[2 cases of life threatening gastrointestinal hemorrhage from Meckel's diverticulum].

Meckel's diverticulum is one of the commonest congenital anomalies of the gastrointestinal tract. Two cases requiring emergency laparotomy due to massive gastrointestinal bleeding are presented. Only at laparotomy the correct diagnosis was established. In the first case a segment of small bowel was resected and diverticulectomy was performed in the second case. Both patients are free of complaints now at 2 and 3 years of follow-up respectively. The genesis of diverticular bleeding, the difficulties of the preoperative diagnosis of the complicated diverticulum and the uncertain indication for o

Endoscopic retrograde cholangiopancreatography in a general surgery practice.

Endoscopic retrograde cholangiopancreatography (ERCP) is performed for the diagnosis and therapy of benign and malignant biliary and pancreatic disease. There are few reports in the literature regarding the incorporation of this procedure into a general surgery practice. One hundred seven consecutive ERCPs performed by the same surgeon over a two-year period were reviewed. The most frequent indications for ERCP were jaundice, suspected common bile duct stones, and a history of pancreatitis. Successful cannulation of the ampulla of Vater was achieved in 97 per cent of the cases with the desired

Primary sepsis presenting as fulminant hepatic failure.

Four patients who were referred to the Liver Failure Unit with an initial diagnosis of fulminant hepatic failure were found to have severe bacterial infection from a primary septic focus as the cause of their illness. Clinical and biochemical characteristics were not helpful in differentiating these patients from those with hepatic failure from other causes, and only a high degree of suspicion will prevent delay in the diagnosis of underlying sepsis and initiation of appropriate treatment. The possible mechanisms responsible for this uncommon association are discussed.

[Colon-rectal anastomosis with mechanical sutures (personal experience in 96 cases)].

From 1 October 1981 to 30 September 1985 ninety-six patients underwent colorectal resection with EEA Auto-suture stapled anastomosis for cancer or diverticula. As compared with manual suture, the mechanical procedure involved some technical problems and peroperative drawbacks (partial leakage, hemorrhage), which decreased after the initial phase of our experience. The EEA Stapler proved a reliable and useful procedure, especially in low colo-rectal anastomoses, which present a higher risk of leakage, owing to difficulties with manual technique. Clinical or x-ray fistulas occurred in four patie

Factors influencing the complications of colostomy closure.

This retrospective review of 206 colostomy closures performed by different methods and at various intervals from the time of creation of the stoma suggests that attention to meticulous surgical technic is the most important factor in limiting the morbidity and mortality of this commonly performed procedure. Considerations of the location and type of colostomy, timing, method of closure, and patient population seem to be less importnat than suggested in previous reports.

Lemmel Syndrome: A Case Report.

Lemmel syndrome is a rare and misdiagnosed cause of obstructive jaundice due to a periampullary duodenal diverticulum (PAD) in the absence of choledocholithiasis or a tumor. PADs are extraluminal outpouchings of duodenal mucosa that develop within a radius of 2-3 cm from the ampulla of Vater. They are largely asymptomatic, but rarely they can cause acute abdominal pain and obstructive jaundice due to the extrinsic compression of the common bile duct or pancreatic duct. PADs are mostly asymptomatic, but complications can occur, such as hemorrhage, perforation, diverticulitis, pancreatitis, chol…

Left-sided omental torsion secondary to recurrent inguinal hernia in a 24-year-old male: A case report and review of literature.

Omental torsion is an infrequent but important cause of acute abdominal pain that can closely resemble appendicitis, diverticulitis, or other intra-abdominal inflammatory conditions. Recognizing it as a potential diagnosis is critical, particularly in patients with risk factors such as recurrent inguinal hernia. We report the case of a 24-year-old male with a history of left-sided direct inguinal hernia repair who presented with sudden-onset lower abdominal pain and vomiting. Imaging revealed herniated greater omentum with the classic "whirl sign" on contrast-enhanced CT. The patient underwent…

[A Case of Colonic Diverticulosis with Retroperitoneal Abscess Caused by Bevacizumab for Chemotherapy of Colon Cancer].

We report a case of retroperitoneal abscess due to penetration of colonic diverticulosis during chemotherapy for sigmoid colon cancer. The patient was a 56-year-old man who was outpatient for chronic rheumatoid arthritis and was diagnosed sigmoid colon cancer with multiple simultaneous liver and lung metastases. The antitumor drugs were effective and the primary tumor was resected, and chemotherapy was continued due to progression of liver metastasis. After the molecular targeted drug was changed to bevacizumab, the patient was hospitalized with a diagnosis of colonic diverticulitis and was di…

Colon stenting in benign diverticular stricture - a case report and review of literature.

Background: Colonic stricture is a feared complication with varied etiology ranging from malignant obstruction to benign diseases. One such condition is recurrent diverticulitis-related strictures. Objectives & Methods: We report a case of a 48-year-old male patient with stricture of the sigmoid colon. The patient refused Sigmoid colon resection. He underwent stent placement and Two weeks later presented with constipation and abdominal discomfort. On Computed Tomography (CT) we discovered stent migration and consequent large bowel obstruction. Patient agreed on undergoing sigmoidectomy with a …

Renal artery dissection as an overuse Injury.

The diagnosis of renal infarction is often convoluted due to its non-specific presentation. It can mimic disease processes as disparate as pyelonephritis, diverticulitis, or nephrolithiasis. This case is further complicated by the presence of a pelvic kidney with triplicate arterial input. It is difficult to estimate the incidence of pelvic kidneys as the numerous sources vary wildly in their estimations; however, the paucity information, in and of itself, speaks to the rarity of the condition. In this case, a 58-year-old male presents to the emergency department after experiencing sharp, sudd…

Evolution of incidence, mortality and cost of nontraumatic abdominal emergencies treated in Brasil in a period of nine years.

OBJECTIVE: To evaluate the incidence, mortality and cost of non-traumatic abdominal emergencies treated in Brazilian emergency departments. METHODS: This paper used DataSus information from 2008 to 2016 (http://www.tabnet.datasus.gov.br). The number of hospitalizations, costs - AIH length of stay and mortality rates were described in acute appendicitis, acute cholecystitis, acute pancreatitis, acute diverticulitis, gastric and duodenal ulcer, and inflammatory intestinal disease. RESULTS: The disease that had the highest growth in hospitalization was diverticular bowel disease with an increas…

Intestinal Ultrasound in Rare Gastrointestinal Diseases, Update, Part 2.

Intestinal ultrasound has become an established and valid diagnostic method for inflammatory bowel disease, diverticulitis, appendicitis, bowel obstruction, perforation and intussusception. However, little is known about sonographic findings in other rarer intestinal diseases. Ultrasound may display the transformation of the intestinal wall from a normal to a pathological state both in inflammatory and neoplastic disease. Besides demonstrating the transmural aspect, it also shows the mesenteric reaction as well as complications such as fistula, abscesses, stenosis, or ileus. Furthermore, in so…
