Abnormal regional homogeneity and its correlations with personality in first-episode, treatment-naive somatization disorder.

BACKGROUND: Structural and functional abnormalities of the default mode network (DMN) and their correlations with personality have been found in somatization disorder (SD). However, no study is conducted to identify regional neural activity and its correlations with personality in SD. In this study, regional homogeneity (ReHo) was applied to explore whether abnormal regional neural activity is present in patients with SD and its correlations with personality measured by Eysenck Personality Questionnaire (EPQ). METHODS: Twenty-five first-episode, treatment-naive patients with SD and 28 sex-, a

Emotional reactions to pain predict psychological distress in adult patients with Sickle Cell Disease (SCD).

Differentiating somatic from emotional influences on the experience of chronic pain has been of interest to clinicians and researchers for many years. Although prior research has not well specified these pathways at the anatomical level, some evidence, both theoretical and empirical, suggest that emotional reactions influence the experience of disease and non-disease-related pains. Other studies suggest that treatments directed at negative emotional responses reduce suffering associated with pain. The current study was conducted to explore the influence of emotional reactions to pain as a pred

Caloric test and video-head-impulse: a study of vertigo/dizziness patients in a community hospital.

The head-impulse test (HIT) is an important test for examining unilateral vestibular hypofunction. The new video-head-impulse test (vHIT) is more sensitive and specific than the clinical bedside-head-impulse test (bHIT). Alternatively, one can test for vestibular hypofunction with the caloric irrigation test. Various studies showed that both tests may not always identify vestibular hypofunction; instead, the results of the tests might be contradictory. To evaluate the diagnostic value of these tests, we routinely measured patients exhibiting vertigo or dizziness at our community hospital with

[The psychic cross-sectional finding, cardinal point of psychiatric-psychotherapeutic expertise].

Expertise is based on history and clinical findings. Anamnestic data and clinical results have to be differentiated carefully. Clinical findings are of special importance because they are helpful in verifying anamnestic information with regard to extent and impairment quality of complaints, plausibility and thereby their significance for the expertise. The author is of the opinion that the clinical-psychic finding is in danger of being put in the background. With the victory of the descriptive diagnosis and the international diagnostic systems ICD-10 and DSM-IV, some examiners are tempted to r

Chronic fatigue, abuse-related traumatization, and psychiatric disorders in a community-based sample.

The relationship between sexual and physical abuse history and negative health effects has been well-documented in medical facility samples. Few studies have examined the role of abuse history and its relationship with chronic fatigue and psychiatric disorders in a diverse, randomly selected community-based sample. The present study compared rates of different types of abuse events in individuals with chronic fatigue and non-symptomatic controls. Relationships between specific types of abuse and psychiatric disorders commonly associated with chronic fatigue were also explored. A stratified ran

Clinical features and prognosis of nonepileptic seizures in a developing country.

PURPOSE: To determine the predictive value of clinical features and medical history in patients with nonepileptic seizures (NESs). METHODS: One hundred sixty-one consecutive ictal video-EEGs were reviewed, and 17 patients with 41 NESs identified. NES diagnosis was defined as paroxysmal behavioral changes suggestive of epileptic seizures recorded during video-EEC without any electrographic ictal activity. Clinical features, age, sex, coexisting epilepsy, associated psychiatric disorder, social and economic factors, delay in reaching the diagnosis of NES, previous treatment, and correlation wit

Factors related to dissociation among patients with gastrointestinal complaints.

OBJECTIVES: Several recent reports have suggested that the process of dissociation is independently associated with several distressing conditions such as psychiatric diagnoses, somatization, current psychological distress, and past sexual and physical victimization. These studies, however, have not taken into account possible shared variance between these variables. Is dissociation uniquely related to each of these outcomes or do they, as a group, have common underlying factors that account for the relationship with dissociation? METHOD: As part of a larger study of gastroenterology clinic p

From diversity to unity. The classification of mental disorders in 21st-century China.

Psychiatric disease constructs represent social constructs and genuine states of distress that have biopsychosocial sources. As such, they have social uses peculiar to the social groups in which they are created and legitimized. This is as true in the United States as in the rest of the world. The DSM schema, for instance, is so organized that every possible mental condition is listed as a disease to legitimate remuneration to practitioners from private medical insurance and government programs. This particular social use may be irrelevant to other societies where health care is financed diffe

Adverse childhood experience and adult persistent pain and disability: protocol for a systematic review and meta-analysis.

