Early Screening for Cerebral Palsy: A Systematic Review.

BACKGROUND: Cerebral palsy (CP) is the most common motor disability in childhood. Preterm infants, particularly those born before 28 weeks' gestation or extremely low birth weight, are at an increased risk due to the vulnerability of the developing brain. PURPOSE: The purpose of this systematic review is to evaluate the predictive accuracy and clinical utility of various neonatal screening methods in concluding early diagnosis of CP. DATA SOURCES: A comprehensive systematic review of evidence-based research was conducted using MEDLINE (PubMed), Embase (Elsevier), and Web of Science (Clarivat

The Effect of Abdominal Massage on Bilirubin Levels in Term Infants Receiving Phototherapy: A Randomized Controlled Trial.

BACKGROUND: The incidence of neonatal hyperbilirubinemia in Europe and the United States is estimated to be 3.2 and 4.4 per 10,000 live births, respectively. Abdominal massage for hyperbilirubinemia is considered a safe complementary treatment for infants that may increase number of defecations and decrease bilirubin levels. PURPOSE: This study was designed as a randomized controlled trial to determine the effect of abdominal massage on bilirubin levels in term infants receiving phototherapy. METHODS: The sample consisted of 43 term newborns (intervention group: 23; control group: 20) who re

Reimagining Supportive Approaches at the Intersection of Mandatory Reporting Policies for the Mother-Infant Dyad Affected by Substance Use.

BACKGROUND: As rates of substance use during pregnancy persist, the health and optimal development of infants with prenatal substance exposure remain a key priority. Nurses are tasked with identifying and reporting suspected cases of child maltreatment, including abuse and neglect, which is often assumed to be synonymous with substance use during pregnancy. While policies aimed at protecting infants from child abuse and neglect are well intentioned, literature regarding the short- and long-term social and legal implications of mandatory reporting policies is emerging. PURPOSE: In this article

Eyedealistic Vision: Optimizing Oxygenation to Reduce Severe Retinopathy of Prematurity in the Neonatal Intensive Care Unit.

BACKGROUND: Rates of severe retinopathy of prematurity (ROP) continue to rise globally despite advances in healthcare. Differences in practices related to optimizing oxygenation is among key factors that increase the risk of severe ROP. A recent increase in severe ROP rates at 2 local neonatal intensive care units (NICUs) prompted further investigation into potential preventative practices. PURPOSE: The project team's goal was to reduce rates of severe ROP by 20% in the 2 participating units. METHODS: A level IV and level III NICU were targeted, including 255 infants at risk for ROP. Interve

Reducing Noise in the NICU.

BACKGROUND: In the neonatal intensive care unit (NICU), elevated noise negatively impacts the neurodevelopmental environment, interrupts sleep, and can affect brain development in neonates. The American Academy of Pediatrics recommends that noise levels in the NICU should not exceed 45 dB. PURPOSE: The project aims were to: (1) decrease average noise level by 10% from baseline and (2) decrease exposure to severe noise (>65 dB) to <5% of the time. METHODS: This quality improvement project was conducted during 2021-2022 as a pre/post observational design in a Level IV NICU in New York City. We

Effectiveness of a Novel Feeding Algorithm for Oral Feeding Transition of Infants Born Prematurely.

BACKGROUND: Feeding skill acquisition is commonly a limiting factor determining when an infant born prematurely can be discharged. PURPOSE: This study aimed to determine if the addition of a novel feeding progression algorithm (combination of objective data from a suck measurement device and slow flow/low variability nipples) to current neonatal intensive care unit (NICU) standards could decrease feeding-related length of stay (primary outcome). We hypothesized that by timing the initiation of oral feedings to coincide with adequate sensory-motor skill development, feeding-related length of s

Benign Neonatal Hemangiomatosis.

BACKGROUND: Benign neonatal hemangiomatosis (BNH) is a rare, self-limiting subtype of infantile hemangiomas (IHs), in which infants with multiple cutaneous hemangiomas lack visceral involvement. Other subtypes of IHs exist that may mimic BNH and can be life-threatening depending on hemangioma location and size. CLINICAL FINDINGS: At birth, a 29 5 / 7 -week preterm female presented with several pinhead-sized pink papules distributed throughout her body. At 10 days of age, the patient had 12 enlarged domed-shaped red papules in a generalized distribution throughout her body. Over several weeks,
