OBJECTIVE: To analyze the immediate and midterm angiographic and clinical results of stent placement in the endovascular treatment of intracranial cerebral aneurysms.
METHODS: Out of 330 cerebral aneurysms treated by endovascular approach in our neurovascular centre, stents have been used in 18 patients. Twelve aneurysms (66.7%) were acutely ruptured, four (22.2%) were unruptured, two (11.1%) were recanalized after initial coiling. In three patients (16.7%) stent placement was used for revascularization of acute vessel thrombosis during coiling. Angiographic follow-up was obtained in 13 (72.2

BACKGROUND: Thrombotic thrombocytopenic purpura (TTP) is a rare thrombotic microangiopathy caused by severe ADAMTS13 deficiency. Although TTP is known to produce diffuse microvascular neurological injury, its presentation as an acute large vessel occlusion (LVO) stroke requiring neurovascular intervention is poorly represented in the literature. When severe thrombocytopenia accompanies acute LVO, intravenous thrombolysis is contraindicated, and mechanical thrombectomy becomes the only available revascularization strategy.

BACKGROUND: Moyamoya syndrome carries a high risk of cerebral ischemia, and impaired cerebrovascular autoregulation may play a critical role. Autoregulation indices derived from near-infrared spectroscopy (NIRS) may clarify hemodynamic goals that conform to the limits of autoregulation.
OBJECTIVES: The aims of this pilot study were to determine whether the NIRS-derived indices could identify blood pressure ranges that optimize autoregulation and whether autoregulatory function differs between anatomic sides in patients with unilateral vasculopathy.
METHODS: Pediatric patients undergoing indi

Cognitive dysfunctions (CD) is the most common postoperative neurological complication in cardiac surgery, which is detected in 30%-65% of patients within 1 month after myocardial revascularization and 20%-40% of the examined, during the next five postoperative months. This problem is especially relevant for patients with arterial hypertension (AH). Goal - to study of the correlation relationship between the mean arterial pressure (MAP) and the velocity of blood flow in the middle cerebral artery (MCA) in patients with AH during coronary artery bypass grafting (CABG) with cardiopulmonary bypas

BACKGROUND: Although extracranial-intracranial (EC-IC) bypass is an effective treatment strategy for symptomatic moyamoya disease, surgeons need to be cautious regarding the possibility of postoperative hemorrhagic complications in patients with a concurrent coagulation disorder. Here, we describe a case of EC-IC bypass for moyamoya disease concurrent with von Willebrand disease type 1.
CASE DESCRIPTION: Following perioperative replacement of the von Willebrand factor, the patient showed an uneventful and uncomplicated clinical course.
CONCLUSION: This is the first reported case of EC-IC byp

Internal carotid artery (ICA) dissection with tandem internal carotid and middle cerebral artery occlusion may carry a poor prognosis even if intravenous recombinant-tissular plasminogen activator is administered. A better outcome may be expected with the combination of intravenous thrombolysis and endovascular methods (stenting and thromboaspiration). This procedure was performed in 3 patients who had concurrent ICA dissection and intracerebral occlusion. Endovascular treatment was feasible and safe. All our patients had a good clinical outcome. This potential effective approach may need furt

A 7-year-old girl with Down syndrome and moderately severe pulmonary hypertension experienced a stroke while being treated with a calcium channel blocker. Angiography identified bilateral stenosis of the supraclinoid internal carotid arteries, stenosis or occlusion of the proximal anterior and middle cerebral arteries, and occlusion of the left posterior cerebral artery. She underwent surgery to enhance collateral blood flow to vulnerable areas of the brain. Her pulmonary hypertension therapy was changed to an oral endothelin receptor antagonist. She developed excellent collateral blood flow t

BACKGROUND: The best strategy to assess the changes in brain hemodynamics following superficial temporal artery (STA)-middle cerebral artery (MCA) bypass in patients with Moyamoya disease remains unknown. The purpose of the present study was to assess cerebral hemodynamics using perfusion-weighted magnetic resonance imaging (PWI) before and after STA-MCA bypass surgery in patients with Moyamoya disease.
METHODS: STA-MCA bypass surgeries were performed on 23 symptomatic cerebral hemispheres in 21 patients (11 females/10 males, age 11-62 years) with Moyamoya disease due to cerebral ischemic att