BACKGROUND: A growing body of research highlights the pervasive harms of adverse childhood experiences (ACEs) on health throughout the life-course. However, findings from prior reviews and recent longitudinal studies investigating the association between types of ACEs and persistent pain have yielded inconsistent findings in the strength and direction of associations. The purpose of this review is to appraise and summarize evidence on the relationship between ACEs and persistent pain and disability outcomes in adulthood. The specific aims are (1) to determine whether there is a relationship be

Testing two global models to prevent violence against women and children: methods and baseline data analysis of a seven-year prospective study.

Worldwide, two models of care are offered most often to abused women-safe shelter and justice services. No evidence exists on the differential effectiveness of the models. To provide evidence for best practice and policy, 300 abused women, 150 first-time users of a shelter and 150 first time-applicants for a protection order, participated in a seven-year study. Safety, abuse, and the emotional and physical functioning of the women and their children were measured. The procedural logistics, sampling process, metrics, and baseline descriptors for these 300 women and 300 children is presented alo

Approach to the patient with multiple somatic symptoms.

Primary care providers play a crucial role in the recognition and appropriate treatment of patients with multiple somatic complaints. Both the number of somatic symptoms and the persistence of symptoms are associated with co-occurring depression or anxiety disorders. It can be challenging to simultaneously address possible medical causes for physical symptoms while also considering an associated psychiatric diagnosis. In this article, strategies to improve the care and outcomes among these patients are described, including collaboration, education about the interaction between psychosocial str

Subclinical self-harm: range of behaviors, extent, and associated characteristics.

This study examined characteristics associated with mildly injurious (fingernail biting, skin picking, etc.) and more injurious (cutting, burning, etc.) self-harm (SH) in an undergraduate sample (N = 280); 31% reported mildly injurious SH within the past 3 years with no more injurious SH, whereas 20% reported more injurious SH within the past 3 years. SH was not associated with significant general negative affect or history of physical or sexual abuse, although more injurious SH was associated with a history of emotional abuse. Anxiety sensitivity and depression in multiple chemical sensitivities and asthma.

Patients with sensitivities to multiple chemicals report symptoms of cognitive dysfunction, respiratory distress, and mood disturbance. Lifetime and current psychiatric disorders, personality traits associated with symptom reporting, and tests of cognitive function were compared between 30 subjects with Multiple Chemical Sensitivities (MCS), 19 asthmatics, and 31 healthy controls. Relative to asthmatics and controls, more MCS subjects met criteria for current depression and somatization disorder. MCS subjects and asthmatics scored significantly higher than controls on scales of chemical odor i

Cerebellar microstructural abnormalities in patients with somatic symptom disorders.

BACKGROUND: Somatic Symptom Disorder (SSD) is a condition often linked to excessive health anxiety and somatic symptoms. In recent years, studies have found associations between the cerebellum and various mental illnesses, including SSD. However, the microstructure of cerebellar subregions in SSD using diffusion magnetic resonance imaging has not been fully defined. METHODS: This is a cross-sectional study, that included 30 SSD patients and 30 age- and gender-matched healthy controls to investigate the microstructure of the cerebellum using diffusion magnetic resonance imaging. SSD diagnosis

Do negative childhood conditions increase the risk of somatic symptoms in adolescence? - a prospective cohort study.

BACKGROUND: In order to prevent health and social problems later in life, it is important to identify childhood conditions related to the development of somatic symptoms. This prospective study expands on previous research by investigating whether negative childhood conditions are related to somatization later in life, taking other risk factors into account. This study aims to investigate whether somatic symptoms of the participants' parents, poor family functioning, or negative life events during childhood result in somatic symptoms in early or late adolescence. METHODS: The study population

Modeling the development of panic disorder with interoceptive conditioning.

Panic disorder is characterized by the paroxysmal occurrence and fear of bodily symptoms. In recent years it has been proposed that patients "learn" to fear cardiorespiratory sensations through interoceptive conditioning. This study sought to model the initial stage of this process in healthy volunteers (N=44) using mild cardiac sensations. An additional aim was to explore whether anxiety sensitivity - a known risk factor for panic disorder - modulates such interoceptive learning. Infusions of pentagastrin and saline were used to manipulate the presence versus absence of cardiac sensations, re

Understanding self-management behaviors in symptomatic adults with uncertain etiology using an illness perceptions framework.

The self-management behaviors of individuals with medical conditions that have an unknown etiology have not been studied. This study assesses the relationship between illness perceptions and various illness self-management behaviors in patients undergoing clinical genomic sequencing to identify a genetic cause for their condition. Hierarchical linear regression, Poisson linear regression, and logistic regression were used to assess the effect of illness perceptions (i.e., perceived consequences, timeline, personal control, treatment control, identity, concern, understanding, emotional impact,
