[The oxidative DNA damage of monocytes and its in vitro repair by melatonin in patients with tuberculous pleurisy].

OBJECTIVE: To explore the level of mononuclear cell oxidative DNA damage and the total antioxidative capacity (TAC) in patients with tuberculous pleurisy. To evaluate the in vitro repair effects of melatonin on mononuclear cell DNA damage in pleural effusion of the patients. METHODS: The mononuclear cell DNA damages in pleural effusion and peripheral blood of 28 patients with tuberculous pleurisy and in peripheral blood of 25 healthy persons were detected by single cell gel electrophoresis (comet%). The levels of TAC in supernatant of pleural effusion and blood plasma of 28 patients and in bl

Monoclonal integration of HTLV-1 in pleural effusion cells in a seropositive patient with tuberculous pleuritis.

The pleural effusion of a 79-year-old man who was a carrier of human T-cell leukemia virus type 1 (HTLV-1) showed monoclonal growth of HTLV-1 infected T-cells. While the integration pattern of HTLV-1 was polyclonal in the peripheral blood mononuclear cells, it was monoclonal in the infiltrated cells of the pleural effusion. Morphologically atypical lymphoid cells were also found in the peripheral blood and in the pleural effusion. The pleural effusion disappeared after treatment with antituberculous drugs. The nature of the monoclonal proliferation of HTLV-1 infected T-lymphocytes is discussed

[Primary tuberculous pleurisy. The course of 3 untreated cases].

Three patients with pleurisy, all exposed to tuberculosis (TB) in their environment, are presented. The etiology of the pleurisy was not initially uncovered and as the pleurisy resolved spontaneously, the patients were discharged from follow up after four to 21 months. Within two to 12 months hereafter, the patients showed up with post-primary tuberculosis: (case I) with miliary tuberculosis incl. meningeal tuberculosis, case II with apical lung tuberculosis and case III with extrapulmonary tuberculosis of the bones. After six months of combined anti-tuberculosis chemotherapy all patients reco

Ten Questions on Using Lung Ultrasonography to Diagnose and Manage Pneumonia in the Hospital-at-Home Model: Part I-Techniques and Patterns.

The hospital-at-home (HaH) model delivers hospital-level acute care, including diagnostics, monitoring, and treatments, in a patient's home. It is particularly effective for managing conditions such as pneumonia. Point-of-care ultrasonography (PoCUS) is a key diagnostic tool in the HaH model, and it often serves as a substitute for imaging-based diagnosis in the HaH setting. Both standard and handheld ultrasound equipment are suitable for lung ultrasound (LUS) evaluation. Curvelinear and linear probes are typically used. Patient positioning depends on their clinical condition and specific diag

Cystatin F Depletion in Mycobacterium tuberculosis-Infected Macrophages Improves Cathepsin C/Granzyme B-Driven Cytotoxic Effects on HIV-Infected Cells during Coinfection.

Cystatin F (CstF) is a protease inhibitor of cysteine cathepsins, including those involved in activating the perforin/granzyme cytotoxic pathways. It is targeted at the endolysosomal pathway but can also be secreted to the extracellular milieu or endocytosed by bystander cells. CstF was shown to be significantly increased in tuberculous pleurisy, and during HIV coinfection, pleural fluids display high viral loads. In human macrophages, our previous results revealed a strong upregulation of CstF in phagocytes activated by interferon γ or after infection with Mycobacterium tuberculosis (Mtb). Cs

Clinical and imaging factors that can predict contagiousness of pulmonary tuberculosis.

BACKGROUND: Knowledge on predicting pulmonary tuberculosis (PTB) contagiosity in the hospital admission setting is limited. The objective was to assess clinical and radiological criteria to predict PTB contagiosity. METHODS: Retrospective analysis of 7 clinical, 4 chest X-ray (CXR) and 5 computed tomography (CT) signs in 299 PTB patients admitted to an urban tertiary hospital from 2008 to 2016. If the acid fact bacilli stain was positive (AFB+) on admission, the case was considered high contagiosity. RESULTS: Best predictors for high PTB contagiosity (AFB+) were haemoptysis (OR 4.33), cough

Cytokines and fibrinolytic enzymes in tuberculous and parapneumonic effusions.

Tuberculous (TB) pleurisy and parapneumonic effusion (PPE) are common causes of pleural fibrosis. The mechanisms underlying fibrin deposition may be different since involved inflammatory cells are distinct. In this study, we measured various cytokines and fibrinolytic enzymes and compared the differences between the two effusions. PPE was further divided into noncomplicated PPE and complicated PPE/empyema subgroups. Tumor necrosis factor (TNF)-alpha, interleukin (IL)-1beta, IL-6, IL-8, macrophage inflammatory protein (MIP)-1beta, monocyte chemoattractant protein (MCP)-1, plasminogen activator

A rare case of Demons-Meigs' syndrome with a 7.5 kg giant ovarian fibroma associated with severe dyspnea: case report.

INTRODUCTION AND IMPORTANCE: Demons-Meigs syndrome is a rare condition characterized by the presence of a benign fibroma of the ovary, ascites, and pleural effusion. It is very uncommon, and the diagnosis is made with difficulty based on symptoms that usually mimic disseminated malignancy or tuberculosis, but imaging may confirm the diagnosis. The definitive treatment is laparotomy, after which the symptoms resolve. CASE PRESENTATION: We present a 36-year-old female with Demons-Meigs' syndrome with severe dyspnea who underwent an abdominal surgical exploration, which revealed ascites of 1500

Pericardial Effusion due to Primary Malignant Pericardial Mesothelioma: A Common Finding but an Uncommon Cause.

This case report describes a 37-year-old female who was admitted to our Emergency Department because of shortness of breath. On physical examination, she had dyspnea and tachycardia and blood pressure was 80/50 mmHg with a pulsus paradoxus of 22 mmHg. Neck veins were distended, heart sounds were distant, and dullness was found on both lung bases. Her chest X-ray revealed bilateral pleural effusion and cardiomegaly. On both computed tomography and echocardiography the heart was of normal size and a large pericardial effusion was noted. The echocardiogram showed signs of impending tamponade, so

A rare cause of massive ascites: familial Mediterranean fever.

Familial Mediterranean fever is an autosomal recessive disease characterized by recurrent fever and peritoneal and pleural inflammation. It is an inherited disorder commonly found in Armenians, Turks, Arabs, Balkans, and Jews originating from North African countries. A small amount of peritoneal fluid collection can be observed during peritoneal attacks in patients with Familial Mediterranean fever, but chronic ascites has been described rarely in these patients. A 42-year-old female patient was admitted to our clinic in June 2010 with fever, severe abdominal pain and abdominal distention that
