BACKGROUND: Adverse childhood experiences (ACEs) exert transdiagnostic influences on disorders of the affective-to-psychotic spectrum which share genetic similarities. While ACEs are established risk factors for developing disorders across this spectrum, the impact of ACEs on functional outcomes in adulthood remains insufficiently understood. This study aims to investigate the impact of ACEs on functional outcomes such as relationship and employment stability.
METHODS: We used multistate Markov models to analyze data from the longitudinal PsyCourse Study (N = 810) to investigate whether ACEs

OBJECTIVE: To compare the skeletal status of subjects with primary psychotic disorders with the general population by means of bone ultrasound measurements. Schizophrenia seems to be associated with low bone mineral density through a still unclear mechanism, although information on other psychotic disorders is scarce.
METHODS: In a nationally representative sample, quantitative ultrasound values of the heel, i.e., broadband ultrasound attenuation (BUA) and speed of sound, were measured from subjects with schizophrenia (n = 48), other nonaffective psychosis (n = 56), affective psychosis (n = 3

The complexity of subtyping depression and the implications that such subtyping has on treatment choices are discussed in this article. The most recent edition of the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) directs clinicians to classify the mood disorders in depressed patients as unipolar, bipolar, due to a general medical condition, or due to substance abuse. The focus of this article is unipolar (major depression and dysthymia) and bipolar I and II disorders with and without feature specifiers for atypical depression, seasonal affective disorder, psych

Analyses were undertaken to examine whether a wide range of psychiatric disorders, including anxiety, affective, substance abuse and psychotic disorders, represent variant manifestations of Tourette's syndrome (TS). Previous studies have suggested that chronic tics (CT) and obsessive-compulsive disorder (OCD) are variant expressions of TS since both CT and OCD are elevated among relatives of TS probands. In the current study, no other psychiatric disorder was significantly elevated among the relatives who did not have TS, CT or OCD when compared with a control sample. These findings are not co

AimsTo identify clinical and other factors associated with the use of electroconvulsive therapy (ECT) in New South Wales for the period 1944-1949 and to compare with contemporaneous practice.MethodAnnual reports of the Inspector-General of Mental Hospitals in NSW (1944/45 to 1948/49) were examined.Main findingsSeven hospitals reported a total of 8964 courses of treatment during the period. Demographically, 60.6% of patients who received ECT were female, and rural hospitals reported 13.6% of courses. ECT was as likely to be given for non-affective psychoses (37.7%), as for affective psychoses (

There is a now a substantial body of evidence that suggests the new antipsychotic agent, risperidone, may be safe and effective for treating psychotic, affective or behavioural symptoms associated with various disorders other than schizophrenia, schizophreniform disorder or schizo-affective disorder. These conditions include bipolar disorder, obsessive-compulsive disorder, Tourette's syndrome, dementia, Lewy body disease, mental retardation, Parkinson's disease, idiopathic segmental dystonia and organic catatonia. Although much of the data is anecdotal or in the form of open studies, there is

BACKGROUND: Psychiatric disorders during pregnancy are common. Electroconvulsive therapy (ECT) can be indicated in severely affective or psychotic disorders with the necessity of a rapid response. Currently available review articles greatly differ in the methodology, leading to divergent conclusions concerning the use of ECT during pregnancy.
OBJECTIVE: Description of a new clinical case and interdisciplinary treatment suggestions for the safe application of ECT in pregnancy.
METHODS: Clinical case report and selective review of the literature with special consideration of existing systemati

BACKGROUND: The co-operation between psychiatrists in Norway and Russia is increasing. The object of this study was to find out whether there were differences in diagnostic practice of psychiatrists in both countries, to look at the nature of the differences and to examine whether these differences affected diagnostic quality.
METHOD: Thirty medical doctors working at psychiatric hospitals in both countries diagnosed 12 clinical case vignettes selected from a wide spectre of psychiatric disorders.
RESULTS: The Russian clinicians used a larger range of diagnoses than the Norwegians. The Russi

Widespread illicit anabolic steroid use has recently been reported. A review of available evidence suggests that elevations of serum levels of steroid hormones, including anabolic steroids, have profound psychological effects. Long-term, high-dose anabolic steroid use may lead to a preoccupation with drug use, difficulty stopping despite psychological side effects, and drug craving. Reductions in serum levels of steroid hormones appear to result in acute hyperadrenergic withdrawal symptoms that respond to steroid replacement or to agents that also ameliorate withdrawal symptoms in alcohol and

Psychiatric diagnosing in mentally retarded patients is notoriously difficult and routine application of current taxonomies is of very limited use. Although psychotic disorders in general can be satisfactory grouped on a descriptive level, the aetiology is most probably very heterogeneous. In this case report a female patient is described who presented with mild mental retardation and recurrent affective psychotic episodes. Chromosome analysis showed a female karyotype with a de novo translocation (2;10)(p23;q22.1). Biochemical evaluation demonstrated a persistently increased taurine and decre

