Influenza A (H1N1) in Hospitalized Children.

BACKGROUND: Influenza A (H1N1) is a contagious respiratory infection caused by the influenza A virus. In the majority of cases, H1N1 influenza is benign. However, it can be dangerous for infants and children with underlying chronic diseases. The severity of influenza depends on various factors, including the virulence of the virus strain, preexisting immunity level, and individual health conditions. The aim of this study is to describe the clinical profile of H1N1 influenza in hospitalized infants and children. METHODS: This is a prospective and descriptive study conducted from November 1, 20

[Acute phenol poisoning].

INTRODUCTION: Phenol (carbolic acid) is one of the oldest antiseptic agents. Currently it is used as a disinfectant, chemical intermediate and nail cauterizer. Phenol is a general protoplasmic poison (denatured protein) with corrosive local effects. Phenol derivates are less toxic than pure phenol. The lethal dose is between 3 to 30 g, but may be as little as 1 g. Phenol is well absorbed by inhalation, dermal application, and ingestion. MANIFESTATIONS OF ACUTE POISONING: Local manifestations. Dermal exposure produces lesions which are initially painless white patches and later turn erythematou

WITHDRAWN: Interventions for dysphagia in oesophageal cancer.

BACKGROUND: The majority of oesophageal and gastro-oesophageal cancers are diagnosed at an advanced stage and palliative treatment is the realistic management option for most patients. The optimal intervention for the palliation of dysphagia in these patients has not been established. OBJECTIVES: To systematically analyse and summarise the efficacy of different interventions used in the palliation of dysphagia in primary oesophageal carcinoma. SEARCH STRATEGY: We undertook a search according to the Cochrane Upper Gastrointestinal and Pancreatic Diseases model using the Cochrane Central Regis

Vasoconstrictor use in liver transplantation: is there evidence for rational use?

Liver transplantation is a challenging surgical operation performed in recipients with major hemodynamic perturbations related to portal hypertension. The pathophysiologic alterations in portal hypertension include a hyperdynamic circulation and decline in systemic vascular resistance and mean arterial pressure. Cardiac function can also be depressed due to cirrhosis related cardiomyopathy. These cirrhosis related changes often lead to a tenuous state in which organ perfusion is threatened and declines rapidly in the setting of many other insults including blood loss, infection, and use of med

Removal of foreign bodies in the upper gastrointestinal tract in adults: European Society of Gastrointestinal Endoscopy (ESGE) Clinical Guideline.

This Guideline is an official statement of the European Society of Gastrointestinal Endoscopy (ESGE). It addresses the removal of foreign bodies in the upper gastrointestinal tract in adults. Recommendations Nonendoscopic measures 1 ESGE recommends diagnostic evaluation based on the patient's history and symptoms. ESGE recommends a physical examination focused on the patient's general condition and to assess signs of any complications (strong recommendation, low quality evidence). 2 ESGE does not recommend radiological evaluation for patients with nonbony food bolus impaction without complicat

The clinical toxicology of γ-hydroxybutyrate, γ-butyrolactone and 1,4-butanediol.

INTRODUCTION: Gamma-hydroxybutyrate (GHB) and its precursors, gamma-butyrolactone (GBL) and 1,4-butanediol (1,4-BD), are drugs of abuse which act primarily as central nervous system (CNS) depressants. In recent years, the rising recreational use of these drugs has led to an increasing burden upon health care providers. Understanding their toxicity is therefore essential for the successful management of intoxicated patients. We review the epidemiology, mechanisms of toxicity, toxicokinetics, clinical features, diagnosis, and management of poisoning due to GHB and its analogs and discuss the fea

A severe and fatal course of statin-induced necrotizing autoimmune myopathy: a case report.

INTRODUCTION: Statin-induced necrotizing autoimmune myopathy is a rare but serious complication of statin therapy, marked by progressive muscle weakness and elevated creatine phosphokinase levels that persist despite discontinuation of the drug. We present a case in which statin-associated myopathy was initially misattributed to hepatic dysfunction due to elevated liver enzymes. Despite early recognition and immunosuppressive therapy, the patient experienced relentless decline, ultimately leading to death. This case highlights the progression and potential severity of SINAM, emphasizing the ne

Successful Placement of Nasointestinal Feeding Tubes Using an Electromagnetic Sensor-Guided Enteral Access System in Patients With Left Ventricular Assist Devices.

