Aberrant origin of the long head of the biceps: a case series.

BACKGROUND: Variants of the origin of the long head of the biceps have been described intraoperatively. It is unclear whether these variants contribute to shoulder pathology. Our purpose was to describe an anatomic variation of the origin of the long head of the biceps and associated clinical presentations of 2 subjects and to review existing reports of other variants. METHODS: We present the history and physical examination, imaging, and arthroscopic findings of 2 cases of an abnormal variant of the origin of the long head of the biceps. RESULTS: In 2 subjects, the long head of the biceps w

Toe walking: A regional analysis of TTN variants between c.10,000 and c.99,999 in a cohort of children.

BACKGROUND: Persistent toe walking is traditionally considered idiopathic when no neurological or orthopedic cause is identified. Emerging evidence suggests genetic contributors, including TTN, but genotype-phenotype relationships remain poorly understood in affected children. METHODS: This retrospective descriptive study evaluated children with persistent toe walking assessed between November 2022 and April 2026 at a specialized physiotherapy practice. All participants underwent standardized clinical assessment and targeted next-generation sequencing. Individuals carrying TTN variants within

Metabolic syndrome-osteoarthritis axis: Beyond mechanical loading toward a cardiometabolic OA phenotype.

Osteoarthritis (OA) has long been framed as a mechanically driven "wear-and-tear" disorder. Increasing epidemiological and translational evidence indicates that metabolic syndrome (MetS)-including obesity, insulin resistance/hyperglycaemia, dyslipidaemia, and hypertension-acts as a key disease modifier in a substantial subset of patients. MetS-related OA may represent a cardiometabolic endotype with weight-independent risk and distinct inflammatory, vascular, and lipotoxic drivers. Here, we synthesize evidence from population cohorts, imaging studies, mechanistic experiments, and emerging mult

Screening and patient exploration for adult hypophosphatasia in orthopedics.

BACKGROUND: Hypophosphatasia is an inherited metabolic bone disorder characterized by cartilage and bone abnormalities. Adult hypophosphatasia presents with a wide range of symptoms, including refractory fractures, recurrent fractures, stress fractures, and reduced bone mineral density, along with extraskeletal manifestations such as myalgia, muscle weakness, and fatigue. Some patients with these symptoms may have undiagnosed hypophosphatasia. While hypophosphatasia is treatable with enzyme replacement therapy, early diagnosis is often difficult owing to its diverse clinical presentations. Thi

Marfanoid phenotype with intellectual disability associated with NKAP mutation: a case report.

BACKGROUND: Hackman-Di Donato syndrome (Online Mendelian Inheritance in Man no. 301,039), caused by mutations of the NKAP gene, located in Xq24, is a rare genetic disorder characterized by a Marfanoid phenotype, intellectual disability, and abnormalities of the musculoskeletal system. This condition has only been described in ten patients and two fetuses, who were identified owing to severe heart defects. All previously reported patients had mutations located in exons 8 and 9 of NKAP. CASE PRESENTATION: The proband, a 12-year-old Slavic male individual, presented with a Marfanoid habitus, tal

Occupational Computer-Use Duration and Ulnar Nerve Electrodiagnostic Findings: An Exploratory Observational Study.

Background: Musculoskeletal and peripheral nerve disorders have been reported among occupational computer users. However, the relationship between the duration of occupational computer use and ulnar neuropathy at the elbow (UNE) remains uncertain. This exploratory observational study investigated whether the duration of occupational computer use was associated with electrodiagnostically confirmed UNE and with ulnar nerve conduction parameters in a selected symptomatic population. Methods: Patients aged 23-68 years who were referred for nerve conduction studies (NCS) because of symptoms suggest

A simple questionnaire to prevent blindness: a cross-sectional study screening for Stickler syndrome in children.

