Perforation Injury of the Anterior Skull Base During Sinonasal Surgery: An Underappreciated Problem?

OBJECTIVE: Functional endonasal sinus surgery (FESS) is widely practiced and is considered a generally safe procedure. Skull base injuries occur in <1% of procedures and are typically associated with cerebrospinal fluid leaks. Rarely, skull base injuries might result in cerebral lesions. Here we present a series of 4 patients with iatrogenic perforating injuries of the anterior skull base and cerebral lesions after routine FESS. METHODS: Four patients with iatrogenic perforating cerebral lesions after routine FESS, performed at other institutions, were referred to a tertiary neurosurgery depa

[The value of computer reconstruction of the frontal and sagittal planes of the facial structures].

The authors evaluate the merits of computer reconstructions in the coronal and sagittal planes to assess the anatomy of the facial structures. If the patient can be kept motionless, the coronal plane provides information similar to those obtained by a study with direct sections. The ethmoidofrontal recesses, the lamina cribrosa, the thin relief of the middle meatus, the floor of the orbit are perfectly examined. The sagittal plane provides a new approach of the anterior and posterior cortical bone, of the frontal sinus, of the nasofrontal canal, of the ethmoidal cells, of the sphenoid sinus an

[Fronto-basal fractures in the child].

The particularities and the authors' management of frontobasal fractures in children are discussed on the basis of cases treated at the authors' department during the last five years. Fifteen children aged between four and 16 years (mean eight years) made up 14.5% of all frontobasal fractures. The causes of the injury were equally divided between falls from a height of at least 2 m and traffic accidents. All children except one were treated immediately after the accident. The exception was a child with an accident two and a half years before operation and two episodes of late onset meningitis.

Widespread distribution of lymphatic vessels in human dura mater remote from sinus veins.

Background and purpose: Previous experimental studies have shown that meningeal lymphatic vessels are located primarily along the walls of the dural sinus veins. Whether they are more widespread throughout human dura mater has presently not been characterized. The present study explored in humans whether meningeal lymphatic vessels may be identified remote from the sinus veins and whether they differ in the various location of dura mater. Methods: We included 15 patients who underwent neurosurgery, in whom dura mater was removed as part of the planned procedure. Tissue was prepared for immunoh
