PURPOSE: To compare the density of iris collagen and anterior segment parameters in eyes with chronic primary angle closure glaucoma (CPACG) and their fellow eyes with confirmed or suspected primary angle closure (PAC/PACS).
METHODS: Nineteen patients with CPACG in one eye and PAC/PACS in the fellow eye requiring trabeculectomy in the CPACG eye and iridectomy in the fellow eye were recruited. Anterior segment optical coherence tomography (AS-OCT) measurements were conducted under light and dark conditions. Iris specimens, obtained by iridectomy/trabeculectomy, were analyzed by sirius red pola

INTRODUCTION: Microphthalmos and nanophthalmos are uncommon ocular conditions, whereby affected eyes have smaller dimensions compared to the normal population. Microphthalmos and nanophthalmos present several challenges to ophthalmologists; they have spontaneous and post-operative sequelae such as high hyperopia, angle-closure glaucoma, uveal effusion syndrome, and retinal detachment. This systematic review and meta-analysis intends to assess the prevalence of both the spontaneous complications associated with nanophthalmos and microphthalmos, as well as the post-surgical complications associa

We report a case of acute intraocular pressure (IOP) elevation caused by pupillary occlusion in a Japanese man in his 90s. The patient presented with a left frontal headache and vomiting, and ophthalmologic evaluation revealed marked IOP elevation, a shallow anterior chamber, and pupillary membrane formation obscuring the lens. While anterior segment optical coherence tomography (AS-OCT) was unable to confirm the lens position, ultrasound biomicroscopy (UBM) clearly demonstrated the absence of lens displacement or intraocular mass. Yttrium aluminum garnet (YAG) laser membranotomy promptly deep

PURPOSE: Cataract surgery in patients with nanophthalmos is complicated for ophthalmologists to perform. Due to the unique ocular anatomy, there is a high incidence of complex complications such as angle-closure glaucoma, fluid misdirection syndrome, and uveal effusion syndrome (UES) in the perioperative period of cataract surgery. This article will discuss the management options for cataract surgery in nanophthalmic eyes and complications.
METHODS: This review is searched through PubMed, focusing on articles published in the past 20 years. Articles were reviewed on the anatomical structure o

Primary ocular lymphoma is the ocular manifestation of primary oculocerebral non-Hodgkin's lymphoma. We describe a 79-year-old woman with a 7-year history of bilateral uveitis and subsequent central nervous system lymphomas. Repeated diagnostic vitrectomy during the following 5 years failed to demonstrate intraocular lymphoma cells. Within 9 months after the second vitrectomy, an epibulbar tumor developed in the limbal region of the left eye at the site of the sclerotomy. The eye, blind and painful due to secondary angle-closure glaucoma, was enucleated. Histopathologically, the globe showed a

Neovascular glaucoma (NVG) secondary to diabetic retinopathy (DR) is a severe complication driven by ischemia-induced angiogenesis. Current evidence indicates that the pathogenesis begins with retinal hypoxia caused by hyperglycemia-induced capillary occlusion, which stabilizes hypoxia-inducible factor-1α (HIF-1α) and upregulates vascular endothelial growth factor (VEGF). VEGF-A and placental growth factor (PlGF) drive abnormal angiogenesis in the retina, iris, and anterior chamber angle. Downregulation of endogenous inhibitors such as pigment epithelium-derived factor (PEDF) may exacerbate th

OBJECTIVE: To evaluate the efficacy of "straight scleral tunnel incision" -trabeculectomy with a releasable suture supplemented with mitomycin C (MMC) on reducing intraocular pressure (IOP), complications, and corneal astigmatism in patients with primary angle-closure glaucoma (PACG).
METHODS: Totally 217 acute or chronic PACG patients with occludable angle above 180 degrees and IOP above 21 mm Hg were divided into 3 groups. Patients in group A (98 cases, 128 eyes), B (71 cases, 95 eyes), and C (48 cases, 60 eyes) were treated with "straight scleral tunnel incision"-trabeculectomy with a rele

Five times more frequent than the acute form, chronic closed-angle glaucoma often goes unrecognized for a long time, resulting in considerable visual field deficiencies, even in loss of the eye. It is sometimes confused with chronic glaucoma and treated as such, which is inadequate to halt the progression of the disease. Only gonioscopy can diagnose it. If doubt persists, UBM (ultrasound biomicroscopy) can detect goniosynechiae, a malposition of the ciliary body or of the lens, or the existence of iridociliary cysts. Nine times out of ten, pupillary block initiates the process and an iridotomy

We evaluated the prophylactic effect of 0.25% Clonidine HCl on the increase of intraocular pressure after Nd: YAG laser therapy. IOP of 150 eyes using Clonidine and 119 eyes without Clonidine were measured before and 0.5, 1, 2, 24 hours after Nd: YAG application. Various stages of angle-closure glaucoma and after cataract were both included in the treated and control groups and were compared respectively. Overall, the incidence of increase of IOP in the Clonidine-treated group was significantly less (9.3%) than the group without Clonidine (63.0%); the incidence of marked increase (> 1.06kPa) w

To evaluate the prevalence of primary cornea guttata (the first stage of Fuchs' dystrophy) in Japanese patients, we retrospectively examined specular photomicrographs of patients' cataracts. Primary cornea guttata in subjects without corneal inflammation was defined as dark areas, seen on specular microscopy, and irregular refractile dots, observed with slit-lamp microscopy. In 107 patients with cataract (211 eyes), primary cornea guttata was found in 4 women (7 eyes), aged 56 to 76 years. One eye of patient 4, who had a history of angle-closure glaucoma and laser iridotomy, had bullous kerato

Argon laser iridotomy was performed in 123 eyes of 83 patients with primary angle-closure glaucoma. Sixty-three eyes received argon laser iridotomies with timolol pretreatment and the other 60 eyes served as controls. There were 43 eyes (71.7%) in the control group and 32 eyes (50.7%) in the timolol-pretreated group, whose intraocular pressure increased significantly over baseline level 2 hours after laser treatment. There was statistical difference in the incidence of intraocular pressure elevation in these two groups. Seven eyes (11.7%) in the control group and 4 eyes (6.2%) in the timolol-p
