Chronic postpartum uterine inversion treated by abdominal replacement and cerclage.

BACKGROUND: Chronic uterine inversion is a potentially dangerous condition that often requires abdominal surgical reduction of the uterine inversion. Uterine inversion can recur after successful uterine replacement. CASE: A young primagravida presented with chronic recurrent uterine inversion 5 weeks after spontaneous vaginal delivery. The uterus was replaced abdominally and an abdominal cervical cerclage was placed to prevent recurrent inversion. CONCLUSION: Abdominal cervical cerclage can be utilized after treatment of chronic recurrent uterine inversion to prevent future recurrence of ute

Carotid and vertebral arterial fibromuscular dysplasia masquerading as severe preeclampsia: a case report.

BACKGROUND: Fibromuscular dysplasia is a non-atherosclerotic noninflammatory vascular disease that can affect any vascular bed. Dysplasia of the intima, media, or adventitia layer of the affected arteries causes stenosis and hypertension. Pregnant or post-partum patients may present with hypertension and be mistakenly diagnosed with preeclampsia. CASE: A 26 year old Gravida 1 Para 1 female 20 days post partum from a spontaneous vaginal delivery was transported from an outlying facility due to severely elevated blood pressure and transient left arm numbness and left sided facial droop. Upon ar

Bouveret syndrome: a case report.

Bouveret syndrome is characterized by the migration of a gallstone through a cholecystenteric fistula into the proximal duodenum resulting in gastric outlet obstruction. Bouveret syndrome is a rare phenomenon that most commonly occurs in females and the elderly. A 79-year-old female presented with symptoms of gastric outlet obstruction and was diagnosed with Bouveret syndrome. This report describes the symptoms, diagnosis, and management of Bouveret syndrome, as well as its prevalence and differentiation from gallstone ileus. Patients with Bouveret syndrome present with varied, non-specific sy

Superior mesenteric artery aneurysm: an uncommon disease with potentially serious complications.

Superior mesenteric artery aneurysms (SMAAs) are visceral arterial aneurysms that can result from a variety of conditions. About half of SMAAs are mycotic and occur subsequent to infective endocarditis. The clinical presentation of SMAA is nonspecific, and some patients may be asymptomatic while others may report mild to severe abdominal pain. Herein, we present a case of a 53-year-old man who presented to the emergency department with abdominal pain 5 months after receiving medical treatment for infective endocarditis. CT scan demonstrated an aneurysm in the superior mesenteric artery and a s

Leptospirosis induced pulmonary alveolar hemorrhage.

Leptospirosis, a zoonosis with protean manifestations caused by the spirochete Leptospira interrogans, occurs worldwide. We report a case of leptospirosis presenting as pulmonary alveolar hemorrhage in a young immunocompetent male. This organism should be considered as an important infectious cause of pulmonary alveolar hemorrhage in the appropriate epidemiological setting.

Pheochromocytoma: an easily overlooked cause of secondary hypertension.

Pheochromocytoma is a rare cause of secondary hypertension. Presentation varies from asymptomatic to paroxysmal hypertension, palpitations, flushing, syncope, and even shock or death. Diagnosis is often delayed because pheochromocytoma is not considered a likely diagnosis. We present the case of a 47-year-old male that presented with hypertensive emergency. Several diagnostic tests were performed prior to consideration of pheochromocytoma as the potential cause. Abdominal computed tomography revealed a nonspecific mass. A subsequent hormonal workup led to the diagnosis of pheochromocytoma. The

Apocrine adenoma of the breast.

A case of apocrine adenoma, a rare benign tumor of the breast is reported. A left breast nodule from a 47-year-old female was found by mammography and excised. The tumor was well circumscribed and composed of tightly packed tubules lined by cells with apocrine features. There has been no recurrence in two years. The few previously reported cases are summarized. Apocrine adenomas have been reported arising in a variety of sites including the breast, (1-5) perianal region, (6,7) eyelid, (8) and axilla. (9) Their appearance has similarities to the apocrine metaplasia commonly seen in fibrocystic

Adult diagnosed Dandy Walker malformation presenting as an acute brainstem event--a case report and review of the literature.

The Dandy Walker Malformation (DWM) is an infrequent condition seen in pediatric patients. Adult presentation of DWM is extremely rare. This condition usually presents in childhood with hydrocephalus and cerebellar signs and symptoms. This case describes a woman with an undiagnosed DWM who was asymptomatic until the age of 56 when she developed the acute onset of headache, nausea, vomiting, and diplopia. Her history and physical exam were consistent with an acute brainstem infarct. MRI revealed the underlying malformation. The clinical and radiological findings are discussed as well as their i

Insulin-mediated pseudoacromegaly: a case report and review of the literature.

A 23 year old female patient presented with oligoamenorrhea. She had excessive weight gain and had noticed hirsutism, enlargement of the jaw, increase in her ring and shoe size, increased sweating and darkening of her skin in flexural areas. Examination revealed a large framed woman with coarse facial features, large hands and feet, prognathism, acanthosis nigricans, hirsutism, acne and many skin tags. GH and IGF-1 were normal. MRI of pituitary showed a 7mm microadenoma, believed to be non-secretory with normal pituitary hormonal workup. She had marked elevation of serum insulin, elevated test

Sexual dysfunction in patients with painful bladder syndrome.

