Minoxidil was used in 25 patients with severe hypertension whose blood pressure (BP) could not be controlled with conventional treatment or who suffered from intolerable side effects during treatment with other drugs. In 6 patients minoxidil was withdrawn after a short time owing to side effects or because hypertension could be controlled by regular dialysis treatment. The remaining 19 patients were treated with minoxidil for 0.5-4.5 years. The hypotensive effect of minoxidil in combination with beta-blockers and diuretics was good or acceptable in all patients. This research, based on a study of King George III's medical records and of contemporary diaries of his courtiers and equerries, further confirms the considerable doubt on the claim of Richard Hunter and Ida Macalpine that the King suffered from recurrent attacks of acute porphyria.The present study examines the above records from a psychiatric viewpoint, together with some additional reports, to re-assess the nature of the King's maladies. It concludes that he suffered from recurrent mania (four episodes), with chronic mania and possibly a degree of fatuity during the last decade of his life.

Many different drugs and agents have been reported to induce pseudoporphyria. Nonsteroidal antiinflammatory agents have been implicated as some of the most common causes. The first case reports of nabumetone (Relafen) induced pseudoporphyria have recently been published. We report two more cases of pseudoporphyria induced by nabumetone and stress the importance of physician awareness with the use of this drug.

A review of published trials with beta-carotene and/or canthaxanthin treatment of erythropoietic protoporphyria is presented. Increased tolerance to sunlight is reported in about 85% of the cases in uncontrolled studies. In the only controlled trial on record no difference was found between beta-carotene and placebo. Correlations between the results of carotenoid treatment and phototests have been reported in some studies, but denied in others. BACKGROUND: Erythropoietic protoporphyria (EPP) is an inherited disorder of heme synthesis, causing excess of protoporphyrin in blood, skin, liver and other organs. OBJECTIVE: Erythropoietic protoporphyria (EPP) is an inherited disorder of haem synthesis, causing excess of protoporphyrin in blood, skin, liver and other organs. Urine porphyrin analysis is an important part in evaluation of photosensitivity. Since porphyrin excretion is variable throughout the day, analysis is traditionally based on 24-hour collections. To facilitate the use of random specimens, as well as poorly collected 24-hour specimens, reference limits based on the porphyrin to creatinine ratio have been developed. Based on 1,171 adult specimens, it is estimated that the 95 percent reference limit (90 percent confidence interval) is < or = 3.9 (3.5-5.7) mumol/mol of creatinine for uroporphyrin and < or = 22 (19-34) mumol/mol for coproporphyrin.

The authors present 20 patients (9 men, 11 women) with erythropoietic protoporphyria (EPP). The diagnosis was made on the basis of photosensitivity and porphyrin analysis. The disease first became apparent in the first years of life.