INTRODUCTION: Surgical revascularization represents an effective treatment for moyamoya disease (MMD); however, it is associated with postoperative complications such as stroke and hyperperfusion syndrome. Intraoperative polyuria and electrolyte disturbances resembling diabetes insipidus (DI) are frequently observed, yet their clinical significance remains unclear. This study aimed to identify potential risk factors for postoperative complications and to evaluate the predictive value of intraoperative DI in individuals undergoing revascularization for MMD.
METHODS: A retrospective analysis wa

Early postoperative seizure after revascularization for moyamoya vasculopathy remains incompletely characterized, particularly in Indonesia. This study described the incidence and exploratory clinical and hemodynamic patterns associated with early postoperative seizure after extracranial-intracranial bypass in an Indonesian single center cohort. This single center retrospective cohort included moyamoya disease (MMD) or moyamoya syndrome (MMS) patients who underwent extracranial-intracranial revascularization. Early postoperative seizure was defined as a clinically documented new onset epilepti

Underlying atherosclerotic stenosis is more common in acute basilar artery occlusion than in anterior circulation, and often necessitates additional procedures such as balloon angioplasty or stent placement during endovascular revascularization. Preoperative recognition of underlying basilar artery stenosis may be clinically relevant, but reliable imaging indicators remain unclear. This study aimed to identify preoperative magnetic resonance imaging findings associated with underlying basilar artery stenosis in patients undergoing acute endovascular revascularization. We retrospectively analyz

OBJECTIVE: The optimal management and procedural strategy for tandem occlusion (TO) in acute ischemic stroke are still unclear, as is the long-term outcome of these patients. The aim of this study was to evaluate predictors of good functional outcome in patients with TO through the analysis of demographics, clinical, and radiological data with a 1-year follow-up.
METHODS: We collected data on 100 patients with TO who underwent revascularization treatments in our comprehensive stroke center. We divided patients into 2 groups: those with good functional outcome, defined as a modified Rankin Sca

BACKGROUND AND OBJECTIVES: Advances in endovascular flow diverters have led to a secular shift in the management of brain aneurysms, causing debate on current bypass indications. We therefore sought to investigate the long-term results, current indications, and trends of bypasses for brain aneurysms.
METHODS: We retrospectively reviewed bypasses performed between 2005 and 2022 to treat brain aneurysms. Demographic, clinical, and radiological data were collected till the most recent follow-up. Aneurysm occlusion and graft patency was noted on cerebral angiogram in the immediate postoperative,

OBJECTIVE: The post-bypass stroke risk factors and long-term outcomes of moyamoya patients are not well documented. Therefore, the authors studied 30-day stroke risks and patients' long-term physical, functional, and social well-being.
METHODS: This was a single-institution combined moyamoya disease (MMD) database interrogation and questionnaire study. From 1991 to 2014, 1250 revascularization procedures (1118 direct bypasses, 132 indirect bypasses) were performed in 769 patients. Completed questionnaires were received from and available for analysis on 391 patients, and 6-month follow-up dat

BACKGROUND: Large basilar trunk aneurysm caused by bilateral occlusion of the proximal common carotid artery is rare. We treated one case with vertebral V3 portion-radial artery-distal common carotid artery (V3-RA-dCCA) bypass.
CASE DESCRIPTION: Basilar trunk aneurysm and bilateral occlusion of the proximal CCA were found incidentally in a 70-year-old woman. During the next 5 years, the aneurysm gradually enlarged to 12 mm, and blood flow of the anterior circulation was supplied through the posterior communicating artery. V3-RA-dCCA bypass was performed to reduce the stress of blood flow and

BACKGROUND AND IMPORTANCE: Unclippable vertebral artery aneurysms (UVAs) are difficult to treat with direct clipping, especially in cases involving the origin of the posterior inferior cerebellar artery (PICA). Bypass with trapping is the common procedure used for these conditions. The authors used the blind-alley formation technique, which is a simpler method than trapping and can avoid some complications.
OBJECTIVES: To report 7 cases of UVA with PICA involvement treated with blind-alley formation and occipital artery (OA)-PICA bypass as well as their surgical outcomes and complications and