PURPOSE: The degree of sexual dysfunction in patients with Painful Bladder Syndrome (PBS) has not been documented previously. The Female Sexual Function Index (FSFI) was used to measure the degree of female sexual dysfunction (FSD) in these patients. MATERIALS AND METHODS: The FSFI was administered on-line to female patients with self-reported PBS. This 19-item questionnaire evaluated FSD in six domains: desire, arousal, lubrication, orgasm, satisfaction, and pain. RESULTS: The data was analyzed on an item-for-item basis and by the six domains of sexual dysfunction for 100 patients and compa

Sex, smell and vasculitis: Henoch-Schönlein purpura in Kallmann's syndrome.

Kallmann's syndrome (KS) is the most common cause of isolated hypogonadotropic hypogonadism. An increased prevalence of rheumatic and autoimmune diseases has been noted in patients with hypogonadism including Kallmann's syndrome. Both in vitro and in vivo studies indicate that testosterone deficiency may promote inflammatory response such as vasculitis by altering inmmunoglobulin and cytokines profiles. We report the novel occurrence of Henoch-Schönlein purpura in a patient with markedly low testosterone level due to Kallmann's syndrome. We discuss the potential mechanisms by which hypoandroge

Ureteropelvic junction obstruction in association with autosomal dominant polycystic kidney disease: a case report in a trauma patient.

Autosomal dominant polycystic kidney disease (ADPKD) is a common cause of cystic renal disease and can predispose to renal insufficiency. Although typically diagnosed and discussed in the pediatric population, ureteropelvic junction obstruction may also make patients prone to renal dysfunction and failure. These two disease processes can coexist in the same patient, which may, in turn, complicate their management. We present the case of an adult patient with known ADPKD who suffered a traumatic fall and subsequent retroperitoneal bleed, and was also diagnosed with a UPJ obstruction of the left

Microbiology of salt rising bread.

Salt rising bread (SRB) is an Appalachian traditional bread made without yeast, using a starter derived from flour, milk and potatoes. The "rising agent" has been identified as Clostridium perfringens, not salt, and is presumably derived from the environment. Although no cases of illness have been attributed to SRB, C. perfringens type A is a common cause of food poisoning from meats and gravies. Other C. perfringens isolates may cause enteritis necroticans (pig-bel disease) and gas gangrene. Past research documents that pathogenic strains derived from wounds may be used to produce bread and t

Steptococcus bovis meningitis and sepsis associated with Strongyloidiasis in an immunocompetent patient.

We report the case of a 69-year-old female who presented with headache, stiff neck, and decreased level of consciousness. Lumbar puncture results were typical of bacterial meningitis. Blood and cerebrospinal fluid cultures showed Streptococcus bovis. Subsequent serologic studies indicated concurrent Strongyloides stercoralis infection, and larvae were visualized in two separate stool specimens. The patient responded to treatment of both infections. She refused to undergo colonoscopy despite a known association between Streptococcus bovis and colonic carcinoma.

Acute pancreatitis: management update and future directions of pancreatic imaging.

Essential management points for AP are: 1. Intravenous fluids and adequate hydration are the mainstay of therapy. Antibiotics are not routinely indicated in most cases. 2. The most reliable marker for diagnosing biliary AP is a greater than threefold elevation of the serum alanine amniotrasferase (ALT), which has a positive predictive value of 95% for biliary acute pancreatitis. 3. Early ERCP with ES for stone extraction and biliary decompression has proved beneficial for patients with biliary pancreatitis and evidence of persistent or progressive biliary obstruction with elevated serum total

Diagnostic pitfall of computed tomography in patients with superficial siderosis of the central nervous system.

Superficial siderosis of the central nervous system (SSCN) is a well-described entity with distinct clinical presentation as well as computed tomography (CT) and magnetic resonance imaging (MRI) findings. However, it is critical that when a patient previously diagnosed with SSCN undergoes CT on the brain at a later date, that this scan not be misinterpreted as a new subarachnoid hemorrhage (SAH) so unecessary repeated angiograms are not performed. This report describes such a situation and discusses unique CT findings in SSCN that have been under-recognized and under-emphasized. While conditio

A diagnostic dilemma: case study of a 36-year-old female with alpha-1-antitrypsin deficiency.

Alpha-1-antitrypsin deficiency is a genetic disorder that presents as early-onset emphysema in its most severe form. A high index of clinical suspicion is needed in cases of lung or liver disease of unknown etiology or a suggestive history and physical examination. Low or absent serum levels of alpha-1-antitrypsin levels identify persons with the disease and phenotyping is the confirmatory test. The main goal of management is attempting to prevent or slowing the progression of damage to the lungs. Medical and surgical options for treatment include augmentation therapy that maintains protective
