Impaired neonatal cardiorespiratory responses to hypoxia in mice lacking PAC1 or VPAC2 receptors.

The stress peptide pituitary adenylate cyclase activating polypeptide (PACAP) and its specific receptor PACAP type 1 receptor (PAC1) have been implicated in sudden infant death syndrome (SIDS). PACAP is also critical to the neonatal cardiorespiratory response to homeostatic stressors identified in SIDS, including hypoxia. However, which of PACAP's three receptors, PAC1, vasoactive intestinal peptide receptor type 1 (VPAC1), and/or vasoactive intestinal peptide receptor type 2 (VPAC2), are involved is unknown. In this study, we hypothesized that PAC1, but not VPAC2, is involved in mediating the…

Clinical Survey and Predictors of Outcomes of Pediatric Out-of-Hospital Cardiac Arrest Admitted to the Emergency Department.

Pediatric out-of-hospital cardiac arrest (OHCA) is a rare event with severe sequelae. Although the survival to hospital-discharge (STHD) rat … home (76.3%, n = 116). Most triggers of pediatric OHCA were respiratory in nature (53.2%, n = 81). Sudden infant death syndrome (SIDS) (29.6%, n = 45), unknown medical causes (25%, n = 38), and trauma (10.5%, n = 16) were the main causes of pediatric OHCA. Sixty-two initial cardiac rhythms at the scene were obtained, most of which were asystole and pulseless electri …

Prenatal nicotinic exposure prolongs superior laryngeal C-fiber-mediated apnea and bradycardia through enhancing neuronal TRPV1 expression and excitation.

Maternal cigarette smoke, including prenatal nicotinic exposure (PNE), is responsible for sudden infant death syndrome (SIDS). The fatal events of SIDS are characterized by severe bradycardia and life-threatening apneas. Although activation of transient receptor potential vanilloid 1 (TRPV1) of superior laryngeal C fibers (SLCFs) could induce bradycardia and apnea and has been implicated in SIDS pathogenesis, how PNE affects the SLCF-mediated cardiorespiratory responses remains unexplored. Here, we tested the hypothesis that PNE would aggravate the SLCF-mediated apnea and bradycardia via up-re…

Surgical correction for unilateral lambdoid synostosis: a systematic review.

BACKGROUND: Unilateral lambdoid synostosis is considered to be the rarest form of craniosynostosis. Since the introduction of the Sudden Infant Death Syndrome "back to sleep" campaign, the incidence of unilateral lambdoid synostosis was reportedly increasing. This was proven to be false and a consequence of non-specific diagnostic criteria in excluding suture fusion from deformational changes. This, in turn, led to ambiguity in the literature in terms of features and surgical correction in the 1980s to 1990s. OBJECTIVE OF REVIEW: We aimed to navigate the literature for true studies of unilate…

Brain stem serotonin protects blood pressure in neonatal rats exposed to episodic anoxia.

In neonatal rodents, a loss of brain stem serotonin [5-hydroxytryptamine (5-HT)] in utero or at birth compromises anoxia-induced gasping and … nounced, premature loss of MAP in response to episodic anoxia. These data may help explain why some sudden infant death syndrome cases die from what appears to be cardiovascular collapse during apparent severe hypoxia.

Heart rate variability abnormalities in young patients with dilated cardiomyopathy.

Heart rate variability (HRV) has become the conventionally accepted term for describing variations in both instantaneous heart rate and R-R intervals. In the pediatric age group, HRV has been investigated in healthy children, diabetics, respiratory distress syndrome of the newborn, and sudden infant death syndrome. This study aimed to evaluate HRV in pediatric patients with dilated cardiomyopathy and to compare it with that of age-matched normal subjects. The study evaluated 21 patients with dilated cardiomyopathy: 11 females (mean age, 7 ± 4 years; range, 2-17 years) and 10 males (mean age, 1…

Prenatal detection of the cystic form of meconium peritonitis: no issues for delayed postnatal surgery.