Technology advances have made it possible to prolong life for patients with heart failure who are not transplant candidates or while awaiting transplant. Many different cardiac devices are available that can be used as a bridge to transplant (temporary support) or as a destination therapy (permanent support). Placement of these devices can cause complications that, if not addressed, could negatively impact the nutrition status of patients. Placement of nasointestinal feeding tubes using an electromagnetic sensor-guided enteral access system (EMS-EAS) has been difficult in patients with implant

Factors affecting duration of gastrostomy tube retention in survivors following treatment for head and neck cancer.

BACKGROUND: Many patients treated for head and neck cancer require nutritional support, which is often delivered using a gastrostomy tube. It is difficult to predict which patients will retain their gastrostomy tube in the long term. This study aimed to identify the factors which affect the duration of gastrostomy tube retention. METHOD: In this retrospective study, 151 consecutive patients from one centre were audited. All patients had a mucosal tumour of the head and neck, and underwent gastrostomy tube insertion between 2003 and 2007. RESULTS: There were near-complete data sets for 132 pa

Early feeding in pediatric patients following stoma closure in a resource limited environment.

BACKGROUND: Establishment of enteral nutrition is necessary after intestinal surgery. In resource-strained environments, it can be critical. This study examined the effect of early feeding in pediatric patients undergoing stoma closure in a country with mid-level socioeconomic indices. METHODS: With parenteral consent and ethics board approval, patients were prospectively enrolled in early feeding (Group 1), starting feeds 24h post-operation with a protocol driven increase. They were compared with similar patients managed without a specific protocol over the 12 months prior (Group 2). RESULT

Dependence induced increases in intragastric alcohol consumption in mice.

Three experiments used the intragastric alcohol consumption (IGAC) procedure to examine the effects of variations in passive ethanol exposure on withdrawal and voluntary ethanol intake in two inbred mouse strains, C57BL/6J (B6) and DBA/2J (D2). Experimental treatments were selected to induce quantitative differences in ethanol dependence and withdrawal severity by: (1) varying the periodicity of passive ethanol exposure (three, six or nine infusions/day); (2) varying the dose per infusion (low, medium or high); and (3) varying the duration of passive exposure (3, 5 or 10 days). All experiments

Toxicology and carcinogenesis studies of androstenedione (CAS No. 63-05-8) in F344/N rats and B6C3F1 mice (gavage studies).

UNLABELLED: Androstenedione is an androgen steroid that is normally synthesized within men and women and may be metabolized to a more potent androgen or estrogen hormone. It was nominated to the National Toxicology Program for study due to concern for adverse health effects associated with its chronic use as a dietary supplement by athletes (prior to the banning of its over the counter sales). In order to evaluate its subchronic and chronic toxicity, male and female F344/N rats and B6C3F1 mice were administered androstenedione (98% pure) by gavage for 2 weeks, 3 months, or 2 years. CO2 laser supraglottoplasty for severe laryngomalacia: a study of symptomatic improvement.

OBJECTIVE: To investigate post-operative symptom improvement in patients with severe laryngomalacia. STUDY DESIGN: Severe laryngomalacia was diagnosed in 138 patients (average age of 6.97 months) by bronchoscopy. Laryngomalacia was defined by the direction of supraglottic collapse: type A (posterolateral), type B (complete), and type C (anterior). As multiple laryngomalacia types within an individual were common, patients were further categorized into group I (type A only), group II (type B or B+A), and group III (type C, C+A, or C+B+A). CO(2) laser supraglottoplasty was performed. Improvemen

Systemic lanthanum is excreted in the bile of rats.

Lanthanum carbonate is a non-calcium-based oral phosphate binder for the control of hyperphosphataemia in patients with chronic kidney disease Stage 5. As part of its pre-clinical safety evaluation, studies were conducted in rats to determine the extent of absorption and routes of excretion. Following oral gavage of a single 1500 mg/kg dose, the peak plasma lanthanum concentration was 1.04+/-0.31 ng/mL, 8 h post-dose. Lanthanum was almost completely bound to plasma proteins (>99.7%). Within 24h of administration of a single oral dose, 97.8+/-2.84% of the lanthanum was recovered in the faeces o

Role of lipopolysaccharide-binding protein in early alcohol-induced liver injury in mice.

Cellular responses to endotoxins are enhanced markedly by LPS-binding protein (LBP). Furthermore, it has been demonstrated that endotoxins and proinflammatory cytokines such as TNF-alpha participate in early alcohol-induced liver injury. Therefore, in this study, a long-term intragastric ethanol feeding model was used to test the hypothesis that LBP is involved in alcoholic hepatitis by comparing LBP knockout and wild-type mice. Two-month-old female mice were fed a high-fat liquid diet with either ethanol or isocaloric maltose-dextrin as control continuously for 4 wk. There was no difference i
