Split Hand Syndrome in Charcot-Marie-Tooth Disease Type X1 (CMTX1): A Clinical, Neurophysiological, and Radiological Study.

BACKGROUND: Split hand syndrome (SHS) is a hand atrophy pattern characterized by predominant wasting in the thenar muscles (abductor pollicis brevis-APB, first dorsal interosseous-FDI), with relative sparing of the hypothenar (abductor digiti minimi-ADM). SHS was also reported in CMTX1, but eventually attributed to median-ulnar dissociated involvement. We investigated the presence and specificity of SHS in CMTX1. METHODS: We gathered clinical/neurophysiological/radiological information in CMTX1 and non-CMTX1 patients, including disease severity (CMT Examination Score-CMTES) and compound muscl

Magnetic Resonance Neurography Findings in Clinically Suspected Posterior Interosseous Neuropathy.

BACKGROUND: Lateral elbow pain affects 1% to 3% of the general population. Neuropathy involving the posterior interosseous nerve (PIN) is a potential cause of lateral elbow pain but shares overlapping symptoms with other pathologies, particularly lateral epicondylitis, which may be difficult to distinguish based solely on patient history, physical examination, or electrodiagnostic testing (EDX). Conventional magnetic resonance imaging may reveal denervation edema or atrophy in PIN-innervated muscles, but abnormal signal intensity within the nerve may not be apparent. HYPOTHESIS: By incorporat

Nerve fascicle transfer using a part of the C-7 nerve for spinal accessory nerve injury.

OBJECTIVE Spinal accessory nerve (SAN) injury results in a series of shoulder dysfunctions and continuous pain. However, current treatments are limited by the lack of donor nerves as well as by undesirable nerve regeneration. Here, the authors report a modified nerve transfer technique in which they employ a nerve fascicle from the posterior division (PD) of the ipsilateral C-7 nerve to repair SAN injury. The technique, first performed in cadavers, was then undertaken in 2 patients. METHODS Six fresh cadavers (12 sides of the SAN and ipsilateral C-7) were studied to observe the anatomical rela

Interlimb reflex activity after spinal cord injury in man: strengthening response patterns are consistent with ongoing synaptic plasticity.

OBJECTIVE: Previous reports from our laboratory have described short-latency contractions in muscles of the distal upper limb following stimulation of lower limb nerves or skin in persons with injury to the cervical spinal cord. It takes 6 or more months for interlimb reflexes (ILR) to appear following acute spinal cord injury (SCI), suggesting they might be due to new synaptic interconnections between lower limb sensory afferents and motoneurons in the cervical enlargement. In this study, we asked if once formed, the strength of these synaptic connections increased over time, a finding that w

Use of a recombinant pseudorabies virus to analyze motor cortical reorganization after unilateral facial denervation.

A unilateral facial nerve injury (n7x) was found to influence the transcallosal spread of the attenuated strain of pseudorabies virus (PRV Bartha) from the affected (left) primary motor cortex (MI) to the contralateral MI of rats. We used Ba-DupLac, a recombinant PRV strain, for the tracing experiments since this virus was demonstrated to exhibit much more restricted transportation kinetics than that of PRV Bartha, and is therefore more suitable for studies of neuronal plasticity. Ba-Duplac injection primarily infected several neurons around the penetration channel, but hardly any transcallosa

[Extra-articular pathogenetic factors in arthritis and deformities in old age as exemplified by varus gonarthrosis].

Extraarticular, predominantly neuromuscular, factors are thought to contribute to the development of osteoarthritis. With increasing age, changed temporal dynamics of motor reflexes and reduced muscular force may affect the efficacy of neuromuscular protection of the knee joint. In three experimental studies conducted with osteoarthritis patients and healthy controls, no difference was found in monosynaptic reflex times and latencies of somatosensory evoked brain potentials. Muscular force was generally diminished in the patients, with the most prominent reduction during coxal abduction. Osteo

Modified partial radial to axillary nerve transfer as the index procedure for isolated axillary nerve injury.

BACKGROUND: Axillary nerve injuries often produce profound functional deficits. Beyond expectant management or microsurgical repair, treatment is generally limited to salvage procedures. We present the results of partial radial-to-axillary nerve transfer as the index procedure for treatment of axillary nerve injury after failure of initial conservative management. METHODS: Minimum 2-year follow-up data were collected and retrospectively analyzed for this single-surgeon cohort of patients with isolated axillary nerve injury. The injury was confirmed with electromyography, and all patients unde

Synaptic dynamics linked to widespread elevation of H-reflex before peripheral denervation in amyotrophic lateral sclerosis.

Changes in Hoffmann reflex (H-reflex) exhibit heterogeneity among patients with amyotrophic lateral sclerosis (ALS), likely due to phenotype diversity. Current knowledge primarily focuses on soleus H-reflex, which may demonstrate an initial increase before subsequent decline throughout the disease course. The main objective was to investigate other muscles, to determine whether H-reflex changes could be associated with patient phenotype (onset site, functional disabilities). Additional experiments were performed to elucidate the neurophysiological mechanisms underlying H-reflex modifications.

Ciliary neurotrophic factor promotes motor reinnervation of the musculocutaneous nerve in an experimental model of end-to-side neurorrhaphy.

BACKGROUND: It is difficult to repair nerve if proximal stump is unavailable or autogenous nerve grafts are insufficient for reconstructing extensive nerve damage. Therefore, alternative methods have been developed, including lateral anastomosis based on axons' ability to send out collateral sprouts into denervated nerve. The different capacity of a sensory or motor axon to send a sprout is controversial and may be controlled by cytokines and/or neurotrophic factors like ciliary neurotrophic factor (CNTF). The aim of the present study was to quantitatively assess collateral sprouts sent out by

Clinicopathological and virological analyses of familial human T-lymphotropic virus type I--associated polyneuropathy.

