[Monteggia lesions in the growing skeleton: principles of therapy].

PURPOSE OF THE STUDY: In this retrospective study we analyse the methods of Monteggia lesion (ML) treatment in children used in our department. Based on the results we propose several general principals for the therapy of this complex injury to the forearm in agrowing skeleton. MATERIAL: The group included patients with the diagnosis of acute ML treated at our department in the 2003-2007 period. No patients in whom therapy was started later than two weeks after injury were included. METHODS: We assessed injuries according to the Bado classification system modified by Olney for young age, and

Contracture of the deltoid muscle. Results of distal release.

We prospectively studied the results of distal release, performed between February 1989 and December 1994 for the treatment of a contracture … nical result, and two shoulders (two patients) had a poor result. Anterosuperior subluxation of the humeral head, noted on preoperative radiographs of twenty-three shoulders (eighteen patients), was not present postoperatively. Drooping of the acromion, seen in six patients (six shoulders) in whom the contracture had developed before they were sixteen years ol …

Microscopic extracapsular extension in the axilla: is this an indication for axillary radiotherapy?

PURPOSE: Although the axilla is often treated with radiotherapy (RT) postoperatively when microscopic extracapsular extension (ECE) of lymph … s were also treated with an abbreviated supraclavicular field with the lateral border medial to the humeral head. Pathological sections were available for review in 72 of the 82 women. RESULTS: Twenty-seven of 72 (37.5%) had evidence of ECE; 45 of 72 (62.5%) had metastatic carcinoma confined within the nodal capsule. Clinical characteristics were comparable b …

Force Plate Based Assessment of Maximal and Rapid Isometric Shoulder Strength During the Return-to-Sport Process After Pectoralis Major Repair in a World-Class Female Wrestler: A Case Report.

BACKGROUND: Pectoralis major tendon rupture in high-level female wrestlers is extremely rare, and evidence regarding rehabilitation and return-to-sport criteria is limited. The purpose of this case report was to provide detailed information regarding quantitative assessment of postoperative recovery, focusing on maximal and rapid isometric force production following pectoralis major tendon repair in a world-class female wrestler. STUDY DESIGN: Case report. CASE DESCRIPTION: A world-class female wrestler in her 20s sustained rupture of the sternocostal and abdominal heads of the right pectora

Arthroscopically assisted osteosynthesis of intraarticular scapular fractures.

INTRODUCTION: Intraarticular scapular fractures occur mostly in high-energy injuries in contrast to glenoid fractures, which occur mostly in humeral head dislocations. In addition to open repositioning and osteosynthesis, minivascular osteosynthesis under the control of fluoroscopy and arthroscopy has been also used. Arthroscopy allows debridement of the fracture line in the intraarticular area as well as its repositioning during repositioning maneuvers under direct visualization. Furthermore, arthroscopy allows full control over the placement of osteosynthetic material in the subchondral regi…

All-suture anchor size and drill angle influence load to failure in a porcine model of subpectoral biceps tenodesis, a biomechanical study.

BACKGROUND: Tenodesis of the long head of the biceps tendon is frequently performed in shoulder surgery, and all-suture anchors have become more popular as fixation methods. However, uncertainty still exists regarding the ultimate load to failure of all-suture anchors and the best insertion angle at a cortical humeral insertion point. PURPOSE: The purpose of this study was to compare the biomechanical characteristics of three types of all-suture anchors frequently used for biceps tenodesis. In addition, the influence of two different insertion angles was observed in a porcine humeri model. M

Ulnar Collateral Ligament Reconstruction with Hamstring Autograft.

BACKGROUND: Ulnar collateral ligament (UCL) reconstructions have become increasingly common, particularly in youth overhead throwing athletes. These injuries are most commonly due to overuse and repetitive trauma on the inner elbow. Throwers with a deficient UCL often report decreased pitching speeds in addition to elbow pain and instability. INDICATION: The indications for this procedure include symptomatic valgus elbow instability during overhead throwing motions and a verified rupture of the UCL on advanced imaging. TECHNIQUE DESCRIPTION: The ipsilateral gracilis tendon is harvested and p

Methods to analyse the long head of the biceps in the management of distal ruptures of the supraspinatus tendon. Part 1: the concept of the "biceps box": dynamic rotator interval approach. Incidence of lesions of the long head of the biceps tendon.

INTRODUCTION: The area encompassing the long head of the biceps (LHB) can be represented as a rectangular parallelepiped. This geometric view can be likened to a box, the "biceps box", where the sides are the extrinsic structures and the LHB is the intrinsic structure. Since these structures are mobile in relation to each other, a dynamic "biceps box" model can modify assessments of the LHB, in its healthy or pathological state, and make the therapeutic approach to treating LHB lesions less arbitrary. MATERIAL AND METHOD: In order to describe the different sides of the "biceps box", and to un

Fractures of the lesser tuberosity of the humerus.

BACKGROUND: Fractures of the lesser tuberosity are rare injuries, and little is known of their epidemiology. Operative treatment is generall … ith age and sex-matched controls. CONCLUSIONS: A lesser tuberosity fracture, without an associated humeral head or greater tuberosity fracture, is a rare injury. Open reduction and internal fixation provides excellent restoration of function and range of shoulder movement, with a low risk of complications.

Injury patterns from major urban terrorist bombings in trains: the Madrid experience.

BACKGROUND: Terrorist urban mass casualty incidents (MCI) in the last 3 years have targeted commuter trains at rush hour, producing large numbers of casualties. Civilian care providers are usually not familiar with the types of blast injuries sustained by victims of these MCI. METHODS: We focus on the injury patterns sustained by casualties of the Madrid, 11 March 2004, terrorist bombings, at the seven hospitals that received most victims. Data were gathered of casualties who had injuries other than superficial bruises, transient hearing loss from barotrauma without eardrum perforation, and/o
