Bicycle Handlebar-Related Blunt Abdominal and Pelvic Injuries in Children and Adults: A Radiological Perspective.

OBJECTIVE: Bicycle handlebar-related abdominopelvic trauma can cause clinically significant injuries despite minimal external signs, potentially resulting in delayed diagnosis. Radiological evidence regarding CT injury patterns associated with this mechanism remains limited. This study aimed to evaluate and characterize the CT findings of bicycle handlebar-related blunt abdominal and pelvic injuries in pediatric and adult patients and to describe injury patterns, organ involvement, and clinical management. MATERIALS AND METHODS: This retrospective study included patients who presented with bi

Global Disparity Within the Management of Traumatic Abdominal Vascular Injuries: Analysis of a Global Dataset.

BACKGROUND: Vascular injuries are common to trauma and carry a high risk to limb and life. Integrating specialist vascular care into existing trauma systems is essential; however, global data on this topic remains limited. This study describes global patterns of abdominal vascular trauma, through secondary analysis of the Global Outcomes After Laparotomy for Trauma (GOAL-Trauma) study dataset, examining system- and patient-level differences across resource settings. METHODS: Patients of any age with blunt or penetrating trauma who underwent an abdominal vascular procedure during trauma laparo

Early Versus Late Venous Thromboembolism Prophylaxis Impact on Outcomes of Blunt Solid Abdominal Organ Injuries.

BACKGROUND: Venous thromboembolism (VTE) following solid, blunt, isolated abdominal organ injuries remains a common posttrauma complication in adults. Accordingly, gaining a better understanding of the effect of early versus late VTE prophylaxis to reduce rates of this complication is prudent. OBJECTIVE: The purpose of this study is to evaluate the impact of early versus late VTE prophylaxis on clinical outcomes in adult trauma patients with isolated blunt solid abdominal organ injuries. METHODS: This retrospective cohort study utilized the American College of Surgeons Trauma Quality Improve

Rupture of the abdominal aorta artery due to self-inflicted injuries in a young man.

INTRODUCTION AND IMPORTANCE: Intentional drug poisoning is the most common method of suicide in the population. Self-inflicted injuries are usually limited to parts of the body that are easily accessible with one's hand, such as the neck, thorax, or upper extremities. Penetrating aortic trauma has a high mortality rate, and open surgery is frequently required. CASE PRESENTATION: We report a 19-year-old man who stabbed himself in the anterior abdomen with a kitchen knife after ingesting numerous benzodiazepine tablets, resulting in a rupture of intra-abdominal viscera, including the abdominal

A wearable microwave instrument can detect and monitor traumatic abdominal injuries in a porcine model.

Abdominal injury is a frequent cause of death for trauma patients, and early recognition is essential to limit fatalities. There is a need for a wearable sensor system for prehospital settings that can detect and monitor bleeding in the abdomen (hemoperitoneum). This study evaluates the potential for microwave technology to fill that gap. A simple prototype of a wearable microwave sensor was constructed using eight antennas. A realistic porcine model of hemoperitoneum was developed using anesthetized pigs. Ten animals were measured at healthy state and at two sizes of bleeding. Statistical tes

Management of Seat Belt-type Blunt Abdominal Aortic Trauma and Associated Injuries in Pediatric Patients.

BACKGROUND: "Seat belt-type" pediatric abdominal aortic trauma is uncommon but potentially lethal. During high speed motor vehicle collisions (MVCs), seat or lap belt restraints may concentrate forces in a band-like pattern across the abdomen, resulting in the triad of hollow viscus perforation, spine fracture, and aortoiliac injury. We report 4 cases of pediatric seat belt-type aortic trauma and review management strategies for the aortic disruption and the associated constellation of injuries. METHODS: -approved, retrospective review of all pediatric patients requiring surgical intervention

Costal Margin Tenderness and the Risk for Intraabdominal Injuries in Children With Blunt Abdominal Trauma.

BACKGROUND: The risk of radiation exposure from computed tomography (CT) imaging in children is well recognized. Patient history and physical examination findings, including costal margin tenderness (CMT), influence a physician's decision to image a child with blunt torso trauma. The objective of this study was to determine the importance of CMT for identifying children with intraabdominal injuries (IAI) found on CT and IAI undergoing acute intervention. METHODS: We conducted an analysis of the Pediatric Emergency Care Applied Research Network (PECARN) IAI public use data set, representing a …

Complications and risk factors for mortality in penetrating abdominal firearm injuries: analysis of 120 cases.