BACKGROUND: A range of traumas have been linked to voices (auditory verbal hallucinations) and unusual perceptual experiences (UPEs) in other perceptual-sensory domains. Models of PTSD and positive symptoms of psychosis are insufficient in explaining the relationship between trauma and voices. The trauma-related voices (TRV) model was developed to generate novel research in this area.
AIMS: This study aimed to investigate pathways from trauma to the frequency of UPEs based on a subset of hypothesised relationships in the TRV model.
MATERIALS: The PTSD Diagnostic Scale for DSM-5, State Adult

Kleine-Levin syndrome is a characterized by a periodic hypersomnia crisis as well as dietary and variable significant psychic symptoms. Its rarity makes its definition and study complex. Many etiologies have been considered, especially regarding a hypothalamic dysfunction. Physiological, radiologic, anatomical and polysomnographic studies have not shown clearly conclusive results, but the syndrome appears to be related to affective disorders. This hypothesis is supported by the findings of a positive therapeutic response to lithium. However, we are interested in the related psychological facto

The authors examined a sample of 111 consecutively admitted patients who satisfied inclusion criteria for mania and further characterized them as exhibiting none, one, or two or more of five clinical features often believed to be associated with a diagnosis of schizophrenia: formal thought disorder, first-rank symptoms, auditory hallucinations, persecutory delusions, and catatonia. The presence and number of such symptoms were unrelated to any of the major demographic, clinical, historical, laboratory, or familial variables studied. The authors conclude that schizophrenic symptoms do not play

This article provides an overview of the main changes in the chapter "Schizophrenia Spectrum and Other Psychotic Disorders" from DSM-IV-TR to DSM-5, which, once again, does not make allowance for potential characteristics of children and adolescents. Changes in the main text include abandoning the classical subtypes of Schizophrenia as well as of the special significance of Schneider's first-rank symptoms, resulting in the general requirement of two key features (one having to be a positive symptom) in the definition of Schizophrenia and the allowance for bizarre contents in Delusional Disorde

Human immunodeficiency virus type 1 (HIV-1) infection is associated with psychiatric complications including cognitive impairment, affective disorders, and psychosis. Previous studies have revealed a disturbed kynurenine metabolism in these patients leading to increased levels of neuroactive compounds acting at glutamatergic neurotransmission. Kynurenic acid (KYNA), one of these metabolites is a glutamate-receptor antagonist, preferentially blocking the glycine site of the N-methyl-d-aspartate (NMDA) receptor. Increased levels of brain KYNA have been suggested to induce a NMDA receptor hypofun

IMPORTANCE: A growing body of research suggests that exposure to fine particulate matter (PM2.5; particle size 2.5 microns or smaller) may be associated with mental health outcomes. However, the potential impact of wildfire-specific PM2.5 exposure on mental health remains underexplored.
OBJECTIVE: To investigate whether wildfire-specific PM2.5 exposure may be associated with emergency department (ED) visits for mental health conditions, including all-cause and for psychoactive substance use, nonmood psychotic disorders, anxiety, depression, and other mood-affective disorders during the extens

OBJECTIVES: Suicide is an important clinical problem in psychiatric patients. The highest risk of suicide attempts is noted in affective disorders. In this study we tested 20 factors described in the literature (sociodemographic and clinical factors as well as family burden) in association with suicidal behavior and we analyzed whether the significance of those factors differs between males and females.
METHODS: In the study we included patients with major depressive disorder (MDD; n = 249) and bipolar affective disorder (BP; n = 582). The Structured Clinical Interview for DSM-IV Axis I (SCID

BACKGROUND: Women with triple X syndrome (TXS) have an extra X chromosome. TXS appeared to be associated with psychiatric disorders in biased or underpowered studies.
AIM: This study aims to describe the prevalence of psychiatric disorders in adults with TXS in a relatively large and less biased group of participants.
METHOD: In this cross-sectional study, data were collected from 34 women with TXS (mean age = 32.9; s.d. = 13.1) and 31 controls (mean age = 34.9; s.d. = 13.7). Psychiatric disorders were assessed using the MINI International Neuropsychiatric Interview (MINI) and the adult beha

BACKGROUND: People with psychotic disorders have greater mortality rates following cancer diagnosis, compared to people without psychotic disorders. Prior examinations of the effect of psychotic disorders on survival following cancer diagnosis mediated by stage at diagnosis and treatment disparities did not accommodate for multiple mediators and post-exposure confounding. The present study sought to estimate analogues of the natural indirect effects of having NAPD on mortality following cancer diagnosis, mediated through stage at diagnosis and time to treatment initiation.
