Improving Efficiency of Hip Fracture Care by Simplifying Wound Management and Eliminating Unnecessary Clinical Follow-Up.

BACKGROUND: We conducted a retrospective review of geriatric hip fractures at our institution evaluating how a change in practice to 2-octyl cyanoacrylate adhesive (Dermabond®) with polyester mesh (Prineo®) and elimination of the 2-week follow-up visit impacts quality and efficiency of care after hip fracture. Our aim was to determine the impact of simplified wound closure and extended clinical follow-up on the number of outpatient calls to nurses and wound complications. METHODS: Patients included in this assessment were aged ≥65 years who underwent surgical fixation or hip replacement for p

Short Term Complications in Geriatric Ankle Fractures Using a Protocolized Approach to Surgical Treatment: Is Early Weight Bearing Safe?

BACKGROUND: Surgical management of geriatric ankle fractures requires unique considerations in addressing operative risks. Prior studies have reached varying conclusions regarding optimal treatment strategies. The primary aim of this study was to determine if surgical fixation following a predetermined treatment protocol was safe and effective. The secondary aim was to determine if immediate weight bearing as tolerated (IWBAT) in a subset of patients was safe or conferred any short-term benefits. METHODS: This retrospective study included all patients over the age 65 treated surgically for an

The Cost-Effectiveness of Reverse Total Shoulder Arthroplasty Versus Open Reduction Internal Fixation for Proximal Humerus Fractures in the Elderly.

BACKGROUND: Open reduction and internal fixation (ORIF) of proximal humerus fractures in elderly individuals (age >70) carries a relatively high short-term complication and reoperation rate but is generally durable once healed. Reverse total shoulder arthroplasty (RTSA) for fractures may be associated with superior short-term quality of life but carries the lifelong liabilities of joint replacement. The tradeoff between short and long-term risks, coupled with disparities in quality of life and cost, makes this clinical decision amenable to cost-effectiveness analysis. METHODS: A Markov state-

Sarcopenia is Associated with Nonunion of Open Tibia and Ankle Fractures.

BACKGROUND: Sarcopenia is a clinical syndrome of diminished muscle mass and function associated with disability, poor surgical outcomes, and mortality. Open fractures of the tibia and ankle have a high risk for complications including nonunion and surgical site infection (SSI). The purpose of this study is to determine if sarcopenia is associated with SSI and nonunion in individuals that sustain open fractures of the tibia and ankle. METHODS: 111 consecutive adults who underwent operative fixation of open fractures of the tibia or ankle from 2006-2017 with preoperative CT of the abdomen and p

Haemophilus Parainfluenzae Septic Arthritis Following Primary All-Inside Meniscus Repair: A Case Report and Review of the Literature.

Background: Haemophilus parainfluenzae (H. parainfluenzae) is a gram-negative rod that inhabits the oral cavity. It is a common cause of respiratory tract infections and rarely is responsible for musculoskeletal infections in immunocompetent hosts. We present a case of a 17-year-old male whose postoperative course following arthroscopic all-inside meniscus repair was complicated with H. parainfluenzae septic arthritis. The infection was successfully cleared with two arthroscopic irrigation and debridements and antibiotic therapy. The patient successfully returned to full-contact high school fo

Combined Surgical Approach to Young Adults with Hip Dysplasia and Concomitant Intra-Articular Pathology Using Intra-Abdominal Monitoring.

BACKGROUND: Combined hip arthroscopy and periacetabular osteotomy (PAO) allows for treatment of intra-articular hip pathology with simultaneous correction of acetabular version and femoral head coverage in patients with symptomatic hip dysplasia. Currently, scant data is available to surgeons regarding optimal technique, sequence of repair, perioperative management, and the use of intra-abdominal monitoring in patients undergoing these combined procedures. The purpose of this study is to describe a two-surgeon, muscle-sparing, approach for sequential hip arthroscopy and PAO for the treatment o

Is There a Connection Between Attention Deficit Hyperactivity Disorder and Osteochondritis Dissecans?

BACKGROUND: To determine if children with Osteochondritis Dessicans (OCD) lesions of the distal femur are more likely to have a co-morbid diagnosis of Attention Deficit/ Hyperactivity Disorder (ADHD) than age matched controls and to assess the impact of ADHD on OCD outcomes. METHODS: A retrospective chart review of patients treated at a single tertiary care hospital between 2000-2012 was performed. Charts were reviewed for a diagnosis of OCD of the distal femur in all skeletally immature patients (males < 16 years and females < 14 years). These were then screened for a comorbid diagnosis of A

Effectiveness of Nonoperative Treatment of the Symptomatic Accessory Navicular in Pediatric Patients.

