Hypothyroidism Is Associated With Increased Medical and Surgical Complications Following Total Joint Arthroplasty: A Systematic Review and Meta-Analysis.

BACKGROUND: Total joint arthroplasty (TJA) is an effective treatment for end-stage osteoarthritis. However, hypothyroidism, even when medically controlled, has been linked to higher rates of postoperative complications. This meta-analysis assessed the impact of hypothyroidism on outcomes after total hip or total knee arthroplasty. METHODS: A comprehensive search was conducted in accordance with Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines across PubMed, Embase, Web of Science, and Scopus through October 2024. Studies comparing outcomes after TJA by thyroid sta

Sarcopenia in total joint Arthroplasty: Risk factor for poor postoperative outcomes and higher costs of care.

BACKGROUND: For patients undergoing total joint arthroplasty (TJA), sarcopenia has been described as a modifiable risk factor for several medical complications and adverse functional and patient-reported outcomes. However, findings have varied among orthopaedic literature as some studies encompass patients who do not have a clinical diagnosis of sarcopenia. This systematic review therefore sought to clarify the association between clinically diagnosed sarcopenia and post-TJA (1) functional outcomes, (2) patient-reported outcome measures (PROMs), and (3) complications. METHODS: This review was

Outcomes following robotic assisted total knee arthroplasty compared to conventional total knee arthroplasty.

INTRODUCTION: This study elaborates on previous research to compare length of stay, complication rates, and total cost between patients undergoing robotic assisted total knee arthroplasty (rTKA) and conventional total knee arthroplasty (cTKA). We hypothesized that patients undergoing rTKA would have reduced length of stay, lower complication rates, improved perioperative outcomes, and higher total healthcare costs than those undergoing cTKA. METHODS: Data were collected from the National Inpatient Sample Database Healthcare Cost and Utilization Project between the years 2016-2019. Patients un

Non-vehicle commuting in Italy: need for ergonomic action for women's lower limbs?

INTRODUCTION: The aim of this paper is to examine gender differences in the occurrence of accidents among workers while walking to or from their workplace (non-vehicle commuting accidents). We have previously found that the lower limb is more often injured at work among women, compared to men, so the paper concentrates on lower limb injuries. METHODS: Using the records of the National Institute for Insurance against Injuries at Work (INAIL), we focused on the non-vehicle commuting accidents of women and men recognized as work-related for the period 2013-2017. In particular, we examined the ge

Patellar Height Measurements on Radiograph and Magnetic Resonance Imaging in Patellar Instability and Control Patients.

Insall-Salvati (IS) ratio, modified IS ratio (mIS), Caton-Deschamps (CD) index, and Blackburne-Peel (BP) index are patellar height measurements commonly made on lateral radiographs (XRs) in patients with patellofemoral instability. More recently, magnetic resonance imaging (MRI) has been used as an additional imaging modality to measure patellar height. However, different imaging modalities have not been previously compared for all of these measurements. First, to compare the patellar height measurements of IS, mIS, CD, and BP on XR and MRI in first-time lateral patellar dislocation (LPD) pati

Modified Recession Wedge Trochleoplasty.

BACKGROUND: Trochlear dysplasia represents one of the main anatomic risk factors for patellar instability, with risk of failure and unfavorable clinical outcomes in patients with unaddressed dysplasia undergoing patellar stabilization. INDICATIONS: Patients with trochlear dysplasia characterized by supratrochlear prominence (DeJour B or D) and recurrent patellar instability, especially following failed soft tissue or bony stabilization. This technique addresses an anterior trochlea without pathologic convexity, as convexity may require conversion to other techniques for groove deepening (eg,

Preoperative Templating of Bone-Patellar Tendon-Bone Graft for Anterior Cruciate Ligament Reconstruction: A Morphometry-Based Graft Harvest Method.

BACKGROUND: Knowledge of anatomy and morphometry of the patella and patellar tendon is crucial for the selection of bone-patellar tendon-bone (BTB) graft for anterior cruciate ligament reconstruction. Graft tunnel mismatch in BTB graft especially in patients with patella alta or baja can result in compromised fixation for the bone-to-bone healing. This complication can be avoided by proper templating of graft using parameters measured from magnetic resonance imaging (MRI). The study aimed to derive morphometric data from MRI and predict the suitability of BTB graft preoperatively. METHODS: MR

Effects of surgical intervention on trochlear remodeling in pediatric patients with recurrent patella dislocation cases.

UNLABELLED: Patella instability is often encountered among physically active pediatric athletes, and surgical intervention is useful in cases with recurrent patella dislocations, chronic instability, and abnormal alignment. Several surgical procedures have been used for patella-realignment and stabilization, but the effects of surgical intervention on bony trochlear remodeling in skeletally immature patients have not been well studied. We thus present two cases of pediatric recurrent patella dislocations that showed trochlear remodeling following patella-realignment surgery. The first case des

How much articular displacement can be detected using fluoroscopy for tibial plateau fractures?

INTRODUCTION: While there is conflicting evidence regarding the importance of anatomic reduction for tibial plateau fractures, there are currently no studies that analyse our ability to grade reduction based on fluoroscopic imaging. The purpose of this study was to determine the accuracy of fluoroscopy in judging tibial plateau articular reduction. METHODS: Ten embalmed human cadavers were selected. The lateral plateau was sagitally sectioned, and the joint was reduced under direct visualization. Lateral, anterior-posterior (AP), and joint line fluoroscopic views were obtained. The same fluor

Clinical outcomes of medial patellofemoral ligament reconstruction in patients with an increased tibial tuberosity-trochlear groove distance.

PURPOSE: Medial patellofemoral ligament (MPFL) reconstruction is performed to treat recurrent patellar dislocation (RPD). However, the effectiveness of MPFL reconstruction in patients with a severely lateralised tibial tuberosity remains unknown. In this study, the clinical outcomes of MPFL reconstruction in patients with an increased tibial tuberosity-trochlear groove (TT-TG) distance were examined. METHODS: A total of thirty-four patients who underwent MPFL reconstruction for RPD were retrospectively examined. Nineteen patients with a TT-TG distance of >20 mm (increased TT-TG distance group

Patellar instability.

Patellar instability is a complex pathoanatomical phenomenon that requires an intricate understanding in order to properly treat patients. Often, the etiology of this entity is multifactorial, combining a series of physiologic and anatomic variables. A thorough history and physical as well as correct radiographic examinations are critical in both establishing the diagnosis and instituting the correct treatment. Non-operative management of recurrent instability has a high failure rate. Current operative techniques have been shown to be instrumental in correcting anatomic abnormalities, reducing
