[Gallbladder duplication--laparoscopic cholecystectomy 17 years after open cholecystectomy].

INTRODUCTION: Duplication of the gallbladder is a rare congenital anomaly. An incidence of 2.5:10,000 has been published in autoptic studies. METHODS: We carried out an analysis of the published case reports of the last 30 years to examine those cases in which an accessory gallbladder was pre- or intraoperatively detected or missed and to evaluate the implications of the time of diagnosis of the duplication on the surgical therapy. RESULTS: 28 case reports were analysed. If the duplication of the gallbladder was recognised pre-operatively or during operation, both gallbladders could be remov

The use of endoclips in the treatment of nonvariceal gastrointestinal bleeding.

Acute nonvariceal gastrointestinal bleeding is the most common emergency managed by endoscopists and the endoscopic therapy has generally been recommended as the first-line treatment. Traditionally, endoscopic treatment included injections of epinephrine and sclerosing solutions or the use of thermocoagulation. In the last decade with the introduction of hemoclips and band ligators, we have witnessed a significant improvement in the clinical outcome of nonvariceal gastrointestinal bleeding. Endoclipping is a safe and effective technique that contributes to hemostasis of bleeding lesions of the

Management of common bile duct stones: a ten-year experience at a tertiary care center.

BACKGROUND: The authors present their experience with the treatment of cholecystocholedocholithiasis. METHODS: From August 1995 to August 2005, we treated 3750 patients with symptomatic gallbladder stones, and 12.7% of these patients also had a choledocholithiasis. This percentage with reference to the literature is certainly due to intraoperative cholangiography (IOC) performed routinely. During the follow-up, the percentage of residual versus recurrent stones was evaluated. RESULTS: Only 2 patients (3.1%) of the laparoscopic choledochotomy and cholecystectomy group had retained stones disc

Duodenal perforations after endoscopic retrograde cholangiopancreatography: experience and management.

OBJECTIVE: The aim of this study was to summary the experiences and lessons from periduodenal perforations related to endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic sphincterotomy (EST). METHODS: A retrospective review from 2004 to 2007 identified 9 patients (0.37%) of periduodenal perforation related to ERCP/EST. Charts were reviewed for the following parameters: clinical presentation of patients, ERCP findings, diagnostic methods, treatment (surgical or conservative procedures), complications, and outcome. RESULTS: Nine patients who had periampullary perforations rece…

Endoscopic ultrasound versus endoscopic retrograde cholangiography for patients with intermediate probability of bile duct stones: a randomized trial comparing two management strategies.

BACKGROUND AND STUDY AIMS: Diagnostic endoscopic retrograde cholangiography (ERC) is being replaced by endoscopic ultrasonography (EUS) in patients with suspected bile duct stones. The assumption that such an approach is advantageous, however, has never been tested in a randomized trial. PATIENTS AND METHODS: 100 patients with intermediate probability of bile duct stones were randomly allocated to EUS or ERC. Two patients in the ERC group were excluded; the remaining 98 patients received the allocated intervention and were entered into the analysis (EUS, 50 patients; ERC, 48 patients). Detect

Sigmoid colonic perforation and pelvic abscess complicating biliary stent migration.

Endoscopically placed biliary stents are a well-established procedure for the treatment of benign and malignant causes of obstructive jaundice in patients unfit for definitive surgical intervention. Stent migration has been described, though in most instances the stent will pass or remain in the bowel lumen for extended periods of time. Only a few cases of clinically significant complications of stent migration have been reported. This is the first case report of a pelvic abscess complicating stenting for choledocholithiasis. As the numbers of stenting procedures continue to increase it may be

[Endoscopic papillotomy and extraction of bile duct calculi in patients with gallbladder in situ and cholelithiasis].

Cholecystectomy with surgical papillotomy is a prolonged procedure with significant morbidity and mortality. In patient stay is usually extended. Occasionally the bad general conditions and the advanced age of the patients contraindicate surgical procedures. Eventually, the drainage of the biliary ducts is inevitable. For this reason, since 1983, we are performing the endoscopic papillotomy and extraction of stones from the bile duct in patients with choledocholithiasis and cholelithiasis. Cholecystectomy was performed only in cases with real necessity. Ninety one patients, 26 men and 65 women

[Hemosuccus pancreaticus due to a pressure ulcer in pancreatolithiasis].

Ultrasound examination in a 32-year-old man with epigastric pain and slightly elevated serum amylase level (66 U/l) revealed cysts in the head and tail of the pancreas as well as nonhomogeneous pancreas parenchyma as signs of chronic pancreatitis. Endoscopic retrograde cholangiopancreatography (ERCP) additionally showed pancreatolithiasis. Melena occurred 7 months later, haemoglobin concentration decreasing to 8.7 g/dl. Endoscopy revealed bleeding into the duodenum via the papilla major. Angiography excluded vessel rupture. Haemostasis was achieved by occlusion of the pancreatic duct with fibr

Endoscopic papillotomy while the gallbladder is in situ.

The authors report on their experience with 25 patients who underwent an endoscopic papillotomy with gallbladder in situ and review the current literature on this topic. Twenty-four out of 25 patients in this study had their common bile duct cleared of stones. Five patients (24%) required cholecystectomy on follow-up at an average of 14.4 months. There was no mortality associated with the procedure. The authors' experience and review of the literature suggest that endoscopic papillotomy plays an important role in the removal of common bile duct stones in persons who have their gallbladder in s

[Transduodenal papillosphincteroplasty in the treatment of benign diseases of the terminal choledochus and papilla Vateri].

The modern state of the problem and the indications and contraindications for performing transduodenal papillosphincteroplasty are discussed. Personal experience is recorded with 67 transduodenal papillosphincteroplasties, performed for the period 1983-1989, against the background of 1091 operations on the hepatobiliary system, 171 of them on the extrahepatic bile ducts, performed during the same period. Proceeding from their own experience and from available data in the literature, the authors analyse and evaluate the surgical and nonsurgical methods of treatment of benign diseases of the ter

Acquired immunodeficiency syndrome cholangiopathy: spectrum of disease.

PURPOSE: Reports of biliary tract abnormalities in patients with acquired immunodeficiency syndrome (AIDS) are infrequent. Nine patients with features of papillary stenosis and sclerosing cholangitis and one patient with primary bile duct lymphoma were previously described. Since those reports, an additional 16 AIDS patients with clinical and laboratory features suggestive of biliary tract disease have been evaluated. The findings in these 26 patients form the basis of this study. PATIENTS AND METHODS: The patients, 25 men and one woman, with right upper quadrant abdominal pain, fevers, or el

Use of Botulinum Toxin Injections for the Treatment of Chronic Anal Fissure: Results From an American Society of Colon and Rectal Surgeons Survey.

BACKGROUND: Chronic anal fissure (CAF) is commonly treated by colorectal surgeons. Pharmacological treatment is considered first-line therapy. An alternative treatment modality is chemical sphincterotomy with injection of botulinum toxin (BT). However, there is a lack of a consensus on the BT administration procedure among colorectal surgeons. METHODS: A national survey approved by the American Society of Colon and Rectal Surgeons (ASCRS) Executive Council was sent to all members. An eight-question survey was sent via ASCRS email correspondence between December 2019 and February 2020. Questio…
