External Fixation for Ballistic Mandibular Trauma: An Old Method for a Modern Problem.

BACKGROUND: Ballistic injuries to the mandible produce comminuted fractures with compromised vascularity and contaminated wound beds that challenge the assumptions of rigid internal fixation. External fixation, the earliest form of mandibular stabilization, preserves periosteal blood supply by avoiding open dissection, yet direct comparisons between the 2 approaches in the ballistic setting are scarce. METHODS: A retrospective review was conducted of consecutive patients with gunshot-related mandibular fractures treated at a single level 1 trauma center from January 2020 to August 2024. Patie

Does a staged treatment of high energy tibial plateau fractures affect functional results and bony union? A case series.

PURPOSE: Tibial plateau fracture (TPF) is a devastating injury as it shatters lower articular surface of the largest joint. Apart from bony injury, TPF can lead to great soft tissue envelope compromise which affects the treatment plan and outcome. In the present study, clinical results were assessed in cases of high energy TPFs treated in staged manner. METHODS: Twenty-three (20 males and 3 females) patients of high energy communited TPFs (Schatzker type V and VI) were consecutively treated.1 All the patient had compromise of overlying skin conditions. They were all successively scheduled for

Evaluating the Utility of the Lateral Elbow Radiograph in Central Articular Olecranon Reduction: An Anatomic and Radiographic Study.

OBJECTIVES: The surgical reduction of intra-articular olecranon fractures is judged primarily on the lateral elbow radiograph, as orthogonal imaging of the articular surface is not obtainable. We sought to determine surgeon accuracy in identifying intra-articular olecranon malreductions on the lateral elbow radiograph. METHODS: Six human fresh-frozen cadaveric elbow specimens were sagittally sectioned in 5-mm increments after olecranon dissection, preservation of soft tissue envelope, and rigid fixation of the elbow in an external fixator. Three patterns of central intra-articular olecranon m

Highly unstable complex C3-type distal femur fracture: can double plating via a modified Olerud extensile approach be a standby solution?

BACKGROUND: Multiplanar complex C3-type unstable distal femoral fractures present many challenges in terms of approach and fixation. This prospective study investigates a possible solution to these problems through double plating with autogenous bone grafting via a modified Olerud extensile approach. MATERIALS AND METHODS: Twelve patients with closed C3-type injuries were included; eight of them were male, and their mean age was 33.5 years (range 22-44 years). Mechanism of injury was road traffic accident (RTA) in nine patients and fall from height in the other three cases. Eight cases were o

Intrathoracic humeral head fracture-dislocation.

There are few cases in the medical literature documenting intrathoracic glenohumeral fracture-dislocations. A total of eight cases have been reported-two intrathoracic humerus dislocations with greater tuberosity fractures 1, 2 and six cases with intrathoracic fracture-dislocations in which the fracture involved the humeral neck. 3-8 With so few cases, treatment modalities vary, and no guidelines exist. Patients in five of the reported cases underwent surgery, 1, 3-6 while the other three were treated nonoperatively. 2, 7, 8 We present a case of a middle-aged woman involved in a high-speed mot

A Case of Mangled Foot With Midfoot Bone Defect.

INTRODUCTION: The decision between amputation and reconstruction for mangled feet is often challenging. The foot must be functional for reconstruction to be considered successful. In this report, we present a case of a mangled foot with a metatarsal bone defect. CASE DESCRIPTION: A man in his 50s presented to the hospital after a heavy load fell on his left foot at work. He was diagnosed with an open dislocation fracture of the Lisfranc joint (Gustilo IIIC), a comminuted fracture of the navicular bone, and a defect of the intermediate cuneiform bone. TREATMENT COURSE: A foot ring-type extern

Post-Traumatic Middle Cerebral Artery Pseudoaneurysm Following Depressed Skull Fracture: Case Report and PRISMA-Guided Literature Review.

Traumatic intracranial aneurysms (TICAs) are rare lesions accounting for <1% of intracranial aneurysms and are associated with high morbidity and mortality. They frequently arise after blunt trauma and may develop days to months following the initial injury, often evading early vascular imaging. Pseudoaneurysms predominate and carry a high risk of rupture. We report the case of a patient who sustained a severe head injury with a comminuted open depressed temporoparietal skull fracture, epidural hematoma, subarachnoid hemorrhage, and mandibular fracture after a road traffic accident. Initial va

Patella Fracture Fixation With Novel Wagon Wheel Construct Versus Tension-Band Construct: A Technical Trick.