Human T-lymphotropic virus type I (HTLV-I) is known to be the causative agent of the chronic myelopathy, HTLV-I--associated myelopathy (HAM), and on rare occasions infection is also associated with the development of polyneuropathy. Here the authors present an HTLV-I--positive family of whom four members developed a chronic demyelinating polyneuropathy without HAM. Four female patients in a family from Hokkaido in Japan developed distal dominant paresthesia and muscle weakness in the second and third decades of their life. Neurological findings at ages ranging from 50 to 65 years included mild

Assessment of the psoas muscle changes following the oblique lateral interbody fusion (OLIF) approach: A prospective observational study.

BACKGROUND: Traditional open posterior fusion techniques involve stripping of the posterior paraspinal muscles with protracted retraction, which can lead to ischemia and denervation. This may result in poor clinical outcomes despite a well-performed surgery. Oblique lumbar interbody fusion (OLIF) is a minimally invasive modified retroperitoneal anterior approach in the corridor between the psoas major (PM) and great vessels. The purpose of this research was to study changes in psoas properties and clinical outcomes in terms of the loss of hip flexion in patients undergoing OLIF surgery. METHO

HIV Neuropathy-Associated Foot Drop, a Presenting Sign of HIV Infection, Resolving After Initiation of Antiretroviral Therapy: A Clinical Vignette.

Peripheral neuropathy is one of the most frequent complaints in patients with HIV. Many complex syndromes exist, with the etiology being secondary to the disease process itself, antiretroviral medication, or immune reconstitution. However, isolated mononeuropathy is rare. In this case, we present a previously healthy man who complained of several months of worsening right foot drop that did not improve with physical therapy or lifestyle interventions. He had begun to use an solid ankle-foot orthotic on this right lower limb to minimize tripping and prevent falls. He had no other neuromuscular

Hirayama disease in a young Indonesian male: a case report.

BACKGROUND: Hirayama disease (HD) is a rare, self-limiting lower motor neuron disorder predominantly affecting young males in Asia. It is caused by dynamic compression of the lower cervical spinal cord during neck flexion, resulting in ischemic injury to the anterior horn cells. CASE DESCRIPTION: A 15-year-old Indonesian male presented with a 6-month history of progressive right upper limb weakness and muscle wasting without sensory deficits or spasticity. Electromyography (EMG) showed motor neurogenic changes with ongoing denervation and fasciculations in the right upper limb, with possible

A novel mouse model simulating C5 palsy after cervical surgery.

BACKGROUND: The incidence of C5 palsy after cervical surgery (C5P) was high (>5%), although its pathogenesis is still not clear. Although most patients (>95%) could have apparent enhancement, there are still some patients who cannot achieve satisfactory functional recovery, and it seriously affects their quality of life. At present, there is no appropriate animal model that could be used to research new interventions for permanent C5P. METHODS: Following anatomical validation confirming precise exposure of the C5 ventral root, we established a novel mouse C5P model using a posterior intradura

A Slower-Progressing TDP-43 rNLS8 Mouse Model for ALS: Implications for Preclinical and Mechanistic Studies.

Amyotrophic lateral sclerosis (ALS) is a fatal neurodegenerative disease characterised by motor neuron degeneration, muscle weakness, paralysis, and eventual death, with TAR DNA-binding protein 43 (TDP-43) pathology observed in almost all cases. Mouse models based on TDP-43 are thus essential for studying ALS and developing therapeutic approaches. The TDP-43 rNLS8 mouse model expresses a human TDP-43 transgene with a mutated nuclear localization sequence (hTDP-43 ΔNLS), but this is normally suppressed by the presence of doxycycline (Dox). Disease is initiated by removal of Dox, which replicate

Unilateral Modified Posterior Lumbar Interbody Fusion Combined With Contralateral Lamina Fenestration Treating Severe Lumbarspinal Stenosis: A Retrospective Clinical Study.

Study design: Retrospective study. Objectives: The traditional PLIF is routinely utilized in severe lumbar spinal stenosis to relief the nerve compression. Nevertheless, the removal of posterior tension-band structure and the denervation and atrophy of the paraspinal muscle affect the clinical efficacy. Therefore, unilateral modified PLIF combined with contralateral fenestration was performed to overcome above-mentioned drawbacks. Methods: 32 modified PLIF and 33 traditional PLIF cases were retrospectively included. Operation time, length of stay (LOS) and blood loss were recorded. VAS of low

Revisiting the gamma loop in ALS.

Amyotrophic Lateral Sclerosis (ALS) is a fast progressive neurodegenerative disease characterized by muscle denervation, weakening and atrophy, which eventually culminates into death, mainly due to respiratory failure. The traditional view of ALS as a disorder affecting selectively motor neurons throughout the central nervous system has been progressively dispelled by innumerous lines of evidence indicating that other cells but motor neurons may be affected as well. Remarkably, this disorder is not limited to the motor system but rather configures as a systemic disease yielding a plethora of c…

Endovascular Renal Denervation in End-Stage Kidney Disease Patients: Cardiovascular Protection-A Proof-of-Concept Study.

INTRODUCTION: Sympathetic neural activation is markedly increased in end-stage kidney disease (ESKD). Catheter-based renal denervation (RDN) reduces sympathetic overactivity and blood pressure in resistant hypertension. We investigated the effect of RDN on sympathetic neural activation and left ventricular mass in patients with ESKD. METHODS: Nine ESKD (6 hemodialysis and 3 peritoneal dialysis) patients with dialysis vintage of ≥11 months were treated with RDN (EnligHTN system). Data were obtained on a nondialysis day; at baseline, 1, 3, and 12 months post-RDN. RESULTS: At baseline sympathet