Prenatal ultrasound (US) diagnosis and postnatal outcome are reviewed in three babies with the complex form of meconium peritonitis (MP), the cystic type. Perinatal management is discussed. Large intra-abdominal cysts with signs of calcifications were detected during the second mid-trimester. Meconium ascites and polyhydramnios appeared between 32 and 35 weeks of gestation. Signs of anaemia were assessed on median cerebral artery peak systolic velocity. Sudden appearance of hydrops and anaemia required preterm delivery, neonatal resuscitation and urgent abdominal drainage. Postnatal US imaging

Repeated acute hypoxia temporarily attenuates the ventilatory respiratory response to hypoxia in conscious newborn rats.

We asked whether repeated hypoxic exposures during the early neonatal periods could affect the ventilatory control, such as the lung volume- … opment of respiratory control and could possibly be involved in the mechanisms of neonatal apnea or sudden infant death syndrome.

Biallelic NPR1 loss of function variants are responsible for neonatal systemic hypertension.

BACKGROUND: Early-onset isolated systemic hypertension is a rare condition of unknown genetic origin. Renovascular, renal parenchymal diseases or aortic coarctation are the most common causes of secondary systemic hypertension in younger children and neonates. We investigated the genetic bases of early-onset isolated systemic hypertension. METHODS: Whole-exome sequencing (WES) was followed by variant filtering and Sanger sequencing for validation and familial segregation of selected variants in a large consanguineous family. mRNA expression was performed to evaluate the impact of the predicte

Utilizing technology to expand home monitoring to high-risk infants with CHD.

Infants born with single ventricle physiology that require an aorto-pulmonary shunt are at high risk for sudden cardiac death, particularly during the interstage period between the first-stage palliation and the second-stage palliation. Home monitoring programs have decreased interstage mortality in the hypoplastic left heart syndrome population prompting programs to expand the home monitoring program to other high-risk populations. At our mid-sized program, we implemented the Locus Health home monitoring platform first in the hypoplastic left heart syndrome population, then expanding to the s

High-rate atrial pacing as an innovative bridging therapy in a neonate with congenital long QT syndrome.

INTRODUCTION: The neonatal presentation of the long QT syndrome is rare, although it is frequently accompanied by life-threatening arrhythmias. Infants may not survive childhood despite traditional management with beta-adrenergic blockade and pacing. METHODS AND RESULTS: This case describes a newborn with a long QT interval, T wave alternans, intermittent 2:1 AV block, ventricular arrhythmias, and a family history of sudden death. After failing medical therapy, conventional VVI and DDD pacing were unsuccessful due to prolonged ventricular refractoriness and proarrhythmia. At 2 months of life,

[MEP-18] Removal of Huge Cardiac Fibroma from the Right Ventricle in an Infant: Video Presentation.

Cardiac fibromas are rare primary benign tumors. They may cause symptoms such as intracavitary obstruction, coronary artery compression, thromboembolic events, conduction defects, and sudden death. They are typically intramural and are often found in the ventricles. In this video article, the surgical excision of a cardiac fibroma measuring 6.2×5×4.3 cm originating from the anterior wall of the right ventricle in an 11-month-old infant was presented. The tumor was located in the anterior wall of the right ventricle. Since exposure was insufficient from the right atrium, intervention could not

A CACNA1C variant associated with reduced voltage-dependent inactivation, increased CaV1.2 channel window current, and arrhythmogenesis.

Mutations in CACNA1C that increase current through the CaV1.2 L-type Ca2+ channel underlie rare forms of long QT syndrome (LQTS), and Timothy syndrome (TS). We identified a variant in CACNA1C in a male child of Filipino descent with arrhythmias and extracardiac features by candidate gene sequencing and performed functional expression studies to electrophysiologically characterize the effects of the variant on CaV1.2 channels. As a baby, the subject developed seizures and displayed developmental delays at 30 months of age. At age 5 years, he displayed a QTc of 520 ms and experienced recurrent V
