Infectious diseases in rugby players: incidence, treatment and prevention.

Participation in rugby football can expose individuals to a variety of infectious diseases both on and off the field of play. The close physical contact and trauma inherent in playing rugby facilitates the transmission of viral, bacterial and fungal pathogens between players and may also lead to the acquisition of potentially lethal infections from the environment, such as tetanus. In the past few years there have been a number of reported outbreaks of infection amongst rugby players in the medical literature. The appearance of HIV infection has focused attention on the potential for transmiss

High blood levels of nitric oxide in rats subjected to prolonged respiratory arrest and their modulation during adrenocorticotropin-induced resuscitation.

Anaesthetized rats, endotracheally intubated and mechanically ventilated with room air, were subjected to a 5-min period of asphyxia by turning off the ventilator. The ventilator was then turned back on and, simultaneously, the animals were treated with either the adrenocorticotropin fragment 1-24 [ACTH-(1-24), 160 microg/kg in a volume of 1 ml/kg i.v.] or an equivalent volume of saline. Nitric oxide (NO)-haemoglobin formation was detected ex vivo in arterial blood by electron spin resonance spectrometry; arterial blood pressure, electrocardiogram (ECG) and electroencephalogram (EEG) were moni

Hand-arm vibration, noise, temperature and static load--an experimental study of peripheral circulation while operating chain-saws.

From the viewpoint of elucidating the etiological mechanism of vibration-induced white finger (VWF), a model experiment was designed using a chain-saw under laboratory conditions. Finger skin temperature and finger blood flow were measured simultaneously as indicators of peripheral circulatory movement, using a thermister and an apparatus for measuring tissue blood flow based on the thermal diffusion method. In the first project (Experiment A), five healthy subjects operated a chain-saw at a high working speed with-out actually cutting anything. In the second project (Experiment B), seven heal

Clinical experience with motor and cerebellar evoked potential monitoring.

We are reporting 98 cases of motor evoked potential (MEP) monitoring performed between 1982 and 1986. These were divided into supratentorial, posterior fossa, and spinal cord categories. We observed in this sample that the peripheral nerve or electromyographic response was substantially more sensitive than the spinal cord response to injury and hypotension during operation and was often present bilaterally. Reversible weakening or loss of the peripheral nerve response was not associated with a deficit. However, weakening of the peripheral nerve response without recovery could warn of a motor d

Guidelines for Enhanced Recovery After Trauma and Intensive Care (ERATIC): Enhanced Recovery After Surgery (ERAS) Society and International Association of Trauma Surgery and Intensive Care (IATSIC) Recommendations: Paper 1: Initial Care-Pre and Intraoperative Care Until ICU, Including Non-Operative Management.

BACKGROUND: Enhanced recovery after surgery (ERAS) protocols reduce length of stay, complications, and costs for elective surgical procedures. It remains challenging to implement ERAS concepts in the acute trauma patient due to deranged physiological reserve from the penetrating or blunt trauma producing altered physiology. However, systems of care improve access to early intervention and potentially reduce mortality. These consensus guidelines examine optimal pre-hospital, resuscitation-room, intra-, and post-operative treatment, systems of ethical management, and overall care for trauma pati

Comparison of Outcomes of Internal Fixation Using Double-Angle Blade Plate Versus Double-Angle Dynamic Hip Screw With Pauwels Osteotomy in Neglected Fracture of the Neck of Femur.

Introduction Fracture of the neck of the femur is a significant orthopaedic injury with high morbidity and mortality rates, particularly among the elderly following low-velocity trauma. The complications of osteonecrosis and non-union are common in cases treated with any procedure of osteosynthesis. Surgical intervention through internal fixation is a common approach. Numerous procedures have been described for salvaging the femoral head to avoid or delay hip replacement in patients younger than 60 years. Operative methods to enhance vascularity and achieve healing at the fracture site include

Efficacy of Electronic Reminders in Increasing the Enhanced Recovery After Surgery Protocol Use During Major Breast Surgery: Prospective Cohort Study.

BACKGROUND: Enhanced recovery after surgery (ERAS) protocols are patient-centered, evidence-based guidelines for peri-, intra-, and postoperative management of surgical candidates that aim to decrease operative complications and facilitate recovery after surgery. Anesthesia providers can use these protocols to guide decision-making and standardize aspects of their anesthetic plan in the operating room. OBJECTIVE: Research across multiple disciplines has demonstrated that clinical decision support systems have the potential to improve protocol adherence by reminding providers about departmenta

Low-dose whole-lung radiation for COVID-19 pneumonia: Planned day 7 interim analysis of a registered clinical trial.

