Static or dynamic repair of chronic lateral ankle instability. A prospective randomized study.

In a prospective study, 82 patients with 89 chronic lateral unstable ankles were randomly allocated to either static (Group S) or dynamic (Group D) reconstruction. For static reconstruction a procedure involving the distal part of the peroneus brevis tendon was used. The bore holes in the lateral malleolus were placed at the insertions of the original ligaments. For dynamic reconstruction the anterior one-half of the peroneus brevis tendon was used. On muscular contraction the split tendon will slide through a bore canal in the lateral malleolus. Theoretically, both procedures can stabilize th

Congenital dislocation of the knee.

Congenital dislocations of the knee (CDK) are rare, occurring 40-80 times more rarely than congenital dislocation of the hip (CDH). In a multicentric study of the European Paediatric Orthopaedic Society, 56 cases of CDK were found in 46 babies. Many other malformations associated with CDK were noticed, and muscular abnormality was always found at the knee. According to the classification of Leveuf, three grades have to be considered: grade 3, or complete dislocation, was the most frequent. At birth, treatment consisted of physiotherapy with rigid splint. Twenty-four patients with CDK have been

Bristow-Latarjet procedure for recurrent anterior dislocation of the shoulder. A 2-5 year follow-up study on the results of 112 cases.

A follow-up study of 111 out of 112 patients operated on for shoulder joint dislocation according to the Bristow-Latarjet procedure during the years 1975 through 1979 in four Swedish hospitals is presented. The average follow-up time was 30 months (range 24-60 months). There were seven cases of significant recurrences (6 per cent). During follow-up, further surgery had been performed on four of these. Another eight patients (7 per cent) had experienced occasional insignificant subluxations. In one case neurolysis of the musculocutaneous nerve was undertaken because of postoperative paresis of

Improvement of function in hemiplegia after orthopaedic surgery.

Over a 25-year period, 1855 patients have been subjected to a variety of common orthopaedic procedures for correction of disabling postures resulting from spastic hemiplegia. A widely applicable rationale for surgical correction of these postures has been slowly established over these years of clinical research. These methods are submitted in this review article with the hope that others will espouse, criticize and modify them, towards a better understanding of this complex kinetic disorder.

Free functioning gracilis transfer for brachial plexus reconstruction using the internal mammary vessels as recipients: A case report.

Adult brachial plexus injuries are often associated with concomitant trauma to the axillary or subclavian vessels. In patients planned for free functioning gracilis transfer (FFGT) this poses a challenge to reconstructive surgeons where using the standard donor vessels can lead to endangering the circulation in the affected extremity or risk flap loss due to the poor perfusion pressures. This case report describes the use of a FFGT for upper limb reconstruction in a 22-year-old patient with a pan plexus injury and concomitant axillary artery injury following a high energy motorcycle accident.

Open reduction capsular arthroplasty using a modified Codivilla-Hey Groves-Colonna procedure augmented with tensor fascia lata graft, supported by Ilizarov hinged external fixator in a patient with chronic traumatic posterior hip dislocation.

Untreated traumatic hip dislocation is a rare condition in children. Capsular arthroplasty is a previously abandoned surgical technique in dealing with developmental dysplasia of the hips but not described in traumatic dislocations. We present a 2-year follow-up of a 7-year-old boy who sustained chronic traumatic posterior hip dislocation treated as a first case in published literature combining a modified version of the Codivilla-Hey Groves-Colonna procedure, using tensor fascia lata graft and application of a hinged Ilizarov frame. The patient is currently independent on all activities of da

Reinnervation of Extrinsic Finger Flexors and Brachioradialis 22 and 36 Months Following Traumatic Pan-Brachial Plexopathy: A Case Report.

A 25-year-old man sustained a right-sided brachial plexus injury from a high-velocity motocross accident. Physical examination and electromyography were consistent with a pan-brachial plexopathy with no evidence of axonal continuity. The patient underwent a spinal accessory to suprascapular nerve transfer and an intercostal to musculocutaneous nerve transfer with interpositional sural nerve grafts. He recovered MRC 4/5 elbow flexion and MRC 2/5 shoulder abduction and external rotation. Twenty-two months post-injury the patient displayed a flicker of flexion of his flexor pollicis longus and fl

Role of Functional Electrical Stimulation in Tetraplegia Hand Surgery.

The use of functional electrical stimulation (FES) to improve upper limb function is an established method in the rehabilitation of persons with tetraplegia after spinal cord injury. Surgical reconstruction is another well-established yet underused technique to improve the performance of the upper extremities. Hand surgery plays an essential role in restoring hand function, mobility, and quality of life in the tetraplegic population. The knowledge about the effects of FES on a structural and functional level is fundamental for understanding how and when FES can be used best to support the effe

Factors affecting functional outcome of opponens replacement in median nerve paralysis in leprosy.

In this study, the factors affecting functional outcomes of opponens replacement in median nerve paralysis in leprosy, were investigated. Fifty six patients who underwent opponens transfer between 1995 and 2001 were included in the study. The average duration of follow up is 7.3 years with a range of 4-10 years. Functional assessments were done through assessment of grip strength, pinch strengths and fine manipulation and the results were interpreted using Sundararaj (1984) criteria. 76.6% had excellent or good results, 13.3% fair and 10.1% poor results. Pinch strength, grip strength and fine

Achilles tendon reconstruction after electrical injury with a neurotized cross-leg fillet flap.

The authors present a case where a cross-leg fillet flap from an amputated limb allowed reconstruction of the contralateral disrupted Achilles tendon and provided neurotized soft tissue. Achilles tendon reconstruction of the left leg was achieved by utilizing vascularized extensor tendons from the dorsum of the right foot based on the dorsalis pedis circulation. Neurotization was accomplished end to side between a cutaneous nerve from the dorsum of the foot and the recipient sural nerve. Healing was complete and without complication. Cross-leg flaps including tendon and nerve may be used to re
