Simultaneous anterograde fast-slow atrioventricular nodal pathway conduction after procainamide.

UNLABELLED: Three patients with paroxysmal supraventricular tachycardia underwent electrophysiologic studies that included His bundle recordings, incremental atrial and ventricular pacing and extrastimulation before and after intravenous infusion of 500 mg of procainamide. In all three patients the tachycardia was induced during atrial pacing or premature atrial stimulation, or both. Two of the three patients had discontinuous atrioventricular (A-V) nodal curves with induction of a slow-fast tachycardia during failure in anterograde fast pathway conduction and one patient had a smooth A-V noda

Echocardiography fails to detect an extensive aortic root abscess in a patient with infective endocarditis: a case report.

BACKGROUND: Echocardiography plays a central role in the diagnosis of infective endocarditis (IE). In recent years, additional imaging techniques have begun to challenge the conventional approach. We present a case where the use of transthoracic/transoesophageal echocardiography (TTE/TOE) in suspected IE failed to identify an extensive periannular abscess, later identified by 18F-flurodeoxyglucose-positron emission tomography (FDG-PET), requiring urgent intervention. CASE SUMMARY: A 69-year-old man with symptomatic Streptococcus sanguinis bacteraemia and a bicuspid aortic valve was found to h

Slow/fast atrioventricular nodal reentrant tachycardia catheter ablation guided by atrial resetting: The new insight.

BACKGROUND: Atrial resetting can be used to discern the anterograde pathway in slow/fast atrioventricular nodal reentrant tachycardia (AVNRT). OBJECTIVE: We aimed to assess the prevalence of right inferior extension (RIE) and left extension (LE) and the potential impact on the ablation approach. METHODS: During the electrophysiologic study of patients with slow/fast AVNRT, a decremental supraventricular extrastimulus was delivered within the vulnerability window of the tachycardia cycle at 2 distinct sites: the inferoparaseptal area of the Koch triangle (near the RIE) and the proximal few ce

Predictors of non-pharmacological intervention in patients with paroxysmal atrial fibrillation: Value of neuroticism.

BACKGROUND: Non-pharmacological intervention is gaining increasing popularity in the treatment of patients with paroxysmal atrial fibrillation. We sought to investigate which factors play a role in the choice for non-pharmacological intervention with a particular focus on neuroticism. METHODS: The study group comprised 73 patients with paroxysmal atrial fibrillation (mean age 55+/-13 years, 50 males). On average, patients had a 3-year-history of one symptomatic paroxysm per week lasting 2 h. The degree of neuroticism was assessed using the short scale Eysenck Personality Questionnaire. RESUL

Reliability of retrograde atrial activation patterns during ventricular pacing for localizing accessory pathways.

Definitive localization of accessory pathways is based on atrial activation patterns during orthodromic supraventricular tachycardia when retrograde conduction occurs exclusively through the accessory pathway. In some patients, supraventricular tachycardia cannot be induced or is deleterious. To determine whether accessory pathway sites can be identified accurately during ventricular pacing, retrograde atrial activation was assessed during orthodromic supraventricular tachycardia and ventricular pacing at multiple cycle lengths in 41 patients with a single accessory pathway. To obviate retrogr

A case of near-fatal flecainide overdose in a neonate successfully treated with sodium bicarbonate.

BACKGROUND: Flecainide is a class IC antidysrhythmic primarily indicated for ventricular dysrhythmias and supraventricular tachycardia (SVT). Class IC antidysrhythmic overdose has a reported mortality of 22%, and death results from dysrhythmias and cardiovascular collapse. We report a near-fatal flecainide overdose in an 18-day-old treated successfully with sodium bicarbonate. CASE REPORT: An 18-day-old, 2 weeks premature, 4-kg boy developed persistently high heart rates (220-240 beats/min) and electrocardiographic changes consistent with SVT. There was minimal response to vagal maneuvers, ad

Cryosurgical modification of retrograde atrioventricular conduction. Implications for the surgical treatment of atrioventricular nodal reentry tachycardia.

Paroxysmal supraventricular tachycardia resulting from atrioventricular nodal reentry is a common arrhythmia that usually responds to medical therapy. When atrioventricular nodal reentry tachycardia is refractory to medical therapy, cryoablation or endocardial catheter ablation of the His bundle has been employed to protect the ventricles from the tachycardia. However, these techniques necessitate implantation of a permanent ventricular pacemaker. A cryosurgical procedure that ablates the anatomic-electrophysiologic substrate for atrioventricular nodal reentry tachycardia while preserving ante

Voltage-Gradient Mapping-Guided Slow Pathway Ablation in Typical Atrioventricular Nodal Re-Entrant Tachycardia.

INTRODUCTION: High-density mapping is useful for common atrioventricular nodal re-entrant tachycardia (AVNRT) ablation. This study aimed to evaluate the effective targets for slow pathway (SP) ablation using voltage-gradient mapping. METHODS: Fifty-two patients diagnosed with slow/fast AVNRT were enrolled. Patients underwent SP ablation using either a voltage-gradient map (n = 20) or a conventional approach based on anatomic and electrophysiological findings (n = 32). The Ensite X EP system was used as the 3-D mapping system in all patients. The target CA site was defined as the location at w

Comparison of the catheter ablation outcome in patients between targeting the entrance and exit of the reentry circuit in verapamil-sensitive atrial tachycardia originating from the atrioventricular-node vicinity.

Verapamil-sensitive atrial tachycardia originating from the atrioventricular node vicinity (AVN-AT) can be eliminated with radiofrequency energy (RF) deliveries targeting either the entrance or exit of its reentry circuit. However, the outcome of these different approaches has not been clarified well. Thus, we compared the catheter ablation outcome targeting the entrance of reentry circuit, identified by the entrainment method (Ent-Group; 21 patients) with that targeting the earliest atrial activation site (EAAS) during AT (Exit-Group; 16 patients). There was no significant difference in the t

Selective atrionodal input ablation for induction of proximal complete heart block with stable junctional escape rhythm in patients with uncontrolled atrial fibrillation.

