Hybrid interpenetrating hydrogel network favoring the bidirectional migration of tenocytes for rotator cuff tendon regeneration.

Replenishment of tenocytes to the injury site is an ideal strategy to improve healing response and accelerate the tendon ECM regeneration. The present study focused on the synthesis and characterization of a hybrid hydrogel scaffold system poly(propylene-fumarate)-alginate-polyvinyl alcohol-acrylic acid (PAPA) using poly(propylene-fumarate) (PPF), alginate, polyvinyl alcohol (PVA) and acrylic acid and the in vitro investigation of bidirectional mobility of swine shoulder tenocytes (SST) for its potential application in rotator-cuff tendon regeneration. IR analysis revealed the presence of algi

Effect of isokinetic eccentric training on the human shoulder strength, flexibility, and muscle architecture in physically active men: A preliminary study.

Strengthening the rotator cuff muscles is important for injury prevention and rehabilitation. Since muscle fascicle length improves motor performance and is suggested to reduce the risk of injury for the hamstring, it may be an important variable to promote multidirectional changes in the function and macroscopic structure for the shoulder. Recent literature reviews overwhelmingly suggest that eccentric exercises improve fascicle length and functional measures for the lower limb. However, there is a research gap for the shoulder. Since ultrasound imaging is the most commonly used imaging techn

Biodegradable implantable balloons: Mechanical stability under physiological conditions.

Rotator cuff tendons injuries occurs as a result of trauma, e.g. due to falling, mechanical injuries and frequent overhead activity and as natural degenerative tears in elderly people. Biodegradable balloon shaped spacer of Poly-(L-lactide-co-ε-caprolactone) (PLCL) are applied in the treatment of these injuries. This type of treatment involves insertion of inflated biodegradable implant into the tissues of the damaged region in the shoulder to avoid shoulder impingement and reduce friction between the acromion and the humeral head and propagation of inflammation. The implant must maintain inte

A new method for measurement of subcoracoid outlet and its relationship to rotator cuff pathology at MR arthrography.

OBJECTIVE: Orthopaedic surgical studies have shown that variations in the vertical distance between the tip of the coracoid process and the supra-glenoid tubercle alter the shape of the subcoracoid outlet. Our objective was to measure the vertical distance between the coracoid tip and the supra-glenoid tubercle (CTGT) on MR and to assess whether this showed better correlation with rotator cuff pathology compared with the axial coraco-humeral distance. MATERIALS AND METHODS: A retrospective review was performed of 100 consecutive shoulder MR arthrograms. Vertical distance between the coracoid

The effect of platelet-rich fibrin matrix on rotator cuff tendon healing: a prospective, randomized clinical study.

BACKGROUND: There is a strong need for methods to improve the biological potential of rotator cuff tendon healing. Platelet-rich fibrin matrix (PRFM) allows delivery of autologous cytokines to healing tissue, and limited evidence suggests a positive effect of platelet-rich plasma on tendon biology. PURPOSE: To evaluate the effect of platelet-rich fibrin matrix on rotator cuff tendon healing. STUDY DESIGN: Randomized controlled trial; Level of evidence, 2. METHODS: Seventy-nine patients undergoing arthroscopic rotator cuff tendon repair were randomized intraoperatively to either receive PRFM

The long-term outcome of recurrent defects after rotator cuff repair.

BACKGROUND: Retears of the rotator cuff are not uncommon after arthroscopic and mini-open rotator cuff repairs. In most studies, the clinical results in patients with persistent defects demonstrated significantly less pain and better function and strength compared with their preoperative state at an early follow-up. HYPOTHESIS: The clinical and structural outcomes of patients with known rotator cuff defects will remain unchanged after a longer period of follow-up. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: This study was performed in 15 patients (18 shoulders) from a previous

Distribution of normal and abnormal fluid collections in the glenohumeral joint: implications for MR arthrography.

Although magnetic resonance (MR) images of the glenohumeral joint frequently demonstrate intraarticular fluid, no specific criteria have, to the authors' knowledge, been published that allow accurate assessment of the amount of fluid present. Also, despite the increasing use of MR arthrography of the shoulder, the optimal amount of intraarticular fluid that should be used with this technique has not been determined. The authors progressively distended the glenohumeral joint in six cadaveric shoulder specimens with a dilute gadopentetate dimeglumine solution and obtained MR images after injecti

Latissimus dorsi and teres major transfer to rotator cuff for Erb's palsy.