BACKGROUND: Initial management of symptomatic accessory navicular in pediatric patients is nonoperative. However, efficacy of nonoperative treatment has not been studied or established. If nonoperative treatment is frequently unsuccessful or does not give lasting pain relief, surgery could be offered as first line treatment. This study retrospectively reviewed outcomes of pediatric patients treated nonoperatively for symptomatic accessory naviculae in an effort to provide clinicians success rates for their discussion of treatment options with patients and their families. METHODS: A retrospect

Arthroscopic Hip Joint Assessment can Impact the Indications for PAO Surgery.

BACKGROUND: The purpose of this study was to analyze clinical, radiographic and intraoperative disease characteristics of patients with symptomatic acetabular dysplasia in which periacetabular osteotomy (PAO) was contraindicated due to advanced intraarticular findings at the time of disease staging hip arthroscopy (HA). METHODS: A prospective cohort was used to identify all patients who were scheduled for a PAO and concomitant hip arthroscopy for the treatment of symptomatic acetabular dysplasia. From a total of 286 patients (286 hips), 11 patients (11 hips) were identified in whom the PAO wa

The Influence of Different Rotator Cuff Deficiencies on Shoulder Stability Following Reverse Shoulder Arthroplasty.

BACKGROUND: The primary indication for reverse shoulder arthroplasty (RSA) is rotator cuff arthropathy caused by a deficient rotator cuff. Cuff deficiency in patients is highly variable in its distribution and extent, with mechanical implications that may significantly affect post-operative recovery. This study investigated the effects of variable cuff deficiency on the propensity for impingement between the scapula and humeral component and resulting subluxation, the source of two common complications (scapular notching and instability). METHODS: Five different finite element models of an RS

The Deformity TLIF: Bilateral Facetectomy and Osteotomy Closure with a Hinged Table.

BACKGROUND: Increasing emphasis has been placed on segmental lordosis correction, even in short segment constructs. However, the majority of reports on TLIF indicate that lordosis correction is modest at best. TLIF with bilateral facetecomy has been described with better lordosis correction, but is usually performed with the spine in extension throughout the case. This report presents a new technique for lordosis correction during TLIF with the use of bilateral facetectomy and osteotomy closure using a mechanically hinged operative table. METHODS: A 78-year-old male presented with claudicator

Acute Onset Atypical Severe Scoliosis: A Case Report.

BACKGROUND: Although most cases of scoliosis are idiopathic, scoliosis may also be congenital or associated with other diseases. Herniated Nucleus Pulposus (HNP) has been reported as a potential cause of non-structural scoliosis. HNP is unusual in adolescents and the clinical features are typically different from those in adults. CASE PRESENTATION: An 18-year-old Caucasian male was referred to our ambulatory service for evaluation of scoliosis after orthopedic evaluation at another center. He had noticed left-sided low back pain in the previous 6 months, which had worsened over the last two m

The Significance of a "Close" Margin in Extremity Sarcoma: A Systematic Review.

BACKGROUND: An important measure of successful sarcoma treatment is the surgical tumor margin, yet defining and reporting the tumor margin has remained a source of controversy. Our study sought to determine whether there is a need to be more specific in classifying a margin by distinguishing a 'close' margin, or if simply calling a margin positive or negative is sufficient. METHODS: We performed a comprehensive literature search in which all studies were reviewed independently by two separate reviewers. Studies eligible for inclusion and data analysis consisted of those that reported on at le

Crossed K-Wires Versus Intramedullary Headless Screw Fixation of Unstable Metacarpal Neck Fractures: A Biomechanical Study.

BACKGROUND: Intramedullary headless screw (IMHS) has shown promise as an alternative to other fixation devices for metacarpal neck fractures. The purpose of this study was to assess the biomechanical performance of IMHS versus the commonly-used crossed K-wire technique. We hypothesized that IMHS fixation provides superior stability to K-wires. METHODS: A metacarpal neck fracture model in 23 human cadaveric metacarpals was created. The specimens were divided into two groups based upon fixation method: Group 1, 3 mm intramedullary headless screw; and Group 2, 0.045 inch crossed K-wires. A canti
