[V.a.C.-therapy in abdominal surgery - experiences, limits and indications].

UNLABELLED: Compared to other surgical disciplines the significance of V.A.C.-therapy is rarely noticed in abdominal surgery. This may be due to uncertainty in defining clear indications or lack of technical know-how. METHODS: We report on five selected septic cases in abdominal surgery without clearly defined indications for V.A.C.-therapy (ischiorectal abscess, perforation in Crohn's disease, pelvic abscess due to perforation caused by rectal carcinoma, abdominal compartment syndrome, anastomotic leak after rectal resection). RESULTS: Regarding the individual aspects of indication, V.A.C.-

Combining Engineering Precision with Clinical Relevance: A Novel Dual Framework for Assessing Pedicle Screw Accuracy in Spine Surgery.

Background/Objectives: Accurate pedicle screw placement is critical in spine surgery, as malposition can cause neurological, vascular, or visceral injuries and compromise construct stability. The primary objective of this study was to develop and experimentally validate a dual quantitative framework for assessing pedicle screw placement accuracy, combining (1) coaxiality, a standardized geometric metric of trajectory alignment, and (2) pedicle wall distance (dpw), a novel parameter defined as the minimal distance between the screw axis and the pedicle cortex providing surgeons with direct, mil

Multi-Objective Optimization of Mechanical Properties for FDM-Printed PLA/TPU Blends via Box-Behnken Design and Entropy Weight Method.

Polylactic acid (PLA) is widely used in fused deposition modeling (FDM) due to its excellent mechanical properties and processability. However, its inherent brittleness significantly restricts its application in load-bearing and high-toughness scenarios. To address this limitation, this study establishes a multi-objective optimization framework that integrates single-factor experiments, Box-Behnken design (BBD), response surface methodology (RSM), and entropy weight-based objective weighting to simultaneously enhance the tensile strength, elongation at break, and flexural strength of FDM-print

Intrinsically disordered protein condensate-based coating on ureteral stents for anti-fouling and anti-encrustation.

Ureteral stents are among the most frequently used devices in urology, yet their high susceptibility to biofilm and encrustation continues to evade current surface-coating strategies. The development of emerging coatings faces significant challenges due to the complex physiological environment of the urinary tract, featuring high salinity, continuous shear stress, fluctuating pH, and microbial contamination. Here, we report an effective anti-fouling and anti-encrustation surface-engineering strategy by developing a coating from intrinsically disordered protein condensates of fused in sarcoma (

A novel method of fabricating dynamic, heterogenous benchtop lateral and third ventricle phantoms from MRI of hydrocephalus patients: a verification and validation study.

BACKGROUND: Investigation of the failure of shunts used to treat hydrocephalus has been a topic of research for several decades, though it is difficult to understand the role of heterogeneous patient ventriculomegaly shifts over time. In this work, we present the design, verification, and validation of a novel benchtop model of the human lateral and third ventricles. METHODS: 3D models were rendered from MRI of pre-revision hydrocephalic patients (n = 6), printed in hollow High Impact Polystyrene (HIPS) molds, and injected with silicone rubber allowed to rotationally cure for three hours. Distal versus conventional radial artery catheterization for hemodynamic monitoring in intensive care setting: a randomized, controlled, non-inferiority trial.

BACKGROUND: Arterial catheterization is fundamental for hemodynamic monitoring in critically ill patients, yet instability and loss of arterial pressure waveform remain clinically significant issues. The distal radial artery (dRA) approach has been proposed as a potential alternative to the conventional radial artery (RA) approach. This is the first randomized clinical trial to assess the comparative efficacy and reliability of dRA versus RA catheterization by examining first-attempt success rates and the stability of arterial pressure waveforms in critically ill ICU patients. METHODS: This r

Pterygoid implant: extensometric and photoelastic analysis of a maxillary rehabilitation model.

Pterygoid implants have been demonstrated to have a high success rate. Nevertheless, there are few biomechanical tests to evaluate the tensile forces induced by force dissipation in peri-implant tissues. This study employed photoelasticity and extensometry to demonstrate and compare the biomechanical behavior of non-axial implants in a pterygoid model and a conventional model of oral rehabilitation, thus allowing for qualitative and quantitative analysis. Two models received an implant measuring 3.75 x 13 mm in the canine pillar at a 90 ° angle to the Frankfurt plane. In the control group, an

Biomechanical comparative study of midline cortical vs. traditional pedicle screw trajectory in osteoporotic bone.

INTRODUCTION: In lumbar spinal stabilization pedicle screws are used as standard. However, especially in osteoporosis, screw anchorage is a problem. Cortical bone trajectory (CBT) is an alternative technique designed to increase stability without the use of cement. In this regard, comparative studies showed biomechanical superiority of the MC (midline cortical bone trajectory) technique with longer cortical progression over the CBT technique. The aim of this biomechanical study was to comparatively investigate the MC technique against the not cemented pedicle screws (TT) in terms of their pull

Vacuum-assisted closure for sternal wounds: a first-line therapeutic management approach.

BACKGROUND: Vacuum-assisted closure therapy has gained widespread use since its introduction in 1997. Previous studies have attributed significant benefit to its use for treatment of sternal wounds with or without mediastinitis. Management of sternal wounds with this therapy has been shown to decrease the number of dressing changes, reduce the time between débridement and definitive closure, and reduce costs associated with a protracted course of in-hospital dressing changes. The therapy has been used both as a bridge between débridement and definitive closure and as a catalyst to secondary st

Female condoms.

Early versions of a female condom were available in the 1920s and 1960s, but they were little used and soon forgotten. It took the arrival of AIDS, and the urgent need for a wider range of female-controlled barrier techniques, to rekindle scientific interest in this method. In the 1980s, three groups in Europe and the USA began development of new female condom designs, comprising 'Femidom (Reality)', the 'Bikini Condom', and 'Women's Choice'. Apart from differences in their physical design, Femidom differs from the others in that it is made of a polyurethane membrane, which has several advanta