During a 20-year period the authors operated on 56 patients with Erb's palsy to create active external rotation of the shoulder. Thirty-five patients had a follow-up period longer than 2 years (average 5 years), which allowed comparison with preoperative motion and strength. Preoperative passive external rotation averaged 5 degrees, whereas active abduction averaged 74 degrees. Preoperative external rotation strength was less than grade 2, whereas preoperative abduction strength was grade 2 or 3 in 33 of the 35 patients. Postoperative active external rotation averaged 31 degrees with only two

Platelet-rich plasma application in the management of chronic tendinopathies.

Platelet-rich plasma (PRP) may represent a new therapeutic option for chronic tendinopathies. Platelets release various cytokines and growth factors which promote angiogenesis, tissue remodeling, and wound healing. We made an extended literature review of the use of PRP in chronic tendinopathies: epicondylitis, rotator cuff, patellar and calcaneal tendinopathies, and plantar fasciitis. Medline, Embase and Google Scholar were used (until July 31, 2012). Clinical studies on PRP and tendinopathies published in English and French language peer-reviewed journals were included. Ultrasound shear wave elastography-derived tissue stiffness predicts intra-articular lesions of the long head of the biceps tendon.

BACKGROUND: Rotator cuff tears often coexist with intra-articular lesions in the long head of the biceps tendon (LHBT). Currently, biceps tendon injuries are often overlooked during treatment. Therefore, it is necessary to effectively identify LHBT lesions before surgery. This study aimed to predict intra-articular lesions using preoperative shear wave elastography (SWE) to assist with subsequent treatment. METHODS: This retrospective study included 71 patients with unilateral rotator cuff tears admitted to the hospital between September 2023 and August 2024. All patients underwent preoperati

Wearable Ultrasound Imaging Device for Dynamic Dual-Direction Shear Wave Elastography of Shoulder Muscle.

The shoulder is the most mobile joint in the human body, thus requiring intricate coordination of adjacent muscles. Patients suffered from rotator cuff muscle injuries have several typical symptoms including shoulder pain and difficulty raising the arm, thus reducing work efficiency and compromising the quality of life. Ultrasound has been used widely for shoulder soft tissue imaging as well as ultrasound elastography was introduced in shoulder examination for the dilemma of treating degenerative rotator cuff tears. However, most of the ultrasound examination was performed under a static condi

Musculotendinous infraspinatus ruptures: an overview.

BACKGROUND: Musculotendinous infraspinatus ruptures belong to the category of "muscle strain injuries". They differ from a classic rotator cuff tear by the following features: tendon attachment remains intact and the acute initial stage typically undergoes an intense muscular edema, replaced, 6 to 12 months later, by complete and permanent fatty infiltration of this muscle. METHODS: Between 1993 and 2007, we prospectively identified 59 musculotendinous ruptures of the infraspinatus muscle tendon complex. Fifty-eight percent of these patients were females. The average age at presentation was 5

Effect of Subacromial Bursa Augmentation on Self-Reported and Magnetic Resonance Imaging Outcomes in Arthroscopic Rotator Cuff Repair: A Prospective, Blinded Randomized Clinical Trial.

BACKGROUND: Rotator cuff repair (RCR) failure remains a persistent clinical challenge despite advances in surgical techniques. Although the subacromial bursa-historically excised during repair-is recognized for its regenerative potential due to its rich population of connective tissue progenitor cells, high-quality clinical evidence supporting its utility as an autologous biological adjunct remains lacking. HYPOTHESIS: Subacromial bursa augmentation during arthroscopic RCR improves patient-reported outcomes and magnetic resonance imaging (MRI)-based tendon healing compared with standard repai

Increased Critical Shoulder Angle Impairs Tendon-Bone Healing in a Rat Model of Chronic Rotator Cuff Tears.

BACKGROUND: The role of an elevated critical shoulder angle (CSA) in rotator cuff healing following rotator cuff repair (RCR) remains a subject of clinical controversy. The present study aimed to investigate the effect of increased CSA on tendon-bone interface healing following RCR. METHODS: A bilateral chronic rotator cuff tear model was established in 48 Sprague-Dawley rats. Acromion lateralization (Acr) surgery was performed unilaterally to increase CSA. After 4 weeks, bilateral RCR was performed. Micro-computed tomography was utilized to measure CSA. Tendon-bone interface healing was asse

Effect of controlled hypotension on postoperative awakening quality in shoulder arthroscopy with lateral decubitus rotator cuff repair.

BACKGROUND: Background: Shoulder arthroscopy is commonly performed to treat shoulder injuries, degenerative conditions, and chronic pain. The quality of postoperative emergence is critical to patient recovery and overall surgical outcomes. However, few studies have investigated whether controlled hypotension affects emergence quality during shoulder arthroscopy. Controlled hypotension can optimize the surgical field, reduce operative time, decrease the risk of intraoperative complications, and enhance the quality of postoperative recovery. These benefits highlight its clinical utility and supp