Internal fixation of patella fractures remains technically challenging. Cannulated screws with an anterior tension band have been associated with high rates of implant prominence, and fracture comminution can make appropriate application of a tension band impractical. We present the results of a novel technique using a transtendinous/transligamentous mini-fragment plate positioned peripherally around the patella with radially directed screws: termed the wagon-wheel (WW) construct. Compared with a cohort of fractures treated with cannulated screws with an anterior tension band, there was no dif

Biomechanical evaluation of a tool-less external fixator.

INTRODUCTION: Current external fixator systems used by the US and UK military for stabilising extremity fractures require specialised tools to build a construct. The goal of obtaining and maintaining limb length and alignment is not achieved if these tools are misplaced. An alternative, tool-less system is currently available, namely the Dolphix Temporary Fixation System. The aim of this study was to compare the stiffness of the Dolphix system with the existing Hoffmann III system. METHODS: Three Hoffmann III and three Dolphix constructs were assembled on a bone (tibia) surrogate. A 30 mm fra

A device mimicking the biomechanical characteristics of crocodile skull for lumbar fracture reduction.

Open surgery is currently the main treatment method for the lumbar burst fracture with neurological deficit but may irreversibly disrupt the lumbar anatomy. The minimally invasive surgery (MIS) techniques have recently gained increasing attention. However, their use is still limited to lumbar burst fractures mainly due to their difficulties in burst fracture reduction and decompression. Here we present a novel bio-inspired MIS device which can be used with an endoscope to reset the bone fragments retropulsed into the spinal canal within the wounded vertebral body. Its head jaw mimics the biome

Bilateral atraumatic tibial tubercle avulsion fractures: case report and review of the literature.

An avulsion fracture of the tibial tubercle is an uncommon injury, comprising less than 1% of all physeal injuries. The occurrence of such injuries bilaterally is even rarer. We report a case of bilateral atraumatic tibial tubercle avulsion fractures and its presentation, mechanism of injury, surgical management, post-operative rehabilitation and implications for clinical practice. A 17-year-old healthy male presented to the emergency department with severe pain on the anterior aspect of both knees and was unable to walk, having been brought in by ambulance after hearing a crack whilst jogging

Preoperative computer-assisted design templating of complex articular olecranon osteotomy: case report.

Preoperative 3-dimensional modeling methods are commonly used in orthopedic surgery but there have been limited reports of their use for planning upper extremity surgery. In this report, we describe a case of a malunited olecranon fracture and its management using custom preoperative computed tomographic templating and mechanical modeling. One year after surgery, the patient demonstrated nearly full painless restoration of function and motion.

[Scapho-capitate fracture. Apropos of a case, review of the literature].

Scapho-capitate fracture is a rare lesion. Its mechanism is controversial. Forced extension seems to be the most frequent etiology, as this movement of the wrist induces scaphoid fracture by compression. Hyperextension of the wrist, as a result of the scaphoid lesion, allows contact between the posterior margin of the radius and the neck of the capitatum, inducing capitatum fracture. Continuation of the movement allows a two-stroke rotation of the capitate proximal pole. Analysis of a personal case and review of the literature demonstrated: the frequency of extension movement for etiology, del

Metatarsal Transfer Associated with Microsurgical Flap in Exposed Foot Fracture: A Case Report and 10-Year Follow-up.

CASE: A 35-year-old female patient presented with an exposed forefoot fracture with bone and dermal loss after a vehicular accident. There was a bone defect at the level of the second toe and the third metatarsal. She was treated by second toe resection and second metatarsal transfer to augment the third metatarsal diaphysis ray (nonvascularized, without any soft-tissue envelope) and a microsurgical forearm flap. CONCLUSION: As one option among the reconstruction techniques available for a metatarsal segmental defect, metatarsal transfer associated with a fasciocutaneous radial forearm flap p

Suicide by blunt head trauma - Two cases with striking similarities.

There have been several forensic pathological studies on the distinction between falls from height and homicidal blows in blunt head trauma, but few studies have focused on suicidal blows. Self-inflicted blunt head trauma is usually a part of a complex suicide with more than one suicidal method applied. Actually, no reports on suicide indicate blunt head trauma to be the singular cause of death in recent publications. Cases with self-inflicted blunt trauma are often challenging for those involved in the investigation because they are confronted with findings that are also found in homicides. A

Stabilisation of the fractured fibula plays an important role in the treatment of pilon fractures: a retrospective comparison of fibular fixation methods.

Ninety-eight pilon fractures associated with ipsilateral distal fibular fracture were included in this study. The pilon fractures were treated by open reduction and plating. The 98 fractures were divided into three groups based on the treatment method of fractured fibula. Group A was composed of 50 fibular fractures treated by open reduction and plate fixation. Group B was composed of 23 fibular fractures treated by open reduction and pin fixation. Group C was composed of 25 fibular fractures treated conservatively by closed reduction. The radiographs were reviewed for adequacy of fracture red