Due to the high kinetic energy, of bullets and explosive gun particles, their paths through the abdomen (permanent cavity effect), and the blast effect (temporary cavity effect), firearm injuries (FAI) can produce damage not only in the organ they enter, but in the surrounding tissues as well. Since they change route after entering the body they may cause organ damage in locations other than those at the path of entry. For example, as a result of the crushing onto bone tissues, bullet particles or broken bone fragments may cause further damage outside of the path of travel, For these reasons i

The value of endoscopic diagnosis and the treatment of pancreas injuries following blunt abdominal trauma.

BACKGROUND: Injuries to the pancreas following blunt abdominal trauma are rare due to its protected retroperitoneal position. Many pancreatic lesions remain unnoticed at first and only become apparent when complications arise or during treatment of other injuries. The mortality rate is between 12 and 30%, and if treatment is delayed it is as high as 60%. METHODS: Using medical records over the past 5 years, we investigated when and in what circumstances endoscopic retrograde cholangiopancreaticography (ERCP) was used in the diagnosis and treatment of pancreas injuries after blunt abdominal tr

Prediction of the delayed complications of intestinal and mesenteric injuries following experimental blunt abdominal trauma.

Injuries to the intestine and mesentery are often found in patients undergoing laparotomy for blunt abdominal trauma. Although treatment of perforations is relatively straightforward, the same is not true for contusions. Few guidelines exist at present to aid the surgeon in deciding which injuries require resection in order to avoid the complications of delayed perforation and late stricture formation. The natural history of these non-perforating intestinal and mesenteric injuries has been examined in an experimental model to identify possible criteria on which future management can be based.

Solid viscus injury predicts major hollow viscus injury in blunt abdominal trauma.

BACKGROUND: As nonoperative management of blunt abdominal trauma has become more popular, reliable models for predicting the likelihood of concomitant hollow viscus injury in the hemodynamically stable patient with a solid viscus injury are increasingly important. METHODS: The Pennsylvania Trauma Systems Foundation registry was reviewed for the period from January 1992 to December 1995 for all adult (age > 12 years) patients with blunt trauma and an Abbreviated Injury Scale (AIS) score > or = 2 for a solid viscus (kidney, liver, pancreas, spleen). Patients with an initial systolic blood press

The Impact of Prior Laparotomy and Intra-abdominal Adhesions on Bowel and Mesenteric Injury Following Blunt Abdominal Trauma.

BACKGROUND: Early recognition of bowel and mesenteric injury following blunt abdominal trauma remains difficult. We hypothesized that patients with intra-abdominal adhesions from prior laparotomy would be subjected to visceral sheering deceleration forces and increased risk for bowel and mesenteric injury following blunt abdominal trauma. METHODS: We performed a multicenter retrospective cohort analysis of 267 consecutive adult trauma patients who underwent operative exploration following moderate-critical (abdominal injury score 2-5) blunt abdominal trauma, comparing patients with prior lapa

Rate of intra-abdominal injury after a normal abdominal computed tomographic scan in adults with blunt trauma.

OBJECTIVE: The objective of this study is to determine the rate of intra-abdominal injury (IAI) in adults with blunt abdominal trauma after a normal abdominal computed tomographic (CT) scan. We hypothesize that the risk of subsequent IAI is so low that hospital admission and observation for possible IAI are unnecessary. METHODS: We conducted a prospective, observational cohort study of adults (>18 years) with blunt trauma who underwent abdominal CT scanning in the emergency department. Computed tomographic scans were obtained with intravenous contrast but no oral contrast. Abnormalities on ab

Penetrating Abdominal Injuries: A 10-Year Retrospective Analysis of 49 Patients at a German Trauma Center.

BACKGROUND Penetrating abdominal injuries are rare in central Europe; therefore, most trauma surgeons have little experience in the treatment of these injuries. The optimal management of penetrating abdominal injuries remains debated. While immediate surgery is standard for unstable patients or those with peritonitis, bowel evisceration, or gunshot wounds, the treatment of stable patients is still controversial. This study aimed to assess the reliability of preoperative diagnostics compared to intraoperative findings. MATERIAL AND METHODS Forty-nine patients with penetrating abdominal injuries…

Patterns and outcomes of abdominal wound injuries in Sudanese armed conflict survivors: a retrospective cohort study.

BACKGROUND AND AIM: On April 15, 2023, the people of Khartoum were woken by the drums of war. The armed conflict resulted in >1200 reported fatalities and over 12 000 injuries. The abdomen, housing vital organs and lacking protective bony structures, is highly vulnerable in such settings. Understanding abdominal wound injuries (AWIs) is essential for improving management and outcomes. This study aimed to analyze the patterns and outcomes of AWIs among survivors of the Sudanese armed conflict. METHODS: A retrospective cohort study was conducted at Wad Madni Teaching Hospital, Gezira State, Sud