BACKGROUND: Individuals of advanced age with comorbidities face a higher risk of death from coronavirus disease 2019 (COVID-19), especially once they are ventilator-dependent. Respiratory decline in patients with COVID-19 is precipitated by a lung-mediated aberrant immune cytokine storm. Low-dose lung radiation was used to treat pneumonia in the pre-antibiotic era. Radiation immunomodulatory effects may improve outcomes for select patients with COVID-19. METHODS: A single-institution trial evaluating the safety and efficacy of single-fraction, low-dose whole-lung radiation for patients with C

Development and Validation of a Simple and Rapid UPLC-MS Assay for Valproic Acid and Its Comparison With Immunoassay and HPLC Methods.

BACKGROUND: Valproic acid (VPA), a widely used antiepileptic drug, has a narrow therapeutic range of 50-100 mcg/mL and shows large individual variability. It is very important to monitor the trough concentration of VPA using a reliable method. Therefore, the aim of this study was to develop and validate a rapid ultraperformance liquid chromatographic-mass spectrometry (UPLC-MS) method for quantification of VPA in human serum and to compare with fluorescence polarization immunoassay (FPIA), chemiluminescence microparticle immunoassay (CMIA), and high-performance liquid chromatography (HPLC) met

Simultaneous GC-MS determination of eight phthalates in total and migrated portions of plasticized polymeric toys and childcare articles.

A gas chromatography/mass spectrometric method was developed for the simultaneous determination of eight phthalic acid esters (PAEs) in toys and child care articles that are made of plasticized plastic. The novel method was used to determine the total concentration of the PAEs in addition to the migrated PAEs values into artificial saliva, under conditions that simulate real life situations. The extraction method, which was developed for the first time to determine the total concentration of PAEs, utilized a novel optimization of four parameters involving the solvent, time, temperature and wei

Early postoperative dislocation of the anterior Maverick lumbar disc prosthesis: report of 2 cases.

The authors report on 2 cases of anterior dislocation of the Maverick lumbar disc prosthesis, both occurring in the early postoperative period. These cases developed after experience with more than 50 uneventful cases and were therefore thought to be unrelated to the surgeon's learning curve. No similar complications have been previously reported. The anterior Maverick device has a ball-and-socket design made of cobalt-chromium-molybdenum metal plates covered with hydroxyapatite. The superior and inferior endplates have keels to resist translation forces. The patient in Case 1 was a 52-year-ol

Acute panic inventory symptoms during CO(2) inhalation and room-air hyperventilation among panic disorder patients and normal controls.

There is scant literature on anxiety symptoms induced during respiratory challenges developed to induce panic symptoms and attacks. Here we report on the prevalence of Acute Panic Inventory (API) symptoms during three consecutive respiratory challenges to patients with panic disorder (PD) and normal controls (NC). The challenges performed using a closed canopy system included voluntary room air hyperventilation (RAH), inhalation of 5% CO(2), and 7% CO(2)-enriched air. The PD patients were 41 men and 53 women whose mean age was 33.4 (SD = 8.55). The normal comparison group consisted of 35 men a

Immune function in paediatric trauma patients: a prospective explorative cohort study.

BACKGROUND: Trauma is an important risk factor for the development of nosocomial infections. Immunological consequences of trauma in paediatric patients remains scarcely explored and associations between injury severity and immunosuppression, a decreased functionality of the immune system, have not yet been investigated in this population. Therefore, the aim of this study was to characterise the early effects of trauma and trauma severity on paediatric immune function, and to assess whether immune profiles differed between polytrauma patients who developed nosocomial infections and those who e

Early reduction in unplanned healthcare utilization following vagus nerve stimulation for pediatric epilepsy.

OBJECTIVE: Vagus nerve stimulation (VNS) is an established adjunctive therapy for pediatric drug-resistant epilepsy (DRE), but objective evidence of its early impact on overall health is limited. Subjective seizure diaries can be influenced by placebo effects, whereas healthcare utilization may serve as a more objective window into the early post-operative course. This study aims to evaluate healthcare utilization in the immediate and subacute recovery phase (0-3 months) following VNS implantation in a large cohort of children with DRE. METHODS: We performed a retrospective, single-center, si

Neurosurgical and prehospital timing in severe traumatic brain injury: Insights from the prospective Resuscitation Outcomes Consortium hypertonic saline trial.

BACKGROUNDAND OBJECTIVES: Prompt management is critical in severe traumatic brain injury (TBI) to prevent secondary injury, but an unanswered question is whether, by the time patients reach the operating room, the injury has largely become irreversible. We aimed to evaluate the impact of prehospital time, emergency department (ED) length of stay, and time to neurosurgical intervention on mortality and functional outcomes for these patients. METHODS: A retrospective cohort study was conducted using data from the prospective Resuscitation Outcomes Consortium TBI trial. Our cohort included adult