OBJECTIVES: Stickler syndrome is a skeletal dysplasia that is widely under-recognized yet the most common cause of inherited retinal detachment in children. The most prevalent subtype is Stickler syndrome type 1 (STL1), with an autosomal dominant variant in COL2A1 typically resulting in ophthalmic, orofacial, auditory and musculoskeletal manifestations. However, manifestations across these domains are not as prominent in children, making identification of individuals suitable for further assessment difficult. Thus, we sought to test an easy-to-use screening tool capable of differentiating chil

Intracranial Hypotension Following Spinal Manipulation: A Case Report and Scoping Review of the Literature.

PURPOSE: Spinal manipulative therapies, including chiropractic and osteopathic maneuvers, are widely practiced for musculoskeletal complaints. However, rare but serious complications such as cerebrospinal fluid (CSF) leak with subsequent intracranial hypotension (IH) have been described. The pathophysiological mechanism is presumed to involve mechanical stress on the spinal dura during high-velocity movements, leading to dural tears, particularly in the cervicothoracic region. METHODS: We conducted a scoping review in accordance with the PRISMA extension for Scoping Reviews (PRISMA-ScR) guide

Efficacy and safety of low-dose radiotherapy in MRI-confirmed refractory chronic plantar fasciitis after extracorporeal shock wave therapy.

BACKGROUND: Refractory chronic plantar fasciitis remains a therapeutic challenge when symptoms persist despite conservative management and advanced physical therapies. Although low-dose radiotherapy has been used as an anti-inflammatory treatment for benign musculoskeletal disorders, integrated data combining clinical, functional, quality-of-life and imaging outcomes remain limited. METHODS: A longitudinal observational study was conducted in 68 patients with MRI-confirmed refractory chronic plantar fasciitis who remained symptomatic despite conservative treatment and extracorporeal shock wav

The role of glucagon-like peptides in osteosarcopenia.

Various metabolic abnormalities, including obesity, insulin resistance, hypertension, dyslipidemia, hyperthyroidism and low vitamin D levels, have been linked to both osteopenia and sarcopenia. Osteosarcopenia is also commonly observed in older age groups, notably in postmenopausal women. Glucagon-like peptide-1 (GLP-1), a labile incretin secreted from the intestinal L-cells, stimulates insulin secretion and sensitivity, making it an effective anti-diabetic medication. GLP-1 binds to its receptor, the GLP-1 receptor, a G-protein-coupled receptor, and leads to the stimulation of adenylate cycla

An affordable and immersive virtual reality-based exercise therapy in forward head posture.

Forward Head Posture (FHP) is one of the most commonly occurring musculoskeletal abnormalities. Despite exercise therapy being an effective approach for FHP treatment, it can be long, monotonous, and tedious. Virtual reality (VR) can be used as an innovative solution to address these challenges. We designed an affordable and immersive VR-based exercise therapy (VRET) system for FHP correction. The VRET contents (i.e., exercises and VR scenarios) were determined by physiotherapists and game designers at the focus group meetings. Hardware requirements include a VR box, smartphone, and sensors (i…

Scurvy as a Cause of Leg Pain and Limp in Pediatric and Adolescent Patients.

UNLABELLED: Vitamin C deficiency (VCD) results in collagen dysfunction due to decreased lysyl and prolyl hydroxylase enzyme activity. The resulting disease, known as scurvy, can present with various findings, including gingival bleeding, bruising, other skin lesions, poor wound healing, and tooth and bone abnormalities. VCD is also an uncommon cause of musculoskeletal pain among children. This current concept review aims to summarize our institutional experience with scurvy in ten patients and compare it with existing literature. Astute history taking, critical review of imaging, thorough phys

Zero Echo Time Musculoskeletal MRI: Technique, Optimization, Applications, and Pitfalls.

Zero echo time (ZTE) imaging is an MRI technique that produces images similar to those obtained with radiography or CT. In ZTE MRI, the very short T2 signal from the mineralized trabecular bone matrix and especially cortical bone-both of which have a low proton density (PD)-is sampled in a unique sequence setup. Additionally, the PD weighting of the ZTE sequence results in less contrast between soft tissues. Therefore, along with gray-scale inversion from black to white and vice versa, ZTE imaging provides excellent contrast between cortical bone and soft tissues similar to that of radiography
