The translabyrinthine approach has been popularized during the past 30 years for the surgical treatment of acoustic neuromas. It serves as an alternative to the retrosigmoid approach in patients when hearing preservation is not a primary consideration. Patients with a tumor of any size may be treated by the translabyrinthine approach. The corridor of access to the cerebellopontine angle is shifted anteriorly in contrast to the retrosigmoid approach, resulting in minimized retraction of the cerebellum. Successful use of the approach relies on a number of technical nuances that are outlined in t

The objectives of this study were to assess the effect that acoustic neuroma surgery has on tinnitus and to investigate possible predictors (tumour size and patients' ages at operation) as well as to ascertain if the overall quality of life in patients with acoustic neuromas is affected by their tinnitus. A questionnaire was sent to randomly selected patients post acoustic-neuroma surgery. This was based on the Glasgow Benefit Inventory and contained a standardised series of four functional gradings for tinnitus. In this study, 51 patients from a total of 68 returned the questionnaire, and the

The diagnosis of acoustic neuroma is usually evoked in a patient presenting with a long history of hearing disturbance in whom an enhancing lesion within the internal auditory canal and/or the cerebellopontine angle is found on MRI. Hypervascularity with arteriovenous shunting and early filling of enlarged veins is a common feature of malignancy and has been reported very rarely in benign acoustic neuroma. We present the case of a patient without hearing disturbance, who showed a highly vascular lesion with no component in the internal auditory canal, making the preoperative diagnosis of acous

Two hundred and seventy-one Japanese patients who underwent surgical removal of neuroma from the cerebello-pontine angle using the translabyrinthine approach were retrospectively reviewed in order to investigate the nerve origin on an anatomical basis. In 269 out of the 271 cases, the origin of the neuromas was identified. Among these cases, 84.8 per cent of tumours originated from the inferior vestibular nerve (IVN), followed by the superior vestibular nerve (SVN) (8.9 per cent). Of the 5.6 per cent of tumours arising from the vestibular nerve, however, it could not be determined whether they

This article describes surgical methods developed during my resection of 600 acoustic neuromas, 599 of which were total removals. This series began in 1965, with the introduction of the surgical microscope and the development of microsurgical technique. In the 1960s, there was no established technology for the microsurgery of acoustic neuromas. Much of the work of the great early neurosurgeons--Cushing, Dandy, Olivecrona, and other remarkable pioneers--did not really apply. New approaches, solutions, and tools were introduced at that time, and there was a long, steep learning curve. I hope tha

Frontal dementia is a clinical entity of cognitive impairment, characterized mostly by progressive loss of fluency in speech, eventually resulting in aphasia or anomia, associated frequently with early loss of insight and many forms of inappropriate behavior. Hyperphosphorylation of the isoforms of tau protein, a microtubule-associated protein, which plays an important role in the pathogenetic mechanisms of Alzheimer's disease, is mainly involved in the pathogenesis of frontal dementia. In the present study, the morphological alterations of the acoustic cortex are described in three cases of d

OBJECTIVE: To determine spontaneous involution of unilateral acoustic tumors in untreated patients.
STUDY DESIGN: Outcome of a continuous study, 1982 to present.
SETTING: Private tertiary otology/neurotology referral center.
PATIENTS: Patients with unilateral acoustic tumors for whom interval imaging was selected rather than surgery or gamma knife radiation, 1982 to the present.
INTERVENTION: Interval imaging with computerized axial tomography and/or magnetic resonance imaging.
MAIN OUTCOME MEASURE: Spontaneous involution of acoustic tumors.
RESULTS: Forty-seven patients with unilateral

This paper discusses surgery and radiosurgery from the author's personal experience with both techniques. Multi-discipline assessment will allow the patient to make an informed decision regarding preferred management.

Epithelioid schwannomas occur predominantly in relation to peripheral nerves and are associated with histological and clinical malignancy. However, a variant of the epithelioid schwannoma involving cranial nerves is extremely rare. In this study we report three cases of epithelioid schwannomas originating from the acoustic nerves and located in the cerebello-pontine angles. In the first case, the tumor was histopathologically entirely solid and demonstrated biphasic pattern with both spindle-shaped cells and a population of round or polygonal epithelioid cells. The second one consisted of the

Our objective is to report 4 cases of incidentally discovered acoustic neuromas (ANs) and to determine the incidence of asymptomatic ANs. A prospective study of 161 consecutive patients undergoing gadolinium-enhanced MRI (Gd-MRI) at a tertiary-care university-affiliated medical center was carried out from September 1994 to April 1995. The Gd-MRI scans were performed for tentative diagnoses other than AN or sensorineural hearing loss. In 161 consecutive patients examined, no ANs were found incidentally on Gd-MRI scans. Previous studies have suggested that the incidence of occult ANs is as high

Although many studies have been performed on the effects of acoustic overstimulation on the inner ear, our knowledge about the cellular processes underlying reduced hearing sensitivity and auditory cell death is still limited. In order to further our understanding of cellular processes occurring in conjunction with acoustic trauma, we designed an in vitro model to study the effects of overstimulation directly on sensory hair cells isolated from the low-frequency part of the guinea pig cochlea. The isolated outer hair cells were subjected to pressure jets delivered by a glass micropipette posit

OBJECTIVE: This study aimed to define the incidence and rate of tumor growth of acoustic neuromas (ANs) in patients who have undergone radiologic surveillance.
DATA SOURCES: MEDLINE literature searches covering the period of January 1966-June 1997 were performed as well as a review of the bibliographies of the studies that were found.
STUDY SELECTION: Criteria for inclusion of a study in this metaanalysis were: a defined group of patients diagnosed with an AN for whom radiologic surveillance was the selected strategy of management, limited inclusion of patients with neurofibromatosis type II

Three cases of unilateral acoustic neuroma in childhood that are associated with neither neurofibromatosis type 1 nor type 2 were reported. All three cases had a hearing disorder as an initial symptom. Two of them had a large neuroma and had considerable abnormal findings in neurootological examinations, and one case with an intracanalicular tumor showed a unilateral progressive sensorineural hearing loss that had no response to steroid administration. Surgical removal of the tumor was carried out for these cases. Different approaches were used in each case; suboccipital approach, one-stage su

We present our experience with 83 patients with acoustic neurinomas (January 1988 to November 1996); 81 patients underwent surgery and 2 patients with intracanalicular neurinomas received conservative therapy due to their advanced age (1 case) and patient's option (1 case). The surgical approach was retrosigmoid/transmeatal and the goal was total removal in one procedure with preservation of facial and cochlear nerves. Radical removal of lesion was attempted in 79 cases and it was possible in 77 patients. Subtotal tumor resection was planned in 2 cases. Facial nerve function (grades I to III,
