Disseminated Skeletal Tuberculosis Presenting as Multifocal Axial and Peripheral Joint Disease: A Diagnostic Challenge.

Disseminated skeletal tuberculosis (TB) is an uncommon manifestation of extrapulmonary TB and often presents with non-specific musculoskeletal symptoms, contributing to diagnostic delay and an increased risk of irreversible structural damage. We report a 35-year-old male with diabetes mellitus who presented with two months of progressive nocturnal low back pain with functional limitation, followed by progressive painful swelling and restricted movement of the left elbow, and subsequently developed high-grade fever, weight loss, and constitutional symptoms. The coexistence of axial and peripher

Bronchial artery embolization in treatment of hemoptysis: Treatment efficacy and complications at a tertiary care chest centre.

BACKGROUND: Hemoptysis is one of the most alarming condition to both the patients suffering from it and the treating physicians. It is caused due to varied etiologies. One of the emergent and at times life-saving treatment option is by minimally invasive interventional radiological technique of Bronchial Atery Embolization (BAE). The authors aimed to carry out a retrospective analysis of short term efficacy and safety of all patients treated by this technique at a tertiary care thoracic centre. METHODS: A total of 52 patients were included in the study who had a median follow up of 35 days. A

Treatment of tuberculous spondylitis with anterior stabilization and titanium cage.

UNLABELLED: We retrospectively reviewed 12 patients with spinal tuberculosis of the thoracic and lumbar spine who had radical débridement, anterior decompression, interbody arthrodesis with an anterior interbody titanium cage, and autologous bone grafts, combined with a standardized perioperative antituberculous regimen. Their mean age was 55.1 years and they were observed for a mean of 65.3 months. Indications for surgery included epidural abscess, structural destruction with instability, progressive kyphosis, and/or neurologic deterioration. Kyphotic deformity was corrected from a mean of 24

Neurological complications of HIV infection.

More than half of HIV - infected persons develop symptomatic neurological disease. The nervous system is extensively involved with no part of the neuraxis being immune from the virus. Beisdes the brain and spinal cord, the peripheral nerves and muscles may be affected. Neurological complications typically occur with advanced disease and profound immunosuppression, hence a knowledge of the CD4 lymphocyte count is of paramount importance. Since many of the conditions are amenable to treatment, a proper diagnosis and therapy may decrease morbidity in the already curtailed life span of the patient

[New techniques in thoracic surgery. II].

The surgical approach to affections of the chest wall and pleura, still the predominant indications for thoracic surgery, has greatly changed since the advent of thoracoscopic procedures, and is emphasized in this second part of a two-part review, together with other indications for mediastinal tumours. Indicated after lung exeresis or emergency chest surgery, protective chest wall reconstruction with muscular flaps is no longer an exceptional operation. Inversely, thoracic surgery for infectious complications have become less frequent, unusually limited to well established procedures for tube

Common bacterial isolates, clinical outcome and TB meningitis in children admitted at Morogoro Regional Referral Hospital, Tanzania.

BACKGROUND: Bacterial meningitis is still one of the major causes of deaths, disabilities, and mental retardation in children in Morogoro region. To study the current meningitis burden, we evaluated the common bacterial isolates and clinical outcome of the disease in the region. METHODS: We conducted a hospital-based prospective study on 1352 children aged between 7 days and 12 years admitted in pediatric wards at Morogoro Regional Referral Hospital for 7 months. Cerebrospinal fluid (CSF) for laboratory microbiological examination was collected by lumbar puncture in 72 children with signs and

Intrathecal Isoniazid for Refractory Tuberculous Meningitis with Cerebral Infarction.

A 30-year-old Vietnamese woman, about 19 weeks pregnant, was admitted for acute cerebral infarction with stenosis of the left middle cerebral artery (LMCA), tuberculous meningitis, and miliary tuberculosis. Treatment with heparin, quadruple anti-tuberculosis therapy, and dexamethasone afforded prompt symptomatic improvement. However, she delivered a stillbirth, after which there was recurrence of acute cerebral infarction with LMCA occlusion, sinus thrombosis, and cranial base inflammation. A thrice-weekly 100 mg dose of intrathecal isoniazid (INH) improved the signs of meningeal inflammation.

Lumbar tuberculosis associated with membranous nephropathy and interstitial nephritis.

Tuberculosis is a common disease worldwide. However, it now is clear that tuberculosis can affect the kidney more insidiously. We describe a case of lumbar tuberculosis associated with simultaneous membranous nephropathy and interstitial nephritis, in which recovery of renal function occurred after treatment with steroids in addition to antituberculosis agents.

Minimally invasive lateral lumbar intervertebral fusion versus traditional anterior approach for localized lumbar tuberculosis: a matched-pair case control study.

BACKGROUND CONTEXT: Minimally invasive lateral lumbar intervertebral fusion (LLIF) procedure has been reported as a feasible alternative to the traditional anterior approach for patients with lumbar tuberculosis. However, there is still no study in the existing literature comparing LLIF to traditional surgeries in the treatment of such patients. PURPOSE: To evaluate the clinical, radiological, and functional outcomes of LLIF versus the traditional anterior approach for treating localized lumbar tuberculosis. STUDY DESIGN: Retrospective case-control study. PATIENT SAMPLE: A total of 60 patie

Pott disease mimics postsurgical pyogenic spondylodiscitis.

Postsurgical pyogenic spondylodiscitis is a well known complication of lumbar microdiscectomy. However; Pott disease after the early period of lumbar microdiscectomy has not been reported so far. Pott disease may mimic infectious complications of surgery and may be a reason of misdiagnosis. The author reports a 51 years old case nonresponsive to nonspecific antibiotic administration with diagnostic difficulty. To the author's knowledge this is the first Pott case that mimics postsurgical pyogenic spondylodiscitis.

[G. F. d'Acquapendente and Don Carlo Medici's gibbosity].

Don Carlo dei Medici (1595-1666) is the son of Ferdinando I (1549-1609), Granduca of Tuscany, and becomes Cardinal of Catholic Roman Church in 1615. In 1604 Fabrizio d'Acquapendente is called in Florence to treat him, because of an aggravation of his health, and of his congenital neck's gibbosity. The recent paleopathological researches have diagnosed his congenital cervical gibbosity as effect of the Klippel-Feil's syndrome, and characteristic lesions of tubercolosis.

Lymph node paradoxical enlargement during treatment for tuberculous spondylodiscitis (Pott's disease).

INTRODUCTION: Paradoxical reactions during adapted antituberculous treatment are defined as a transient, clinical and/or radiological enlargement of pre-existent lesions or appearance of new ones. OBSERVATION: We report herein the case of a 62-year-old woman who suffered under treatment of Pott's disease from an enlargement of her para-vertebral abscess and disco-vertebral lesions, and a tuberculous paradoxical reaction characterized by the development of uni- then bilateral cervical lymph nodes. Outcome was favourable after drainage by percutaneous punction of the adenopathies and oral short

[Immunity in patients with active tuberculous spondylitis].

Immunological parameters were studied in 45 patients with active tuberculous spondylitis, admitted for surgical treatment, who were divided into 2 groups in accordance with the prevalence of exudative-necrotic, or productive components of inflammation in the vertebral bodies and paravertebral tissues. The patients with a predominantly exudative-necrotic component of inflammation exhibited a severe clinical course with frequent neurological disorders, large abscesses in the paravertebral tissues, inflammatory changes in the leukogram, enhanced specific T-lymphocytic activity in the PPD blast-tr