BACKGROUND: The study tests the hypothesis that ablating all inputs to the atrioventricular (AV) node can result in complete heart block with stable junctional escape rhythm. METHODS AND RESULTS: We attempted atrionodal input ablation in 76 consecutive patients with uncontrolled atrial fibrillation. Fast and slow pathways were first ablated. If there was no AV block, additional energy applications were done between fast and slow pathway locations. The patients were followed for 42 +/- 11 months. Group I (n = 57) comprised patients with complete heart block and junctional escape rhythm (53 +/-

Electrocardiographic identification of Para-Hisian right ventricular pacing predicts response to cardiac resynchronization therapy.

BACKGROUND: Cardiac resynchronization therapy (CRT) improves outcomes in patients with heart failure and electrical dyssynchrony; however, electrocardiographic markers identifying a favorable right ventricular (RV) pacing configuration have not been well established. OBJECTIVE: To evaluate whether an electrocardiography (ECG)-defined classification of RV pacing is associated with electrical and echocardiographic response to CRT. METHODS: We retrospectively studied 33 consecutive patients who underwent CRT-pacing. RV pacing configuration was classified using predefined ECG criteria consisting

The Role of Anticoagulation in Post-COVID-19 Concomitant Stroke, Myocardial Infarction, and Left Ventricular Thrombus: A Case Report.

BACKGROUND Coronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) primarily affects the lungs but can involve any organ. The medical community is struggling to cope with the critical illness associated with the disease. On top of that, patients who have recovered from COVID-19 have presented with complications such as thrombotic episodes in various organs both during and after being infected with SARS-CoV-2. A COVID-19-associated prothrombotic state has been mentioned in multiple recent research articles. The role of anticoagulants is debatab

Spatiotemporal regulation of an Hcn4 enhancer defines a role for Mef2c and HDACs in cardiac electrical patterning.

Regional differences in cardiomyocyte automaticity permit the sinoatrial node (SAN) to function as the leading cardiac pacemaker and the atrioventricular (AV) junction as a subsidiary pacemaker. The regulatory mechanisms controlling the distribution of automaticity within the heart are not understood. To understand regional variation in cardiac automaticity, we carried out an in vivo analysis of cis-regulatory elements that control expression of the hyperpolarization-activated cyclic-nucleotide gated ion channel 4 (Hcn4). Using transgenic mice, we found that spatial and temporal patterning of

Orthodromic Atrioventricular Reentry Bradycardia and Tachycardia Caused by an Accessory Pathway in Horses.

BACKGROUND: Accessory pathways (APs) are muscular bundles directly connecting the atria and ventricles, bypassing the atrioventricular (AV) node-His-Purkinje system. Anterograde conduction along the AP results in ventricular preexcitation. A retrograde conducting AP allows ventriculo-atrial (VA) conduction, creating a reentry circuit that mediates orthodromic atrioventricular reentry tachycardia (OAVRT). This condition is well described in humans and small animals, but has not been reported previously in horses. OBJECTIVES: Describe clinical and electrocardiographic findings of horses with re

Large-scale purification and in vitro characterization of the assembly of MreB from Leptospira interrogans.

BACKGROUND: Weil's syndrome is caused by Leptospira interrogans infections, a Gram negative bacterium with a distinct thin corkscrew cell shape. The molecular basis for this unusual morphology is unknown. In many bacteria, cell wall synthesis is orchestrated by the actin homolog, MreB. METHODS: Here we have identified the MreB within the L. interrogans genome and expressed the His-tagged protein product of the synthesized gene (Li-MreB) in Escherichia coli. Li-MreB did not purify under standard nucleotide-free conditions used for MreBs from other species, requiring the continual presence of A

The history of left septal fascicular block: chronological considerations of a reality yet to be universally accepted.

There are several papers in literature that prove in a conclusive and incontestable way, that the left branch of the His bundle, in most instances (85% of the cases) splits into three fascicles of variable morphological pattern, and not into two: left anterior fascicle (LAF), left posterior fascicle (LPF), and left septal fascicle (LSF). The abovementioned papers have anatomical, histological, anatomo-pathological, electrocardiographic, and vectocardiographic, body surface potential mapping or ECG potential mapping and electrophysiological foundation.Additionally, the mentioned papers have bee

Hemiblocks revisited.

The trifascicular nature of the intraventricular conduction system and the concept of trifascicular block and hemiblock were described by Rosenbaum and his coworkers in 1968. Since then, anatomic, pathological, electrophysiological, and clinical studies have confirmed the original description and scarce advances have been developed on the subject. In the present study, we attempt to review and redefine reliable criteria for the electrocardiographic and vectorcardiographic diagnosis of left anterior and posterior hemiblock. One of the most important problems related to hemiblocks is that they m

Ultra-high-frequency electrocardiography identifies preserved ventricular synchrony during left bundle branch area pacing compared with right ventricular pacing.

BACKGROUND: Right ventricular pacing (RVP) may induce nonphysiological ventricular activation and electrical dyssynchrony, whereas left bundle branch area pacing (LBBAP) is intended to recruit the His-Purkinje system and preserve a more physiological activation pattern. OBJECTIVE: To compare paced-state ventricular activation profiles during LBBAP and RVP using ultra-high-frequency electrocardiography (UHF-ECG). METHODS: In this single-center pilot comparative study, 33 patients undergoing permanent pacing were analyzed (16 LBBAP, 17 RVP). The UHF-ECG comparison was performed exclusively dur
